Abdominal pain is a frequent presentation to the Emergency Department and it is often difficult to find a clear diagnosis. This is a unique case of a woman with worsening abdominal pain over 2 days with no clear etiology despite extensive, repeat evaluations with eventual spontaneous resolution. The diagnosis of chemical peritonitis was established when the patient revealed an extremely rare mechanism of injury the night before presentation that has never been described previously. Access to the abdominal cavity through the uterus has been documented by retrograde flow during menses, and throu…

Antibiotherapy is a fundamental for the treatment of peritonitis. It may be used before surgery or as a complementary treatment after. Experimental models have demonstrated that infections are both aerobic and anaerobic. During the first stage, septicemic with a high death rate, the infection is due to enterobacteria, mostly Escherichia coli. Between D5 and D7 in surviving animals, there is a second stage with abscesses due to anaerobic bacteria, mostly Bacteroides fragilis. The antibiotic treatment must include these two types of bacteria in its spectrum. The role of Enterococcus faecalis is …

To clarify the relationship between clinical features in utero and postnatal prognosis, 20 fetuses who underwent ultrasonic (US) evaluation for meconium peritonitis (MP) over a 17-year period were reviewed. According to final US findings in utero, patients were classified into three types. Type I (massive meconium ascites) was noted is 5 cases, type II (giant pseudocyst) in 4, and the other 11 were classified as type III (calcification and/or small pseudocyst). Abdominal calcifications were identified in only 5 cases (2 type I, 1 type II, 2 type III). Seven fetuses who had associated polyhydra…

A peritonitis caused by an ascending infection is a rare complication postpartum. A 37-year-old woman presented with a secondary peritonitis due to Streptococcus pneumoniae. The patient had given birth to a healthy boy 4 weeks before and showed no symptoms of a bronchitis on admission. An operation was performed after the patient developed an acute abdomen, showing a diffuse peritonitis. High vaginal swabs and blood cultures taken on admission were positive for S. pneumoniae as well as the specimen taken during the operation. Thus we concluded that this was a case of an ascending infection. Af…

The diagnosis of tuberculous peritonitis may be difficult and elusive. The patient may present with non-specific symptoms of fever, general ill-health or vague abdominal pains. There may be no pulmonary symptoms and the chest X-ray may be normal. The CT scan of the abdomen is sometimes helpful in suggesting the diagnosis. We have found that laparoscopic examination of the abdominal contents and the peritoneum is an effective way to obtain a conclusive diagnosis.

BACKGROUND: Primary peritonitis is a rare cause of the acute abdomen in children. It is often found at laparotomy for suspected appendicitis, or diagnosed by paracentesis in a child with an associated medical disorder.
METHODS: Our experience over the past 5 ++years with this condition at the Royal Children's Hospital is reviewed and compared with a larger series previously published from the same institution 25 years ago.
RESULTS: There were 26 cases from 1989 to 1994; 22 (85%) were female and 6 (23%) of the patients had a major underlying medical disorder.
CONCLUSIONS: The incidence of pr…

The granulomatous peritonitis by ascaris (GPA) is a rare complication of intestinal ascariasis due to the perforation of the digestive tract by the adult worm, which deposits its ova in free peritoneum; therefore, provoking an intense granulomatous inflammatory reaction. We report a case of an 8-year-old boy with GPA, emphasizing the clinical and diagnostical aspects and the therapeutics procedures.

A case of granulomatous peritonitis produced by starch is presented. This is a rare pathology caused by abdominal contamination with starch from surgical gloves during an operation. The symptoms can be very similar to acute abdomen, therefore, it is sometimes necessary to carry out a second operation in order to confirm the diagnosis. A peritoneal biopsy or cytology of the ascitic liquid usually provides the correct diagnosis as the typical starch granules show up. With non-steroid analgesics, or steroids in some cases, the process can controlled.
