Photochemotherapy has been the most successful treatment of vitiligo to date. In this study we tried to evaluate the efficacy, safety and patient acceptability of photochemotherapy (PUVA, or psoralen and ultraviolet A, therapy) for vitiligo patients treated in King Khalid University Hospital over the period of 1987-1994. The treatment success strongly depended on the number of treatments. More than 75% repigmentation was achieved in 42% of the patients who received 100 to more than 200 treatments. Unfortunately, however, only 27% of the patients received this number of treatments. Reasons for …

Photochemotherapy with 8-methoxypsoralenes and PUVA has been found to be effective in the treatment of superficial skin inflammations, such as lichen planus. However, there is little bibliography on the treatment of oral lichen planus with PUVA. A case of oral erosive lichen planus was treated with oral psoralenes (0.6 mg/kg body weight) two hours before ultraviolet exposure. Twenty sessions were held, three weekly, for a total cumulative dose of 35.9 J/cm2. After treatment concluded, clinical symptoms and erosive lesions disappeared. PUVA therapy may be useful for severe forms of erosive oral…

BACKGROUND: Using autologous cryopreserved mononuclear cells (MNCs) for extracorporeal photochemotherapy (ECP) offers several advantages, such as treating patients from geographically distant care centers or maintaining ECP schedule while dramatically reducing number of apheresis sessions. We previously reported that cryopreserved cells retain their immunomodulatory properties when exposed to UVA and psoralen. To date, there are no clinical data on the use of cryopreserved MNCs for ECP ("cryo-ECP").
CASE REPORTS: Three patients were treated by cryo-ECP for refractory dermatomyositis, juvenile…

In this report, we describe the use of extracorporeal photochemotherapy in n the treatment of two patients with rapidly advancing progressive systemic sclerosis. Both patients experienced improvement in the cutaneous as well as the systemic manifestations of their disease while undergoing therapy. The potential therapeutic mechanisms are discussed.

Photochemotherapy, a recently developed method of treatment, is based on the effect of light on a systemically administered photo-active substance. 152 patients with severe generalized psoriasis were exposed, after oral administration of 8-methoxy-psoralen, to a high-intensity UVA light source radiating at a continuous spectrum between 320 and 390 nm (maximum at 365 nm). Radiation doses ranged from 1.4 to 4.8 J/cm2 and were progressively increased. Complete remission occurred in 141 patients, satisfactory improvement in the remaining 11. It required 12+/-6 radiation sessions (-/x +/- s) to obt…

Although traditional nanomedicines have enhanced the therapeutic efficacy and improved the survival quality of cancer patients, random drug release and drug resistance are deep-rooted problems hindering their clinical application. A precise nanoplatform combing chemotherapy and photodynamic therapy (PDT) is developing as a new therapeutic strategy to overcome the above challenges. Herein, a novel supramolecular nanomedicine is ingeniously constructed for in situ self-boosting cancer photochemotherapy. Hydrophilic polyethylene glycol (PEG) chains or β-cyclodextrin (β-CD) hosts are first conjuga…

BACKGROUND: Extracorporeal photochemotherapy (photopheresis), an immune-modulating therapy, has been demonstrated to elicit a therapeutic response in the treatment of several autoimmune disorders. We evaluated the use of photopheresis in the treatment of patients with progressive systemic sclerosis (PSS; scleroderma).
METHODS: Five patients with early-onset, diffuse PSS were treated with photopheresis on 2 successive days monthly for an average of 59 months (range 54-89 months). We initially reported the response this group of patients had to photopheresis treatment at an average of 11 months…

We report a patient with severe epidermolysis bullosa acquisita (EBA) whose disease was refractory to conventional treatments. New bullae continued to develop over greater than 50% of his body surface area despite therapy. His course was complicated by hyperglycaemia, sepsis, hypoxia caused by pulmonary Aspergillus infection and an idiopathic cardiomyopathy. His EBA resolved after treatment with extracorporeal photochemotherapy (ECP). Hence, ECP may be effective in the treatment of severe EBA which has failed to respond to standard treatment regimens.

BACKGROUND: This original cohort of patients with erythrodermic cutaneous T-cell lymphoma (CTCL) was reported to have clinical improvement with photopheresis during the 12 months of the original study. No long-term follow-up data have been available to examine the impact of this therapy on the disease.
OBJECTIVE: Our purpose was to provide long-term follow-up on the original 29 erythrodermic CTCL patients treated with photopheresis and to compare these results with historical controls.
METHODS: Files of patients from the original photopheresis study centers were reviewed and their current st

In nine of 121 patients unusual side effects of PUVA-therapy were observed: hypertrichosis, subungual hemorrhage of fingernails, nevus spilus-like hyperpigmentation, acneiform eruptions, and a photoallergic dermatitis to 8-methoxypsoralen.

Narrowband UVB (NB-UVB) phototherapy and psoralen-UVA (PUVA) photochemotherapy are widely used phototherapeutic modalities for a range of skin diseases. The main indication for NB-UVB and PUVA therapies is psoriasis, and other key diagnoses include atopic eczema, vitiligo, cutaneous T-cell lymphoma (CTCL), and the photodermatoses. The decision on choice of phototherapy is important and NB-UVB is usually the primary choice. NB-UVB phototherapy is a safe and effective therapy which is usually considered when topical agents have failed. PUVA requires prior psoralen sensitization but remains a hig…

Vitiligo is a disorder characterized by the decrease in melanin pigment of skin. This depigmenting disorder has prevalence among worldwide, irrespective of age and sex. There is an existence of different treatment modalities for the management of vitiligo. But irrespective of treatment methods, the main drawback in the management of vitiligo is the occurrence of side effects during the implication of treatment. Among the treatment modalities, photochemotherapy seems to be the better choice of treatment for vitiligo. Photochemotherapy involves the usage of UV rays for photoactivation of the dru…

In a random study of 150 patients with psoriasis vulgaris, oral psoralen photochemotherapy using natural sunlight (PUVASOL) used alone was compared to PUVASOL plus adjunctive topical therapy with tar. The combined PUVASOL and topical therapy with tar in 75 patients (group-I) with 30 minutes sunlight exposure done in every alternate day showed complete clearing of lesions in 68 (90.6%) patients. The average rate of clearance of lesions started to appear 12-24 days with a mean of 18 days. Group I patients who received topical therapy in conjunction with PUVASOL, their skin lesions cleared more q…

A trioxysalen bath is a safe alternative to systemic 8-methoxypsoralen in long-term psoralen plus ultraviolet A (PUVA) treatment. The kinetics of its main side-effect, the strong phototoxicity, has not been thoroughly studied. This study determined the degree and persistence of phototoxicity after a single 10 min bath at a trioxysalen concentration of 0.33 mg/l. The buttock skin of 16 healthy volunteers was irradiated with UVA 10 min, and 1, 3, 9 and 24h after the bath. The minimal phototoxic dose (MPD) was assessed 48, 72 and 96h after the bath. In general, the 96 h reading showed the lowest
