Fracture of highly cross-linked all-polyethylene patella after total knee arthroplasty.

Recent advances in polyethylene fabrication have led to the introduction of highly cross-linked polyethylene tibial and patellar components for use in total knee arthroplasty (TKA) with the goal of reducing wear-related osteolysis. However, some reports suggest decreased mechanical strength as a result of the additional thermal and sterilization treatments in the manufacturing of implants. Complications related to the patella are among the most common causes of failure in TKA, but patellar component fracture is rare. The authors report a case of a highly cross-linked all-polyethylene patellar

Knee locking secondary to osteochondral fracture of the patella: an unusual presentation.

Acute locking is a sign of internal derangement of the knee and is an orthopaedic surgical emergency. Differential diagnoses of this condition are torn meniscus, intra-articular loose bodies and torn anterior cruciate ligament. We present a 15-year-old schoolboy who developed acute locking of the knee during dancing. Arthroscopy revealed that he had sustained an osteochondral fracture of the patella and the loose fragment from it caused locking of the knee. That this was not pseudo-locking was confirmed by examination of the knee under anaesthesia. Locking of the knee by a loose osteochondral

Pin and Wire System Fixation for the Treatment of Comminuted Patella Fractures.

We describe the wiring technique and evaluate the radiographic and clinical outcomes of treatment with a pin and wire system (PWS) for comminuted patella fractures. From June 2013 to October 2018, 33 patients with comminuted patella fractures were treated using a PWS. Open reduction and internal fixation was performed with multiple pins and a wire. All patients were allowed full weight bearing without a brace. Radiographs were obtained to evaluate bone union, implant breakage, back-out of pins, and intra-articular gaps and step-off. Clinical outcomes and postoperative complications were assess…

Treatment for transverse patella fractures with minimally invasive techniques (Review).

The transverse fracture pattern is the most common pattern among patellar fractures. Although open reduction with internal fixation using tension band wiring is the common method for treating the majority of transverse patellar fractures with displacement, this approach has distinct disadvantages, including damage to the blood supply and general post-operative complications. Therefore, minimally invasive techniques have been introduced to overcome these problems. In the present review, the advanced surgical procedures using Kirschner wires with cerclage, cannulated screw optioning of supplemen

Sleeve fracture of the adult patella: Case report and review of the literature.

RATIONALE: The patellar fractures are common in adults, but rare in children. As a particular type of patellar fracture, however, sleeve fractures are almost always limited to children in the under 16's group. PATIENT CONCERNS: Herein, we report a rare case of a 19-year-old healthy adult female who presented sleeve fracture at the superior pole of the left patella. The clinical and radiological features are found including joint effusion, anterior tilt of the patella and a shell of bone lying proximally to the patella. DIAGNOSES: Computed tomography and magnetic resonance imaging examination

Sleeve fractures of the patella: current concepts in diagnosis and surgical management.

PURPOSE OF REVIEW: Patella sleeve fractures are the most common patellar fracture in children yet remain frequently missed. Additionally, the options for their repair have grown. This review revisits how the injury is recognized and how the expanding range of fixation choices should be navigated. RECENT FINDINGS: Recent cohorts confirm that recognition rests on the examination rather than the radiograph, as the injury is frequently confused with Sinding-Larsen-Johansson syndrome. In sleeve fractures that are displaced or minimally displaced and in which the extensor mechanism is disrupted, op

Suture Augmentation of Internal Fixation for Highly Comminuted Patella Fractures.

Treatment of comminuted patella fractures is challenging. Augmentation of internal fixation has been proposed to improve stability. The authors describe an effective technique to augment internal fixation consisting of locked sutures within the quadriceps and patellar tendons. This technique functions to off-load internal fixation and restore tension to the extensor mechanism. The locked suture augmentation technique was performed for 13 patients undergoing open treatment of patella fractures by the senior author from 2016 to 2019. Patients were followed for an average of 10.2 months. No patie…

[Misunderstanding about the tension band principle in the treatment of patella fractures].

Kirschner's wire tension band was the first choice in the treatment of patella fracture. But the principle of tension band had been misunderstood for long time. In the traditional principle, it was believed that tension band could change the separating force between fracture fragments into compressing stress when exercising. Eccentric fixation and articular surface cracking were disadvantageous. These could make the fracture fragments separate either at resting or exercising, which cause the gaps without pressure. Thus excercising could not change the separating force into compressing stress.

Acute dislocation of the patella with osteochondral fracture: a review of eighteen cases.

Eighteen patients with acute dislocation of the patella had associated osteochondral fractures. This fracture complicates approximately 5% of all acute dislocations of the patella occurring in children. Three types of fracture patterns were noted. All patients who were treated by immediate arthrotomy and excision or replacement of the osteochondral fragment and repair of the acute dislocation of the patella made an uneventful recovery with no recurrence of the dislocation. In those patients in whom the osteochondral fragment was removed but with no repair the dislocation recurred.

Double-Row Suture Anchor Fixation for Inferior Pole Patella Fracture: A Novel Technique.

Inferior pole patella fractures are challenging to treat due to small, comminuted fragments and the high tensile forces generated by the extensor mechanism. Conventional surgical fixation techniques are associated with implant irritation, migration, hardware failure, and the need for secondary procedures. This case report describes a novel double-row suture anchor fixation technique with suture tape augmentation for treating an inferior pole patella fracture. An 80-year-old patient with a displaced comminuted inferior pole fracture underwent surgical fixation using obliquely inserted double-ro…
