Tumor-To-Tumor Metastasis of Poorly Differentiated Gastric Carcinoma to Uterine Lipoleiomyoma.

The rare phenomenon of tumor-to-tumor metastasis was first described in 1930. The donor neoplasm is most frequently lung or breast carcinoma, whereas intracranial meningiomas are reportedly the commonest recipient neoplasm. Here we report a case of metastasis from a primary gastric cancer to a uterine lipoleiomyoma. A 65-year-old woman presented with locally advanced gastric cancer with computed tomography (CT) evidence of peritoneal dissemination and a 9 cm pelvic mass. She underwent 16 courses of TS-1/cisplatin chemotherapy, which achieved significant tumor reduction. However, repeat CT and

A new case of primary signet-ring cell carcinoma of the cervix with prominent endometrial and myometrial involvement: Immunohistochemical and molecular studies and review of the literature.

BACKGROUND: As a rule, endocervical tumours with signet-ring cell are classed as metastatic extra-genital neoplasms. In a patient aged 45 years, we describe primary cervical signet-ring cell carcinoma (PCSRCC) characterized by prominent endometrial and myometrial involvement, simulating primary endometrial adenocarcinoma with cervical extension. In addition, a review was made of the literature to identify the clinical and pathological features of this rare malignancy. CASE PRESENTATION: A 45-year-old woman was referred to our Gynaecology Department due to persistent abnormal vaginal bleeding.

Lateral Pelvic Sidewall Nodes.

A 68-year-old woman presented with rectal bleeding, urgency, and tenesmus. A digital rectal examination confirmed a craggy mass infiltrating into the sphincter complex. Follow-up colonoscopy noted a low-rectal tumor (3 cm from the dentate), and histopathology confirmed a moderately differentiated adenocarcinoma. Subsequent staging with MRI confirmed a 5-cm circumferential low-rectal neoplasm with extramural vascular invasion and threatened circumferential resection margin. The neoplasm abutted the posterior vaginal wall and was invading the internal sphincter complex. Four enlarged mesorectal

A Unique case of primary squamous carcinoma of the salpinx associated with serous carcinoma of the omentum: a pathological and molecular study.

In this article, we report a case of primary squamous cell carcinoma of the salpinx (PSCCS) with immunohistochemical and molecular studies to evaluate the phenotype and define the etiopathogenesis of this neoplasm. A 77-year-old woman, 38 years postmenopausal, was admitted to the Department of Obstetrics and Gynecology for ascites. Her clinical history showed breast carcinoma and left salpingooophorectomy as a result of extrauterine pregnancy. Cytological examination of the free peritoneal fluid showed clusters of malignant cells consistent with ovarian carcinoma. Transvaginal ultrasonography

Neoplastic involvement of the sacroiliac joint: MR and CT features.

The radiological findings in five patients with pelvic soft tissue neoplasms directly involving the sacroiliac joint, are described. All patients had Computed Tomography (CT) examinations, two of the patients also having Magnetic Resonance Imaging (MRI). The role of imaging in this uncommon entity is discussed as well as the importance of making this diagnosis, thereby excluding unilateral sacroiliitis. The therapeutic implications of this diagnosis relate to local neural involvement, especially the sciatic nerve, and the fact that involvement of the sacroiliac joint by tumors significantly co

[Malignant tumors in a region of endemic nephropathy].

In villages with endemic nephropathy (EN) high incidence and mortality rates of some urinary system tumors are reported. Age-adjusted incidences of renal pelvic and ureteral neoplasms were 46/10(5) in females and 27/10(5) in males; for urinary bladder tumors these figures were 16/10(5) and 35/10(5). The frequency of parenchymal kidney tumors was similar to that reported for the country as a whole. An increased relative risk of developing urinary system tumors was observed in patients with EN and with advancing age (only in women). Smoking did not relate to the occurrence of these neoplasms. No

Castleman disease presenting as a pelvic mass.

BACKGROUND: To our knowledge, giant lymph node hyperplasia (Castleman disease) may present as a pelvic mass on magnetic resonance imaging (MRI). CASE: A postmenopausal woman with rapidly enlarging leiomyoma uteri was found to have a suspicious left adnexal mass mimicking an ovarian neoplasm on preoperative MRI. At laparotomy, the suspected uterus and normal ovaries were extirpated. In addition, a firm, 4 x 8-cm solid mass within the sigmoid colon mesentery was found and resected. The final histologic diagnosis was Castleman disease. CONCLUSION: Entities such as Castleman disease should be co

Pure Sertoli cell tumor of the right ovary managed with fertility-sparing surgery: a case report.

Sertoli cell tumors (SCTs) of the ovary are rare sex cord-stromal neoplasms. We describe a 37-year-old Ethiopian woman (G2P2) with six months of progressive lower abdominal pain but no virilization. Ultrasound revealed a solid 11 cm right adnexal mass. Hormonal profile was normal. An open fertility-sparing right salpingo-oophorectomy was performed. Histopathology confirmed a FIGO Stage IA well-differentiated pure SCT. Postoperative surveillance was conducted via clinical pelvic examinations and abdominal/pelvic ultrasounds every three months. At 12-month follow-up, there was no recurrence. Thi

Synchronous Low-grade Appendiceal Mucinous Neoplasm and Primary Peritoneal Low-grade Serous Carcinoma: A First Description of These 2 Neoplasms Presenting Together as Suspected Peritoneal Carcinomatosis.

Low-grade appendiceal mucinous neoplasm is a neoplasm typically of appendiceal origin, which is characterized by diffuse peritoneal involvement by pools of mucin with mucinous epithelium lacking high-grade cytologic atypia, and clinically presents as suspected peritoneal carcinomatosis. A similar clinical presentation can sometimes be seen with disseminated low-grade serous carcinomas of the peritoneum, fallopian tubes, or ovaries; however, this neoplasm is histologically characterized by tubal-type epithelium and invasive or confluent growth. In this case report, we describe a patient present

[Hydronephrosis associated with elevated serum levels of CA 125 antigen. Report of a case].

High level of the CA 125 serum antigen is typically associated with ovarian malignancies. However the CA 125 antigen can also be found in association with inflammation or neoplasms of tissues other than ovaries, i.e. mesothelial cells, mullerian duct derivatives and gastroenteric tract. Therefore the marker is not specific for ovarian neoplasms. In childhood there is not a large experience about clinical meaning of high CA 125 serum levels. We report a case in a 14-year-old patient, female, affected by hydronephrosis. Our case confirms that high CA 125, especially in childhood, is not necessar

First-reported case of vaginal late recurrence of Wilms tumor treated with complete pelvic exenteration.

Late relapses of Wilms tumour are extremely uncommon but still represent possible events. Even more rarely Wilms tumours present as extrarenal neoplasms, for which it could be difficult to diagnose and treat them promptly. We present a unique case of late recurrence of Wilms tumour 16 years after the primary diagnosis, with location in the gynaecological system. The relapse presented as a vaginal mass, and it gradually raised up to involve the majority of pelvic organs. We accurately studied the tumour extension, even realising a 3D preoperative reconstruction, and we managed to evaluate the pa

[Pelvic and abdominal actinomycosis presenting as a parietal mass].

INTRODUCTION: Actinomycosis is a rare disease. It has a highly varied clinical picture and may simulate genital or gastrointestinal neoplasms. CASE: This 45-year-old woman was referred for suspected ovarian cancer, with secondary lesions of the liver and pelvic wall. Pelvic actinomycosis was first suggested by the presence of an intrauterine device (IUD), which had been in place for several years without any follow-up. The final diagnosis was based upon histological examination of a biopsy sample of the parietal mass. COMMENTS: The association of apparent pelvic tumors with infection and inf

Update on Rectal Neuroendocrine Neoplasms.

Rectal neuroendocrine neoplasms (R-NENs) are increasingly identified in clinical practice, mainly due to the expansion of colorectal cancer screening programs. These tumors are most often detected incidentally as small, well-differentiated lesions. Although many R-NENs exhibit indolent behavior, prognosis and management depend on accurate staging, with particular emphasis on evaluating regional lymph nodes and distant metastases. Cross-sectional imaging is central to this assessment. Endoscopy and endoscopic ultrasound provide essential information regarding tumor size and depth of invasion, w

Mixed epithelial and stromal tumor of the seminal vesicles: report of a rare case with diagnostic, therapeutic, and prognostic insights.

BACKGROUND: Mixed epithelial and stromal tumors (MESTs) of the seminal vesicle are exceptionally rare neoplasms composed of both epithelial and stromal elements, posing significant diagnostic challenges due to their rarity and overlapping characteristics with other pelvic neoplasms. CASE PRESENTATION: We describe a 45-year-old patient with chronic pelvic pain and obstructive urinary symptoms. Imaging revealed a large cystic and solid mass involving his seminal vesicles, with significant mass effect on adjacent structures. Differential diagnoses included seminal vesicle adenocarcinoma and sarc…

Primary carcinoma of the fallopian tube in a Nigerian.

The first documented report of a rare neoplasm, primary carcinoma of the fallopian tube in a postmenopausal Nigerian is presented. The patient complained of postmenopausal bleeding which persisted after curettage and satisfactory treatment of a pre-existing senile vaginitis and cervicitis. The diagnosis of tubal carcinoma was made only at the time of exploratory laparotomy at which a total hysterectomy and bilateral salpingo-oophorectomy was performed and confirmed at histopathological examination of the specimen. Problems relating to its unusual symptomatology, its association with pelvic inf

Chlamydia Peritonitis and Ascites Mimicking Ovarian Cancer.

Background. Pelvic inflammatory disease (PID) rarely results in diffuse ascites. Severe adhesive disease secondary to PID may lead to the formation of inclusion cysts and even pelvic peritoneal nodularity due to postinflammatory scarring and cause an elevation of serum CA-125 levels. The constellation of these findings may mimic an ovarian neoplasm. Case. We report a case of a 22-year-old female who presented with multiple pelvic cysts and diffuse ascites due to Chlamydia trachomatis infection. The initial gynecologic exam did not reveal obvious evidence of PID; however, a positive Chlamydia t

Lipomatous uterine masses: potential to mimic ovarian dermoids on endovaginal sonography.

Lipomatous uterine masses are uncommon hyperechoic pelvic neoplasms composed partly of adipose tissue. Because of the high level of echogenicity, these uterine masses can produce produce a sonographic appearance strikingly similar to the "dermoid plug" that is considered characteristic of benign cystic teratomas. The key to distinguishing the two tumors is to ascertain the parent organ: the lipomatous uterine mass should clearly originate from the myometrium. In our experience, however, the origin of some echogenic pelvic masses can be difficult to determine, particularly from the endovaginal …

Subepithelial pelvic hematoma of the kidney (Antopol-Goldman Lesion).

Renal pelvic hematoma (Antopol Goldman lesion) is a rare but significant condition that may clinically mimick a renal or a pelvic neoplasm. Differential diagnosis and optimal treatment are still not known certainly. A 80-year-old male patient admitted to the emergency department with gross hematuria/clot retention and right flank pain. Magnetic resonance imaging (MRI) imaging revealed a filling defect in the right renal pelvis. Diagnostic flexible uretrorenoscopy was performed and a renal pelvic tumor was excluded. A 6 Fr double J (DJ) ureteral catheter was placed for 4 weeks while the patient

Giant pelvic solitary fibrous tumor obstructing intestinal and urinary tract: a case report and literature review.

We are reporting a giant pelvic neoplasm, a rare solitary fibrous tumor that presented with a large bowel obstruction and bilateral ureteral obstruction because of its size and location. Preoperative diagnosis required complex pathological studies to exclude a high-grade sarcoma suspected clinically. Complete resection was required for resolution of obstructive symptoms. Prognosis for solitary fibrous tumors is usually good after complete resection. Recurrence and metastasis may be related to rare aggressive histological features, including nuclear atypia, hypercellularity, greater than four m
