Decreased pregnancy rate is linked to abnormal uterine peristalsis caused by intramural fibroids.

BACKGROUND: The relationship between fibroids and infertility remains an unsolved question, and management of intramural fibroids is controversial. During the implantation phase, uterine peristalsis is dramatically reduced, which is thought to facilitate embryo implantation. Our aims were to evaluate (i) the occurrence and frequency of uterine peristalsis in infertile women with intramural fibroids and (ii) whether the presence of uterine peristalsis decreases the pregnancy rate. METHODS: Ninety-five infertile patients with uterine fibroids were examined using magnetic resonance imaging (MRI)…

Total fundoplication is superior to partial fundoplication even when esophageal peristalsis is weak.

BACKGROUND: About a decade ago, partial (240 degrees) fundoplication became popular for treating gastroesophageal reflux disease in cases where the patient's primary esophageal peristalsis was weak. A total (360 degrees) fundoplication was reserved for patients with normal peristalsis (tailored approach). The theory was that partial fundoplication was an adequate antireflux measure, and by posing less resistance for the weak esophageal peristalsis to overcome, it would give rise to less dysphagia. Short-term results seemed to confirm these ideas. STUDY DESIGN: This study reports the longterm …

Polysectoid: a hyperredundant soft-bodied robot for modeling the role of parapodia in undulation and peristalsis.

Biological inspiration offers new and innovative solutions to exploring challenging terrains, and implementations in bio-inspired robotics in turn offers insights to biological form and function. In particular, annelids (segmented worms), such asNereissp. (bristleworms), are useful subjects for their multi-modal locomotion through differing environments. This research aims to mimic key anatomical features of nereid worms in order to develop a new bio-inspired soft robot, named 'Polysectoid', that effectively moves through challenging terrains using both peristalsis and undulation. The muscles …

Spontaneous electrical and Ca2+ signals in the mouse renal pelvis that drive pyeloureteric peristalsis.

1. Peristalsis in the smooth muscle cell (SMC) wall of the pyeloureteric system is unique in physiology in that the primary pacemaker resides in a population of atypical SMCs situated near the border of the renal papilla. 2. Atypical SMCs display high-frequency Ca(2+) transients upon the spontaneous release of Ca(2+) from inositol 1,4,5-trisphosphate (IP(3))-dependent stores that trigger cation-selective spontaneous transient depolarizations (STDs). In the presence of nifedipine, these Ca(2+) transients and STDs seldom propagate > 100 mum. Synchronization of STDs in neighbouring atypical SMCs …

Effect of metoclopramide on peristalsis and antiperistalsis in isolated frog stomach and guinea-pig ileum.

The action of metoclopramide was studied on peristaltic as well as antiperistaltic activity in frog stomach and guinea-pig ileum. It was observed that metoclopramide enhances normal peristaltic activity by acting on dopamine-receptors whereas it inhibits antiperistalsis by some unknown mechanism. It is, therefore, inferred that inhibition of antiperistaltic activity might be an additional mechanism for its antiemetic effect. It also explains the pharmacological basis for its use in oesophagitis due to gastro-oesophageal reflux.

From Pixels to Peristalsis: Comparing Achalasia Diagnosis With Artificial Intelligence With Expert Endoscopists.

BACKGROUND & AIMS: Esophageal motility disorders, such as achalasia, require the use of multiple tests to establish a diagnosis. Patients are first assessed with upper endoscopy, but subtle disease indicators are often overlooked during the procedure leading to delayed diagnosis. Artificial intelligence has the potential to prevent these misdiagnoses and serve as an early screening tool to enhance the physician's decision-making. METHODS: We developed a video-based transformer model capable of detecting achalasia from upper endoscopy videos. Videos were collected from 1203 patients who presen

Esophageal acid burden in reflux patients with normal endoscopy: Does esophageal peristalsis matter?

BACKGROUND: A majority of patients with gastroesophageal reflux disease (GERD) have normal endoscopy. We aimed to investigate whether esophageal primary and secondary peristalsis influence esophageal reflux parameters in patients with normal endoscopy. METHODS: We enrolled consecutive patients with typical reflux symptoms and normal endoscopy. All patients underwent High resolution manometry (HRM) and 24-h impedance-pH studies off therapy. During HRM, secondary peristalsis was evaluated using ten 20-mL rapid air infusions into the esophagus, while primary peristalsis was evaluated using ten 5…

Validation of secondary peristalsis classification using FLIP panometry in 741 subjects undergoing manometry.

BACKGROUND AND AIMS: This study aimed to systematically evaluate a classification scheme of secondary peristalsis using functional luminal imaging probe (FLIP) panometry through comparison with primary peristalsis on high-resolution manometry (HRM). METHODS: 706 adult patients that completed FLIP and HRM for primary esophageal motility evaluation and 35 asymptomatic volunteers ("controls") were included. Secondary peristalsis, that is, contractile responses (CRs), was classified on FLIP panometry by the presence and pattern of contractility as normal (NCR), borderline (BCR), impaired/disorder…

Quantifying esophageal peristalsis with high-resolution manometry: a study of 75 asymptomatic volunteers.

The vastly enhanced spatial resolution of high-resolution manometry (HRM) makes it possible to simultaneous monitor contractile activity over the entire length of the esophagus. The aim of this investigation was to define the essential features of esophageal peristalsis in novel HRM paradigms and establish their normative values. Ten 5-ml water swallows were recorded in each of 75 asymptomatic controls with a solid-state manometric assembly incorporating 36 circumferential sensors spaced at 1-cm intervals positioned to record from the hypopharynx to the stomach. The data set was then subjected…

Pyeloureteral motility and ureteral peristalsis: essential role of sensory nerves and endogenous prostaglandins.

The cellular mechanisms that underlie the initiation and propagation of the peristaltic contractions, which transport urine from the kidney … ndogenous release of both tachykinins and prostaglandins are essential in the maintenance of normal peristalsis, as well as in monitoring and responding to any chemical or mechanical stimulation. However, the cellular mechanisms underlying the action of these endogenously-released agents remain to be elucidated.

The role of the L-arginine-nitric oxide pathway for peristalsis in the opossum oesophageal body.

BACKGROUND: The aim of the study was to determine the effect of NG-nitro-L-arginine (L-NNA), an inhibitor of nitric oxide (NO) synthesis, on primary peristalsis in the oesophageal body. METHODS: Peristalsis was induced by pharyngeal stroking in 14 lightly anaesthetized opossums. Oesophageal pressures were monitored with a four-channel, perfused catheter assembly and registered with external transducers 1, 4, 7, and 10 cm proximal to the oesophagogastric junction. Propagation time was the time taken for a contraction to travel between two recording sites and was determined in the proximal, mid…
