A new surgical technique to uniformly harvest the middle one-third of the patellar tendon for anterior cruciate ligament reconstruction is described. The technique uses a guide system of 8 templates of varying widths to guide the saw cuts and "collared," depth-controlled saw blades and drill bits to produce a standard cutting and drilling depth. This kind of template system lessens the risks of graft harvesting complications such as patella fracture, bone graft fragmentation, suture pull out, and judgment errors pertaining to graft depth, length, or width. The Patellar Tendon Graft Guide was u

BACKGROUND: Medial patellar luxation is a common orthopedic condition in small breed dogs, though its etiology remains poorly understood. The aim of this study was to assess the morphometry of the femoral trochlea, patella and patellar ligament in small breed dogs with and without grade 2 medial patellar luxation. This retrospective study evaluated small breed dogs weighing ≤ 15 kg including 80 stifles with Grade 2 MPL and 80 controls. The femoral trochlear length to patellar length ratio (TL: PL), trochlear curvature to condylar curvature ratio (TC: CC) and patellar ligament length to patella…

Recurrent lateral patellar instability is a complex pathology that requires orthopaedic surgeons to consider a variety of factors with an à la carte approach to surgical solutions. In cases of isolated medial patellofemoral ligament reconstruction, patellar height is known to reduce significantly, and in some cases even normalize from a preoperative patella alta status. This becomes particularly important to consider through the lens of direct attachments to the patella, such as the vastus medialis oblique and retinacular tissues, and the influence of patellar height on quadriceps activation a

PURPOSE: The primary outcome of interest of this study was to determine the frequency, location, and extent of chondral injuries and medial patellofemoral ligament (MPFL) tears, along with the rate of loose bodies, in the knee after acute primary patellar dislocation. The secondary outcome of interest was to conduct a multivariate analysis to investigate whether the presence of pathoanatomic abnormalities, including structural differences in the knees of the patients, is associated with the features of the lesions.
METHODS: Patients who underwent magnetic resonance imaging after primary acute

A 1-year-old athletic golden retriever dog was presented for a non-weight-bearing right pelvic limb lameness that occurred during a canicross race. Orthopedic examination revealed pain and inflammation of the right stifle joint. Radiographs showed a traumatic comminuted patellar fracture. Surgery was elected and a midsubstance patellar ligament rupture was also identified. Surgical management consisted of a combination of techniques for patellar fracture fixation and primary patellar ligament reconstruction. All orthopedic implants were removed secondary to implant failure. Postoperative 20-mo…

BACKGROUND: Medial patellofemoral ligament (MPFL) reconstruction is the preferred operative treatment for recurrent patellar dislocation. The purpose of this study was to report a novel suture-tie technique of patellar side fixation in medial patellofemoral ligament reconstruction for recurrent patellar dislocation, and to evaluate the intermediate-term results.
METHODS: We retrospectively reviewed the results of 22 MPFL reconstructions in 21 patients (6 male and 15 female) with a suture-tie technique performed by a single surgeon between March 2004 and July 2009. All patients had been diagno

We present a case of combined avulsion fracture of the tibial tubercle (AFTT) and avulsion of the patellar ligament in a 15-year-old boy. This injury was treated by open reduction and fixation of both lesions using staples with satisfactory results. This constellation of injury is rare but a high index of suspicion is needed when faced with a displaced AFTT. Open reduction and internal fixation should be the treatment of choice.

PURPOSE: The importance of the medial patello-femoral (MPFL) and medial patello-tibial ligaments (MPTL) to the stability of the patella is undoubted. The purpose of this work was to present a technique for the reconstruction of both ligaments and prospectively record its outcome.
METHODS: Cadaveric part: the MPFL and MPTL were identified in five knee specimens. Sequential cutting and reattaching of these ligaments were performed to study their effect on patellar stability. CLINICAL PART: In the years between 2005 and 2008, twenty-five knees in twenty-one patients had reconstruction of their M

OBJECTIVE: To describe a surgical technique for splitting the proximal third of the medial patellar ligament (MPL) in horses and ponies with upward fixation of the patella (UFP), and to report outcome.
STUDY DESIGN: Prospective clinical study.
SAMPLE POPULATION: Four horses and 3 Shetland ponies.
METHODS: With ultrasound guidance, the proximal third of the MPL was split by percutaneous use of a no. 15 scalpel blade. The rationale for this surgical procedure was to induce a localized desmitis with subsequent ligament thickening. No anti-inflammatory drugs were administered, and horses and po…

We describe a technique of reconstruction for complete midsubstance ruptures of the anterior cruciate ligament in patients with wide open physis, using the central third of the patellar tendon. This specific procedure is named the Clocheville technique. Results were very good in five cases and good in three cases at 2 years minimal follow-up. We neither noticed angular deformity, nor significant change in lower leg length. The Clocheville technique is an excellent alternative for young patients, with residual growth of the knee greater than 15 mm, if presenting an objective instability in acti

The operative result in 91 patients who had a repair of a ruptured anterior cruciate ligament and replacement with a bone-patellar ligament-bone autogenous graft are analyzed. The average time between initial injury and operation was 2 years. Instability of the knee had been present for 6 months to 6 years and the mean follow up was 4 years. Indications for the intraarticular repair were a positive Pivot shift test and Lachman test and a positive anterior drawer sign at the preoperative examinations while under anesthesia. Seventy percent of the injuries were sports related and the remaining t…

BACKGROUND: Recurrent patellar dislocation (RPD) occurs in people who have their own patellofemoral dysplasia and who have not been properly treated after their first patellar dislocation. For RPD where conservative treatment is ineffective, medial patellofemoral ligament (MPFL) reconstruction is the first choice for surgical treatment, but there are various and controversial ways of MPFL reconstruction and fixation. Initially, more scholars adopted the patellar lateral tunneling (PT) approach to contain and stabilize the graft, but with the newer materials and techniques, some experts adopted

CASE: We report the case of an 11-year-old boy with Ehlers-Danlos syndrome (EDC) who exhibited simultaneous medial and lateral patellar instability. The patient presented with a medial patellar dislocation, and subsequently, the patella became very unstable both medially and laterally. Despite distal realignment, the patellar instability was so significant that he underwent simultaneous reconstruction of the medial and lateral patellofemoral ligament using the semitendinosus tendon, with a good result.
CONCLUSION: Simultaneous reconstruction of the medial and lateral patellofemoral ligament i

Several techniques for patellar fixation for reconstruction of the medial patellofemoral ligament (MPFL) have been described in the literature. Despite the success of MPFL reconstruction reported in the literature, there is insufficient evidence to recommend a standard method of patellar fixation. A hybrid 2-point fixation technique allows for increased contact area and contact pressure between the insertion of the graft and the patella and offers a broad insertion of the graft onto the width of the patella, thus allowing for a more native attachment site to be re-created. The technique involv

BACKGROUND: Lateral patellar luxation (LPL) is commonly diagnosed in large or giant breed dogs. In the surgical outcome for canine LPL, approximately half of the patients had complications, including reoperation and relaxation. Medial patellofemoral ligament (MPFL) reconstruction has been widely used for human repetitive patellar luxation. This case report describes that modified MPFL reconstruction with other surgical techniques might be effective for severe canine LPL repair.
CASE DESCRIPTION: An 11-month-old intact male Siberian Husky was referred to us with the main complication reported
