Osteonecrosis after arthroscopic medial meniscectomy.

Osteonecrosis of the medial femoral condyle is an uncommon complication of meniscal surgery. Whether this disease is associated with the original medial meniscus lesion or with the arthroscopic procedure is not yet known. We present two cases in which osteonecrosis has developed shortly after arthroscopy was performed for medial meniscus pathology. Magnetic resonance imaging has been the best tool to diagnose and follow up the changes in the involved area. Concerning the etiology, the authors raise the question of whether the medial compartment overloading observed in both of these cases had a…

Osteonecrosis associated with HIV infection.

OBJECTIVE: To report cases of osteonecrosis in patients who were HIV positive but had no apparent risk factors for osteonecrosis. PATIENTS AND METHODS: A computer search of records from two university-affiliated teaching hospitals yielded eight cases of osteonecrosis in HIV-positive patients and patients with AIDS. The patients' charts and radiographic records were reviewed to determine possible risk factors for osteonecrosis. RESULTS: In four of the eight patients no definite risk factors for osteonecrosis could be found. CONCLUSION: Osteonecrosis may be a musculoskeletal complication of H…

[Osteonecrosis, metastases and bronchial carcinoid].

The authors report the case of a 35-year-old woman undergoing surgery in 1976 for a bronchial carcinoid tumour, considered to be benign. Twelve years later, diagnostic evaluation of liver metastases led to the discovery of an asymptomatic lesion of D10, the CT scan and NMR appearances of which were suggestive of osteonecrosis, a diagnosis confirmed histologically. Investigations were repeated 2 years later because of spinal pain. While NMR and CT scan remained compatible with bone necrosis, biopsy on this occasion confirmed the presence of carcinoid tumour. These rare tumours metastasise littl…

Total hip arthroplasty for osteonecrosis.

The success of Charnley cemented total hip arthroplasty for osteonecrosis of the femoral head was assessed using survival analysis. Eighty-seven total hip arthroplasties were followed up for an average of 5.7 years (range, 1.4-13.0 years). There were 59 women and 28 men. The average age was 60 years (range, 30-84 years). Using Charnley hip scores, all patients showed significant progress. The survival analysis showed that using femoral loosening, revision, and pain scores less than 5 as end points, survival was 0.61, 0.94, and 0.81, respectively. With revision as an end point, there were no di…

Total knee arthroplasty in osteonecrosis.

Thirty-eight total knee arthroplasties in 36 patients were studied at 4 +/- 2 (mean, +/- SD) years follow-up evaluation. The Hospital for Special Surgery knee score improved from 59 +/- 9 (mean, +/- SD) before surgery to 83 +/- 11 (mean, +/- SD) (p less than 0.0001) at last evaluation. Good or excellent results were achieved in 87%. Implant survivorship at five years was predicted to be 85% using an endpoint of revision, or 68% using an end point of revision for moderate or severe pain. There was one failure for deep infection but no clinical or roentgenographic evidence of loosening.

Magnetic resonance imaging of osteonecrosis.

Because the available preliminary data strongly indicate that MRI is accurate in diagnosing osteonecrosis, MRI of the hips is recommended in patients suspected of having osteonecrosis, especially if other diagnostic studies are equivocal. MRI also may be useful in following patients who have strong risk factors for osteonecrosis, such as corticosteroid therapy, femoral neck fracture, traumatic femoral head dislocation (Fig. 13), slipped capital femoral epiphysis (Fig. 14), and congenital hip dislocation. In diseases such as systemic lupus erythematosus, both hips eventually may become involved…

Steroid-induced osteonecrosis: a review.

Between 1963 and 1980 cortisone-induced osteonecrosis has been seen in 122 patients at the Royal Victoria Hospital, Montreal. Of these, 68 had had renal homotransplants; the others suffered from a variety of conditions. The femoral head was most commonly affected; the humeral head, distal femur, proximal tibia, talus and capitellum were also involved. There were two patterns of involvement, similar in all locations. The more common is subchondral osteolysis, which may heal without operation. The other is similar to osteochondritis dissecans and requires operation. Alteration in fat metabolism …

Imatinib associated temporal bone osteonecrosis.

Tyrosine kinase inhibitors have substantially improved survival in patients with gastrointestinal stromal tumors (GIST) and chronic myeloid leukemia (CML). We report the first association between long-term imatinib use and temporal bone osteonecrosis, highlighting the importance of prompt ENT evaluation of such patients with new otological symptoms.

Joint Preservation Procedures for Osteonecrosis.

Osteonecrosis of the femoral head is characterized by reduced intraosseous blood flow to the subchondral bone. The management of early osteonecrosis usually involves joint preservation procedures to provide pain relief, prevent disease progression, and avoid joint replacement. A thorough clinical evaluation is crucial to identify at-risk patients and allow early intervention with joint preservation. The decision to use one joint preserving method over another is dependent on staging and patient characteristics. Surgeons should have a thorough understanding of the available joint preservation p…

Patellar Osteonecrosis Following Knee Arthroscopy.

Patellar osteonecrosis is a rare condition, although knee osteonecrosis has been described in the arthroplasty literature. This is the first case describing knee arthroscopy as the cause of osteonecrosis. A 50-year-old woman who was experiencing knee pain during her marathon training and singles tennis underwent knee arthroscopy for a torn medial meniscus. The result of her partial medial meniscectomy led to patellar osteonecrosis 3 months following the index procedure. Osteonecrosis of the knee has been reported in the arthroplasty and sports medicine literature following surgical interventio…

[Maxillary osteonecrosis after denosumab treatment].

A 58-year-old patient with a history of intravenous use of denosumab was referred to the department of oral and maxillofacial surgery with inadequate recovery after a series of extractions. During physical and radiological examination, several sites of exposed necrotic bone with purulent discharge were seen, and the maxilla appeared fractured at the level of Le Fort 1. Treatment consisted of intravenous administration of antibiotics with consecutive sequestrectomy of the maxilla under general anaesthesia. While Medication Related Osteonecrosis of the Jaw (MRONJ) is most commonly seen in patien…

Core decompression for juvenile osteonecrosis.

Core decompression may be used as adjunct for treatment in some cases of Legg-Calvé-Perthes disease (LCPD). The primary application is for patients with onset at 12 years of age or older. We recommend classifying these older patients as idiopathic juvenile osteonecrosis and treating them similarly to adults with avascular necrosis. Juvenile osteonecrosis may benefit from core decompression combined with shelf acetabuloplasty during the early stages of necrosis. Younger children with LCPD may benefit from decompression by fenestration of the femoral head. Experience in adult-onset osteonecrosis…

Avascular osteonecrosis after renal transplantation.

Avascular osteonecrosis (AVN) is a serious osseous complication after renal transplantation. Its prevalence clearly decreased from 20% to 4% after introduction of cyclosporine and reduction of steroid doses. The aim of our study was to evaluate the frequency of AVN among kidney transplant recipients and to determine the risk factors by comparing them with a population without AVN. Among 326 kidney transplant recipients between June 1986 and December 2004, 15 patients developed AVN with mean age of 40.86 years, including 11 men and 4 women. Fifteen kidney transplant recipients without AVN were …

Osteonecrosis complicating chronic myeloid leukaemia.

BACKGROUND: There are previous reports of osteonecrosis among Nigerians with sickle cell diseases and some other medical conditions. To the best of the authors' knowledge there is, however, no known such report attributed to chronic myeloid leukaemia, especially in Nigeria. Apart from presenting this as the first report, it is also to emphasize the need for early full blood count in persons presenting with otherwise non-haematological condition. METHOD: The case note of the patient and the available literature on the subject were used. RESULT: The 15-year-old girl who presented with a fiftee…

Risk factors for dysbaric osteonecrosis.

OBJECTIVES: Dysbaric osteonecrosis (DON) is a complication of ineffective decompression following exposure to high-pressure environments. This study was designed to determine risk factors for the occurrence of DON in divers. METHODS: Fifty-six male divers received skeletal examinations by radiography to assess the occurrence of DON. A questionnaire was used to obtain clinical and diving information, including diving experience and maximum diving depth. Blood samples were collected to analyse the levels of plasminogen activator inhibitor (PAI)-1, cholesterol, triglyceride, low-density lipoprot…

[Osteonecrosis in HIV-infected patients].

Osteonecrosis, also known as avascular necrosis, is chiefly characterized by death of bone caused by vascular compromise. The true incidence of osteonecrosis in HIV-infected patients is not well known and the pathogenesis remains undefined. Hypothetical risk factors peculiar to HIV-infected individuals that might play a role in the pathogenesis of osteonecrosis include the introduction of protease inhibitors and resulting hyperlipidemia, the presence of anticardiolipin antibodies in serum leading to a hypercoagulable state, immune recovery and vasculitis. Hereby we present a series of 13 HIV-i…
