BACKGROUND: Cleft lip and palate is the most common craniofacial birth defect in the UK. Orofacial clefts have functional and aesthetic implications requiring intensive multi-disciplinary follow-up to optimise development. Failure to attend follow-up is likely to have a negative impact on patient outcomes. The aim of this retrospective audit is to establish if socioeconomic status influences attendance, DNA and cancellation rates in cleft patients.
METHODS AND RESULTS: A retrospective audit of 74 orofacial cleft patients born and operated on at the Royal Hospital for Sick Children Glasgow bet

Acute deterioration of patients with cirrhosis manifests as multiple organ failure requiring admission to an intensive care unit. Precipitating events may be viral hepatitis, typically in Asia, and drug or alcoholic hepatitis and variceal hemorrhage in the West. Patients with cirrhosis in the intensive care unit have a high mortality, and each admission is associated with a mean charge of US $116,200. Prognosis is determined by the number of organs failing (sequential organ failure assessment [SOFA] score), the presence of infection, and the degree of liver dysfunction (Child-Turcotte-Pugh or

The many comorbidities in the severely ill patient also make thrombocytopenia very common (∼40%) in intensive care unit patients. The risk of bleeding is high with severe thrombocytopenia and is enhanced in intensive care patients with mild or moderately low platelet counts when additional factors are present that interfere with normal hemostatic mechanisms (eg, platelet function defects, hyperfibrinolysis, invasive procedures, or catheters). Even if not associated with bleeding, low platelet counts often influence patient management and may prompt physicians to withhold or delay necessary inv

Epidural analgesia is a common technique used to manage acute pain after major surgery and is viewed as the 'gold standard'. When managed effectively, epidural analgesia is known to reduce the risk of adverse outcomes following major surgery. There are two main classes of medications used in epidural analgesia: opioids and local anaesthetics. Both of these drugs are beneficial in reducing or eliminating pain, but are also responsible for the common side effects associated with this method of pain relief. There are also some rare and potentially fatal side effects of epidural therapy. The nurse

There has been a remarkable rise in the number of kidney transplant recipients (KTR) in the US over the last decade. Increasing use of potent immunosuppressants, which are also potentially diabetogenic and atherogenic, can result in worsening of pre-existing medical conditions as well as development of post-transplant disease. This, coupled with improving long-term survival, is putting tremendous pressure on transplant centers that were not designed to deliver primary care to KTR. Thus, increasing numbers of KTR will present to their primary care physicians (PCP) post-transplant for routine me

Public health nurses in local health departments may receive the first call regarding a potential case of avian influenza, monkeypox, or viral hemorrhagic fever. One public health approach to containing these dangerous infectious disease outbreaks is the use of specialized isolation units. Early access to a biocontainment patient care unit (BPCU) for isolation during a bioterrorism or public health emergency event along with appropriate use of epidemiological and therapeutic interventions in the community may dramatically impact the size and severity of a disease outbreak (Smith et al., 2006).…

PURPOSE OF REVIEW: Cataract surgery has evolved significantly during the last few years. The introduction of aspheric, presbyopic correcting and toric intraocular lenses have shifted the emphasis of cataract surgery from just treating patients' functional symptoms to optimizing the refractive outcome of the procedure.
RECENT FINDINGS: Studies emphasizing patient satisfaction and refractive visual outcomes have shifted the focus of cataract surgery to a more patient-centered approach. Refractive surgical technologies such as limbal relaxing incisions, laser-assisted in-situ keratomileusis, pho

OBJECTIVES: An important factor contributing to the steep rise in health care costs in the late 1960s was a reversal from the predominance of acute illness to that of chronic disease. Beginning with the philosophy of Illich and Levin, and the practical instruments of Fries, Sehnert, Vickery, and Ferguson, a new movement in patient self-care emerged. However, such programs were not integrated into organized medical care plans and though theoretically attractive had not yet proven to improve health or decrease costs.
METHODS: The contributions to the self-care movement made under the intellectu

As the use of standardized patients (SPs) for education and assessment continues to grow, there becomes an ever-increasing cohort of patients in our clinical practices who have participated as SPs. The present study is part of a five-year longitudinal study to examine the impact of participation as a standardized patient on the perceptions of the standardized patient's own health care. SPs participating in the 1993, 1994 and 1995 Objective Structured Clinical Examinations (OSCE) for medical students at the end of their third year were requested to respond to mailed questionnaires before, immed

Transportation of the intensive care unit (ICU) patient to the operating room for tracheotomy has been implicated as an unnecessary source of complications and has been cited as a relative indication for percutaneous tracheotomy. However, there is very little evidence in the literature to support this claim. We evaluated 100 consecutive patients who were transported from the ICU to the operating room for tracheotomy. There were no complications related to patient transportation. A total of five complications occurred, all unrelated to patient transportation. Two patients receiving pressure con

Computerization of these protocols is in progress. This project is funded for one year with projected completion in January 1994. This study … as what is the impact on patient outcomes in regard to cost of care and complications. Computerized patient care protocols can be further developed to meet patient-specific needs. A computerized data base will facilitate managing large amounts of patient data and tailoring instructions to these patients. One of the goals of this project was to measure the feas …

Cytotoxic therapy for paediatric malignancy continues to be associated with significant mortality and morbidity. However, survival figures continue to improve despite increasingly toxic treatment protocols. Aggressive supportive care, more than any other modality, has produced a dramatic reduction in mortality during the past 30 years, and includes the appropriate management of infection, provision of adequate nutrition, and proper use of blood and blood products. The paediatric surgeon plays an integral role in the multidisciplinary team of cancer management and together with other team speci

Self-mutilative behavior is common among patients with multiple personality and other dissociative disorders. Nursing staff members face particular challenges in managing these patients because one act of self-mutilation can disrupt the entire inpatient milieu. The authors present an approach to nursing care that focuses on working with patients to understand and develop a specific plan to curtail the self-mutilative behavior.

The treatment plan for the pediatric cardiac patient must include the following: 1. A complete medical history that will elicit adequate information with respect to the patient's possible heart condition. 2. Consultation with the family physician, pediatrician and/or cardiologist to learn the specific nature of the defect, specific past history, the child's ability to tolerate stress and anxiety, current medication and any specific recommendations for patient management. 3. Antibiotic prophylaxis, in an effort to prevent endocarditis, for all dental procedures that are likely to result in ging

Family physicians are often unfamiliar with the care of patients with spinal cord injuries because they may have only one such patient in their practice. Urinary tract infections, constipation, and decubitus ulcers are the most common problems, and autonomic dysreflexia the most serious emergency that family physicians treat in this population. This article addresses these areas, as well as spasticity, sexuality, depression, and the acute abdomen.

That elderly hypertensive patients need to be treated is now well established, as major clinical trials have confirmed the reduction in serious complications and organ damage in the treated patient. However, the concomitant findings on the high rates of adverse side effects of drug therapies to lower blood pressure are of concern. The physiologic and disease changes that occur with aging may explain some of the adverse drug responses encountered in this population. Nondrug treatment of hypertension, including sodium restriction and weight reduction, would be desirable in the elderly, but there

In dealing with the traumatized pregnant patient, certain special factors must be kept in mind, as follows: Remember you are dealing with two lives, the mother and her developing fetus. Be aware of the anatomic and physiologic changes that occur during pregnancy and be able to take them into account in treating the injured pregnant patient while using the ABCs of trauma care. Particular emphasis must be placed on prevention of fetal anoxia by maintenance of the mother's respiratory status. Frequent blood gas measurements are required. A careful history of the course of the patient's present an
