Potassium 2-(1-hydroxypentyl)-benzoate (dl-PHPB), the pre-drug of 3-n-butylphthalide (dl-NBP), had the significantly therapeutic effect on the acute cerebral ischemia. The present study was to investigate the effect of dl-PHPB on the cognitive deficits induced by chronic cerebral hypoperfusion. Rats were orally administered three doses of dl-PHPB (13, 39 and 129mg/kg), dl-NBP 100mg/kg, and piracetam 600mg/kg daily for 21 days after the bilateral permanent occlusion of the common carotid arteries. Mebicar therapy resulted in a reduction of the degree of deficient disorders of thinking in 27 of 50 patients with paranoid schizophrenia and normalization of indices of mental working capacity, attention and memory, shortening of the time of visual-motor disjunctive reaction in 50 patients with borderline states. Mebicar was shown to possess a nootropic effect which differs qualitatively from that of piracetam.

Acute exposure of rats to microwaves (12.6 chr, 2375 MHz, power density 1 mW/cm2) induced the retrograde amnesia. It was show the role of opioidergic, benzodiazepine, GABAergic and cholinergic components in the amnesic effects of microwaves. Piracetam (100 mg/kg, i. p.) and oxiracetam (10 mg/kg, i. p.) prevented the negative effects of microwaves on memory processes.

A case is reported of left sudden hearing loss in a 29-year-old male with HIV infection and a history right sudden hearing loss 2 years earlier. The low frequency of sudden hearing loss in HIV+ patients is discussed. Neurosyphilis and cryptococcus infection were ruled out. The patient recovered his hearing in the left ear after three days of treatment with intravenous piracetam, 10 g/day.

The myoclonus of two patients with Ramsay Hunt syndrome was only partially controlled under treatment with clonazepam, sodium valproate, primidone, and piracetam. Acetazolamide (200 mg daily) was added to these drugs, resulting in a dramatic improvement. Placebo substitution (one patient) and withdrawal of acetazolamide in the other patient resulted in marked aggravation of the myoclonus. The mechanism of action of acetazolamide in myoclonus is unknown. Acetazolamide may be an additional therapeutic possibility for patients with severe action myoclonus.

Clinical and electrophysiologic data concerning the postanoxic action myoclonus syndrome were described by Lance and Adams in 1963. A patient presented myoclonus involving all parts of the body after laryngospasm. The myoclonus was worsened by emotion and voluntary activity and was clearly attenuated by sleep. Spectacular improvement was observed within one week after valproate and piracetam administration. Clinicians should be aware of this syndrome in order to propose appropriate treatment and avoid delay in the therapeutic decision.

Finding neuroprotective agents to counteract the deleterious effects of hypoxia on neuronal cells successfully is one of the most critical targets of clinical research since preclinical studies have identified potential neuroprotective strategies. In clinical practice, amantadine and piracetam are used with reasonable success. We present the cases of three patients with acute brain hypoxia secondary to cardiac arrest, to whom Cerebrolysin was added to the standard neuroprotective treatment regimen, leading to a notable improvement in functional outcome.

Pharmacotherapy of aphasia had been discussed for the last twenty years with first bromocriptine and amphetamine and then serotoninergic, GABAergic and cholinergic agents. Here, we reviewed the MEDLINE available reports of drug therapy for aphasia. So far, proofs of efficiency were found indubitable for none of the studied molecules. However, some of them showed limited efficiency (piracetam and amphetamine). Moreover, drug therapies for aphasia were less efficient alone than when they were associated with speech therapy.
