Basilar impression complicating osteogenesis imperfecta type IV: the clinical and neuroradiological findings in four cases.

OBJECTIVES: To describe the clinical and neuroradiological features of basilar impression in patients with osteogenesis imperfecta type IV. METHODS: Four patients with basilar impression were ascertained in a population study of osteogenesis imperfecta. All four had detailed clinical and neuroradiological examination with both CT and MRI of the craniocervical junction and posterior fossa structures. RESULTS: All four showed significant compression of the posterior fossa structures and surgical decompression was performed with relief of symptoms. CONCLUSION: Symptoms of cough headache and tr

Endoscopic endonasal transclival transodontoid approach for ventral decompression of the craniovertebral junction: operative technique and nuances.

The transoral approach is considered the gold-standard surgical route for performing anterior odontoidectomy and ventral decompression of the craniovertebral junction for pathological conditions that result in symptomatic cervicomedullary compression, including basilar invagination, rheumatoid pannus, platybasia with retroflexed odontoid processes, and neoplasms. Extended modifications to increase the operative corridor and exposure include the transmaxillary, extended "open-door" maxillotomy, transpalatal, and transmandibular approaches. With the advent of extended endoscopic endonasal skull …

Surgical Treatment of Basilar Invagination without Evident Atlantoaxial Instability (Type B) - A Systematic Review.

Chiari 1 malformation (CM1) and Basilar Invagination (BI) are the most common congenital bone and neural disorders of the craniovertebral junction. There is still discussion in the literature regarding best treatment practices for Type B BI. This systematic review aims to evaluate the treatment options for symptomatic type B BI especially concerning the need for craniocervical fusion. We performed a systematic literature review, focusing on surgical studies of patients with type B BI that reported the surgical technique used and the clinical outcomes. The MINORS instrument was utilized for met

Transoral intraarticular cage distraction and C-JAWS fixation for revision of basilar invagination with irreducible atlantoaxial dislocation.

BACKGROUND: The revision surgery of basilar invagination (BI) with irreducible atlantoaxial dislocation (IAAD) after a previous occipitocervical fusion (OCF) is challenging. Transoral revision surgery has more advantages than a combined anterior and posterior approach in addressing this pathology. The C-JAWS is a cervical compressive staple that has been used in the lower cervical spine with many advantages. Up to now, there is no report on the application of C-JAWS in the atlantoaxial joint. We therefore present this report to investigate the clinical outcomes of transoral intraarticular cage

A case of basilar impression complicated with left frontal meningioma.

CASE REPORT: A 75-year-old baikarian woman was admitted to our hospital for treatment of seizures. From the results of neurological and radiological examination, a left frontal meningioma was suspected and the patient was referred to our department for neurosurgical intervention. At admission, the MRI showed a basilar impression accompanied by Klippel-Feil syndrome of C4/5/6/7, but neurological symptoms of basilar impression were absent. Subsequently, the tumor was resected via the left frontal approach using microsurgical technique. Histological examination disclosed fibroblastic meningioma.

Traumatic basilar impression: case report.

A very rare case of traumatic basilar impression is reported. The patient, a 57-year-old man, was hit on the head vertically in the parietal region. X ray of the cervical spine and computed tomography (CT) scans showed intracranial indentation of the atlas and the odontoid process with a depressed fracture around the foramen magnum. There are no previous reports about this type of fracture.

Brainstem auditory evoked potential (BAEP) abnormalities in subjects with craniovertebral malformations.

We report the data on 42 monaural BAEP recordings from 10 patients with craniovertebral malformations. Of 8 preoperative recordings from 3 patients 6 showed an increased I-III interpeak latency (IPL). Of 28 recordings taken shortly (1-12 months) after surgery 21 showed an abnormal I-III IPL. On the other hand, of 6 recordings from 3 patients 20 years after surgery 5 were in the normal range and 1 showed an abnormal I-III IPL. Although the most important site of craniovertebral malformations is the cervicomedullary junction, the results reported suggest unilateral or asymmetrical bilateral invo

Basilar impression in osteogenesis imperfecta. A report of three cases in one family.

Basilar impression is a well-recognised though rare complication of osteogenesis imperfecta. Three patients, all members of the same family, with advanced basilar impression complicating osteogenesis imperfecta tarda, are described. The clinical features in these cases illustrate the natural history of this condition: from asymptomatic ventricular dilatation, through the foramen magnum compression syndrome, to death from brain-stem compression. The radiological criteria on which the diagnosis is based, are defined. Review of the literature reveals only seven previously documented cases, all in

Chromosome 22q11.2 deletion and phenotypic features in 30 patients with conotruncal heart defects.

This report describes the dysmorphic features and frequency of 22q11.2 deletion (del22q11) in 30 Turkish patients with conotruncal heart def … situs inversus dextrocardia, DORV, pulmonary stenosis, and VSD. Radiographs of this patient showed platybasia, complete fusion of C2-C3, and posterior fusion of the T1-T2 vertebrae. This particular case indicates that the phenotypic features of del22q11 and OAVS may overlap.

Angular craniometry in craniocervical junction malformation.

The craniometric linear dimensions of the posterior fossa have been relatively well studied, but angular craniometry has been poorly studied … Chiari malformation group does not show significant differences when compared with normal controls. Platybasia occurred only in basilar invagination and is suggested to be more prevalent in type II than in type I. Platybasic patients have a more acute clivus canal angle and show greater cervical lordosis than non-platybasics. The Chiari group does not show s …

A Novel Technique for Basilar Invagination Treatment in a Patient with Klippel-Feil Syndrome: A Clinical Example and Brief Literature Review.

Objectives and Background: To present a novel technique of treatment for a patient with basilar invagination. Basilar invagination (BI) is a congenital condition that can compress the cervicomedullary junction, leading to neurological deficits. Severe cases require surgical intervention, but there is debate over the choice of approach. The anterior approach allows direct decompression but carries high complication rates, while the posterior approach provides indirect decompression and offers good stability with fewer complications. Materials and Methods: A 15-year-old boy with severe myelopath

The role of clivus length and cranial base flexion angle in basilar invagination and Chiari malformation pathophysiology.

BACKGROUND: The craniovertebral junction is an anatomically well-defined transitional zone located between the skull and the cervical spine. Multiple malformations can affect this region with the most prominent being basilar invagination (BI) and Chiari malformation (CM). Despite numerous studies, the origin, pathophysiology, and classification of these pathologies remain controversial. The objective of this study was to evaluate the implication of cranial base flexion angle and clivus length in the development of these conditions. METHODS: Midline tomography and magnetic resonance imaging of

Cerebellar infarctions secondary to cranio-cervical anomalies: a case report.

We report a case of cerebellar infarctions which occurred in the territories of the bilateral posterior inferior cerebellar arteries. This case was complicated with cranio-cervical anomalies composed of assimilation of the atlas, atlanto-axial dislocation, and basilar impression. The 40-year-old male patient had no detectable risk factors predisposing to atherosclerotic arterial occlusion or cardiogenic embolism, and there were no angiographic findings of atherosclerosis. It was, therefore, postulated that the cerebellar infarctions were secondary to those cranio-cervical anomalies. The develo

After a review of the literature and an analysis of the clinical and radiographic features of basilar impression (b.i.) the authors describe 27 personal cases. In 6 b.i. was present in its pure form and in 21 it was associated with other anomalies at the cranio-vertebral junction. Because of the frequent misdiagnosis with multiple sclerosis or other chronic neurologic diseases and because of the possible improvement of symptoms following decompressive occipital craniectomy and cervical laminectomy, the importance of a correct and complete radiographic study of the cranio-vertebral junction in
