The surgical suite.

In this short essay I focus on two ethnographic scenes from the operating room in order to show the OR as a place both assiduously sterile and deeply intimate. In the process of these Facial Feminization Surgery procedures, surgeons and other OR staff reflect on the social changes that the surgery may (or may not) enable even while they are working to enact physical changes in the faces of their patients.

Importance of education and the role of the patient and family in the care of anorectal malformations.

During this early part of the 21st century online technology has prompted many major advances in medical care. In this section we argue that this is particularly evident in the treatment and care of patients born with Anorectal Malformation (ARM) and Hirschsprung's Disease (HD). Our stories show that anyone born with these complex colorectal conditions in the 20th century was destined to a life of isolation and stigma. Here we explore the lack of understanding and recognition of the psychological effects on children and families which has characterised this period. We show that advances in cli

Dual relationship activities: principal component analysis of counselors' attitudes.

The British Columbian Members of the Canadian Guidance and Counselling Association were surveyed to explore their attitudes regarding dual relationships. Of 529 deliverable surveys, 206 usable returns yielded a response rate of 39%. The survey instrument collected data regarding respondents' characteristics and ethicality ratings of 39 dual relationship activity items. An exploratory principal components analysis was performed on responses, resulting in a 4-factor equation, which accounted for 44% of the total variance. The results suggest that, although conceptual considerations of dual relat

Facilitating patient-professional dialogue about complementary and alternative medicine in routine cancer care: A scoping review of interventions.

OBJECTIVE: To provide an overview of interventions developed to facilitate dialogue about complementary and alternative medicine (CAM) between patients with cancer and health care professionals in routine cancer practice. METHODS: Following Joanna Briggs Institute (JBI) methodology and reported according to PRISMA-ScR guidelines, nine databases were systematically searched for publications presenting or evaluating interventions that facilitate CAM-related communication in cancer care. Data were extracted on intervention type, target group, delivery format, evaluation methods, reported evaluat

Setting up a new team of support staff for people with mild intellectual disability or borderline intellectual functioning and severe challenging behaviour: A concept mapping study.

BACKGROUND: Studies about teams of staff supporting people with intellectual disability have focused on team performance of existing teams. This study aimed to examine important factors in the process of setting up a new team of support staff. Specifically, we considered the process for a team that supports service users with mild intellectual disability or borderline intellectual functioning who display severe challenging behaviour from the orthopedagogical perspective (i.e., with a focus on contextual factors). METHOD: Three participant groups (service users, support staff, and professional

Conceptualisation of Empathy in Interactions Between Healthcare Professionals and People With Fibromyalgia Syndrome: A Mixed-Methods Study.

Psychological and relational processes, including empathy, are increasingly recognised as central to effective pain care. Fibromyalgia Syndrome (FMS), a complex chronic pain condition, poses significant challenges for both patients and healthcare professionals (HCPs) across medical settings. Patients with FMS often report lack of empathy from HCPs, negatively affecting healthcare experiences. Whilst empathic therapeutic relationships are linked to improved satisfaction and reduced pain in FMS, how empathy is conceptualised in practice remains underexplored. Using Q-methodology, 20 HCPs and 20

Alternative Visions for Pastoral Work with LGBTQ Individuals, Families, and Communities: A Response.

Multiple theological perspectives provide frameworks for pastoral work with lesbian, gay, bisesxual and trans individuals, families, and communities. One model is offered by those who argue for celibacy or heterosexual marriages for those who self-identify as part of LGBTQ communities. This article names other theologically grounded perspectives with the goal of inviting practitioners to broaden their understandings and wrestle with the implications of their theological and ethical stances. When reflecting on the intersection of spirituality and sexuality, the meaning of theological terms such

The role of the analyst in the analytic cure during times of chronic crises.

The role of the analyst in psychoanalytic treatment during periods of chronic crises is illustrated with material from two case studies. The first clinical vignette shows an analyst able to stay with fears evoked in the patient by the traumatic external reality, even as the analyst tried to explore with the patient an inner universe that handled this reality in unique ways. The second case study focuses on how the analyst's countertransference during this period of chronic crises, which she was experiencing along with the patient, made it difficult for her to contain the patient's fears and an

Client Perspectives of Premature Discontinuation in Couple Therapy.

Premature discontinuation, defined as unilateral termination before goal completion against therapist advice, affects approximately 36% of couples who begin therapy. While research often examines factors predicting premature discontinuation, few studies directly ask clients their reasons for stopping. This study explored these reasons by surveying 160 individuals (61% female, 38% male, 1% other; primarily aged 25-44; 92% heterosexual relationships; median income $50-60k) who prematurely discontinued couple therapy without discussing the decision with their therapist. The sample was predominant

Breadth psychology.

Freudian psychoanalysis holds a conception of personhood in which patients are divided by a conflict between their outward presentation and their hidden, inner turmoil. The Freudian analyst too is split, occupying a position within the patient's internal dialogue while also listening in from outside of it. Addressing the analyst as both self and other, the patient's confidentiality is necessarily preserved but also inherently violated. As digital technologies reshape psychic structure, this paradox in confidentiality, constitutive of the analytic situation, may no longer arise. Personhood in t

The smoothness advantage: Session quality and symptom change in a training clinic.

While both emotional depth and relational smoothness are considered therapeutic, their relative contributions to symptom change remain debated. This study disentangles the within-person (state) and between-person (trait) effects of client-rated session depth and smoothness on symptom trajectories in a university training clinic. Utilizing longitudinal data from 84 clients (1,338 session observations) treated by 33 novice therapists, this study employed multilevel modeling with two complementary specifications: an autoregressive model testing immediate session-to-session effects and a baseline-

Notes on the Analytic True Self for Another.

This personal essay explores the core of the author's analytic way of being and relating. This fundamental place he calls the "analytic true self for another," an aspirational engagement that embodies two ethical intentions: being present and relating with true feeling. To be present is what Gabriel Marcel names presence and availability: a porousness in the analyst, an openness to the influx of the patient's emotions and states of mind that the analyst inhabits. True feeling, a phrase after the painter Robert Motherwell, describes the creative process of acting from a place of freedom, honest

The impact of incontinence on mental health.

Bladder and bowel issues, particularly those related to continence, can significantly impact an individual's mental health. These challenges often lead to isolation, anxiety, stress, depression and social anxiety, all of which can profoundly affect a person's quality of life. Healthcare professionals are uniquely positioned to recognise these concerns and incorporate them into assessments for bladder and bowel health. By actively listening, offering empathy and guiding patients toward further support-alongside providing practical treatment and management strategies-clinicians can play a vital

The first session matters: Therapist responsiveness and the therapeutic alliance in the treatment of borderline personality disorder.

OBJECTIVES: The focus of the present research is to investigate the impact of therapist responsiveness at the very first session of therapy on the evaluation of therapeutic alliance from the therapist's perspective and from patient's perspective in the context of guideline-based treatment for borderline personality disorder. DESIGN: The study has a correlational and longitudinal design applied to a 10-session therapy in a naturalistic setting. METHODS: A total of four trained raters evaluated therapist responsiveness during the first session of therapy. After each therapy session, therapists

Alliance rupture profiles by personality disorder pathology in psychotherapy for depression: Tendencies, development, and timing.

OBJECTIVE: Clinical and theoretical considerations presume that patients with different personality disorder (PD) clusters will be associated with distinct alliance rupture profiles; however, there is scarce empirical literature examining this. The present study adopted a systematic framework for investigating profiles of alliance ruptures for individuals belonging to each of the three PD clusters. METHOD: The sample consisted of 94 patients from a randomized controlled trial for treatment of depression. PD cluster features were assessed at intake and ruptures were assessed across treatment.

How therapists' interpersonal behaviour is perceived by their patients and close others: A longitudinal and cross-situational study.

Due to their predictive abilities, therapist interpersonal behaviour is of great relevance for psychotherapy. However, there is a lack of knowledge about its stability inside but also outside of the therapy room within and between therapists. The current study investigates interpersonal behaviour of trainee therapists (N = 20) as perceived by four patients each suffering from generalized anxiety disorder and three closely related persons of every therapist (close others). Investigating repeated measures, four patients per therapist completed the Impact Message Inventory (IMI; Kiesler, 1987) th

Clown's view as respiciō: looking respectfully to and after people with dementia.

Clowns seem suspect when it comes to respect. The combination of clowning and people with dementia may seem especially suspicious. In this argument, I take potential concerns about clowning in dementia care as an opportunity to explore the meaning of a respectful approach of people with dementia. Our word 'respect' is derived from the Latin respiciō, meaning 'looking back' or 'seeing again', as well as 'looking after' or 'having regard' for someone or something. I build upon this double meaning of respiciō by examining how simultaneously we look to and after people with dementia. I do so empir
