Actinomycosis is an uncommon entity caused by an anaerobic bacterium, Actinomyces species, most often Actinomyces israelii. We present a patient who suffered from progressive lumbar pain and high fever. Actinomycosis infection was diagnosed after extensive radiological and pathological evaluation. Treatment with i.v. ampicillin was started, followed by oral antibiotics for one year. The radiographic features of a unique autosomal dominant bone dysplasia are presented. The features are classified as generalised and/or focal. Generalised features are either altered trabecular pattern or modelling abnormalities. Twenty-eight knees in 26 patients underwent revision TKA requiring surgical management of major osteolytic defects. Three groups of osteolytic defects were identified based upon the degree of implant stability and the magnitude of bone loss. Outcome measures included the Knee Society Clinical Rating Score (KSCRS), visual analog pain score, and radiographs. Erythema elevatum diutinum (EED) is a rare chronic skin disease with cutaneous vasculitis, characterized by the absence of systemic vasculopathy. We describe a 79-year-old white woman who has been followed with the diagnosis of EED for 27 years. Our patient has a previously unreported combination of EED and acro-osteolysis. After reviewing possible reasons for the association, her vasculitis seems the most likely factor.
