Patient and Family Perspectives on Statutory Duty of Candour Following Serious Adverse Patient Safety Events.

BACKGROUND: The Statutory Duty of Candour (SDC) legally requires healthcare organisations to notify patients, families, and carers who experience a serious adverse patient safety event (SAPSE), provide an apology, and explain steps taken to prevent recurrence. However, despite growing international adoption of SDC, evidence on its implementation and impact on patients and families ('consumers') remains limited. This study examined consumer awareness, understanding, and lived experiences of SDC in Victoria, Australia, two years after its introduction. METHODS: A mixed-methods study integrated

Caregiver bodywork: family members' experiences of caring for a person with motor neurone disease.

AIM: This paper reports a study of how family members caring for people living with motor neurone disease managed the deteriorating body, their own bodywork and the associated emotional labour. BACKGROUND: People living with the neurodegenerative condition of motor neurone disease face the prospect of dying in 3-5 years from progressive loss of voluntary muscle mass and function, culminating in respiratory failure. Theories concerning the body in illness have been used to illustrate patient perspectives; however, family caregivers' experiences of the body have been neglected. METHOD: An ethn

Assisted Dying Makes Us Think Differently About Care.

Assisted dying (AD) is increasingly available as a legal end-of-life option in wealthy nations. Literature on AD considers its operation and the experience of clinicians, people receiving AD, and their loved ones. It is inward-looking. The nature of AD suggests it focuses less on the technical aspects of care and more on its humanity. Accordingly, AD offers lessons for healthcare more broadly-specifically, the importance of patient- and family-centred care, respect for patient wishes, and relational care. LIVED EXPERIENCE: JH is a family member of a person who received assisted dying. The vign

Effectiveness of family-provider communication interventions on family's psychological health, communication quality, and health care utilization in the ICU: A systematic review and meta-analysis of randomized controlled trials.

OBJECTIVE: This review aims to assess evidence regarding the effectiveness of family-provider communication interventions. METHODS: We searched six databases (PubMed, PsycINFO, Web of Science, EMBASE, CINAHL and the Cochrane Library). Two researchers independently extracted data and assessed bias. Data were analyzed using Stata software and Review Manager. RESULTS: Nineteen studies were included. Communication interventions had a moderate effect on family needs (SMD = -0.43, 95 % CI: -0.81 to -0.06, P = 0.025), small effects on 6-month psychological distress (Overall: SMD = -0.14, 95 % CI: -

Communicating to Patients and Families About Post-Intensive Care Syndrome.

Millions of people around the world survive critical illness each year only to realize that they and their loved ones are grappling with a new "normal" after hospital discharge for which their medical team may not have adequately prepared them. Up to one-half of all ICU survivors suffer from new or worsening impairments in physical, cognitive, and psychological domains of health that are often not realized until they attempt to re-enter their previous lives. These devastating long-term sequelae of critical illness, collectively described as post-intensive care syndrome (PICS), can carry enormo

Life history and meanings: end of life in an intensive and semi-intensive care unit.

This study addressed the end-of-life experience of a patient admitted to an intensive and semi-intensive care unit based on the accounts of a family member who accompanied her therapeutic journey. The study sought to understand how healthcare teams consider patients' life histories and how this influences care strategies. We also sought to identify the interaction between healthcare teams and patient family members when patients are no longer able to communicate. Immersion in an intensive and semi-intensive care unit enabled an ethnographic approach involving participant observation, in-depth

Overview: cerebrovascular accident and the hospitalized elderly--a multidimensional clinical problem.

A cerebrovascular accident is unwelcome at any age, but for the elderly it involves concerns. The abrupt onset of a cerebrovascular accident signals anew the already present fears of loss of control, death, insanity, disfigurement, loss of physical function, and sexual impairment. The accident can also result in worry about the possibility of explosive recurrence, disruption of thoughts and emotions, lengthy treatment away from home, and exhaustion of retirement funds. Therapeutic endeavors must be eclectic and individually tailored to address the special needs of the elderly patient, the high

End-of-life care and dying: issues raised by staff supporting older people with intellectual disability in community living services.

BACKGROUND: The aim of this study was to explore the current status of end-of-life care and dying of people with intellectual disability based on the experiences of staff in community living services. MATERIALS AND METHODS: Focus groups and individual interviews were conducted, guided by grounded theory methodology. RESULTS: The current status of end-of-life care and dying comprised five key 'issues': knowledge of dying, ethical values, the where of caring, the how of caring and post-death caring. These issues occurred in relationship with 'partners', including the dying person, other client

Empowering Young People Living With Juvenile Idiopathic Arthritis to Better Communicate With Families and Care Teams: Content Analysis of Semistructured Interviews.

BACKGROUND: Young people living with juvenile idiopathic arthritis (JIA) face a number of communication barriers for achieving optimal health as they transition from pediatric care into adult care. Despite growing interest in mobile or wireless technologies to support health (mHealth), it is uncertain how these engagement tools might support young people, their families, and care teams to optimize preference-based treatment strategies. OBJECTIVE: This study aims to examine how an mHealth patient support system (mPSS) might foster partnership between young people living with JIA, their familie

Shared Decision-Making for Children with Medical Complexity and Their Caregivers in the Perioperative Time.

Many decisions faced by children with medical complexity during the perioperative period do not have a clear best treatment option and thus must balance the clinician's knowledge and the families' treatment preferences and values. Shared decision-making is a communication process through which providers and families can make high-quality collaborative decisions that consider the child's best interests. The objective of this article is to define the core elements of shared decision-making, discuss aspects of shared decision-making unique to children with medical complexity, and describe practic

"All the magic happens in between therapy sessions": Speech-language pathologists' experiences of collaborative approaches with families.

PURPOSE: Interest is growing for collaborative approaches in speech-language pathology. How these approaches can be achieved with families is less commonly discussed in research. This study explores speech-language pathologists' perspectives and experiences in implementing collaborative approaches with children and families in clinical and research settings. METHOD: An online survey was conducted to explore questions: 1) What do speech-language pathologists perceive to be the core elements of collaborative approaches with children and families? and, 2) How do speech-language pathologists impl

The photography of neonatal bereavement at Wythenshawe Hospital.

A large number of babies are stillborn or die in the neonatal period in Britain every year. This is only a small percentage of births overall, but these babies represent a considerable loss to the parents, and their hopes and ideals both for themselves and for their child. One of the key aids to grieving is memory, and in those cases where babies are born dead or die in utero, a history of the baby needs to be created. Photography can help create this history, and bereaved parents and families have expressed the importance of photographic images in assisting with their grieving process.

Initial evaluation of suspected dementia. Asking the right questions.

The initial clinical approach to suspected dementia involves asking the right questions of the patient and a knowledgeable family member or caregiver. These questions should focus on the nature of the cognitive problem (e.g., the onset, duration, and severity of memory difficulties). The clinical interview is supplemented with thorough examination of the patient's physical, neurologic, mental, and functional status. Once a diagnosis of dementia is established, the underlying cause should be identified by a standardized workup, including laboratory testing and, in most cases, neuroimaging. Gene
