Atypical presentation of xanthogranulomatous pyelonephritis: a case report.

Xanthogranulomatous pyelonephritis (XPN) is a rare form of chronic pyelonephritis, which is usually caused by calculous obstructive uropathy. We present a previously healthy 45-year-old housewife, who was admitted to The Dammam Central Hospital, Dammam, Saudi Arabia with left loin pain and increased frequency of micturition of four days duration. She also had icterus and features of disseminated intravascular coagulation. Abdominal ultrasound and computed tomography of the abdomen was suggestive of XPN. She responded well to treatment with antibiotics and nephrectomy. Histology of the resected

Feasibility of laparoscopic approach in management of xanthogranulomatous pyelonephritis.

OBJECTIVES: To report the feasibility of the laparoscopic approach for management of xanthogranulomatous pyelonephritis (XGP). METHODS: From 2002 to 2005, 11 patients underwent laparoscopic (n = 7) or hand-assisted laparoscopic (n = 4) nephrectomy by a single surgeon for pathologically confirmed XGP. The preoperative presentation, operative details, postoperative recovery, and complications were recorded. RESULTS: A total of 5 men and 6 women were included in this study, and the mean patient age was 51 years (range 28 to 78). All cases began with the laparoscopic approach (6 left and 5 right

Is laparoscopic nephrectomy the preferred approach in xanthogranulomatous pyelonephritis?

OBJECTIVES: To report our experience with laparoscopic nephrectomy in patients with xanthogranulomatous pyelonephritis (XGP) and compare it with the open approach. XGP is an atypical and severe form of chronic renal infection. Open nephrectomy has been the procedure of choice for XGP. The laparoscopic approach has been used in cases of XGP but with increased complications and operative time. METHODS: A retrospective analysis of all the patients who underwent laparoscopic nephrectomy with a pathologic diagnosis of XGP from January 2001 to August 2005 was done. The demographic profile and data

Laparoscopic and open surgical nephrectomy for xanthogranulomatous pyelonephritis.

BACKGROUND AND PURPOSE: Xanthogranulomatous pyelonephritis (XGP) is a severe, chronic renal-parenchymal infection. Nephrectomy is the treatment of choice. Because of the renal and perirenal inflammatory changes that commonly accompany XGP, the laparoscopic approach is difficult. We compared our experience with laparoscopic and open surgical nephrectomy for XGP. PATIENTS AND METHODS: A retrospective chart review of all adult nephrectomy specimens with the pathologic diagnosis of XGP between January 1997 and May 2003 was performed. Preoperative presentation, operative details, and postoperative

Diffuse xanthogranulomatous pyelonephritis in a child with severe complications.

Xanthogranulomatous pyelonephritis is a rare disease in childhood. Because the symptoms and signs are chronic and non-specific, preoperative diagnosis is usually difficult. We report an 8-year-old boy who had an abdominal mass and anemia for more than 6 months. Fever and dyspnea occurred 4 days prior to admission. Ultrasonography revealed an enlarged right kidney with multiple parenchymal hypoechogenic areas, absence of normal parenchymal structures, and perinephric thickening with multiple calcifications. An abdominal computed tomogram demonstrated an irregular, enlarged right kidney with mul

Primary renal lymphoma and xanthogranulomatous pyelonephritis in childhood.

Primary renal non-Hodgkin's lymphoma is very rare in childhood. A six-year-old boy presented with bilateral non-obstructive multinodular nephromegaly and renal failure. Percutaneous needle biopsy showed large-cell lymphoma. The patient was started on chemotherapy. A right nephrectomy was done when systemic hypertension developed in the presence of a non-functional right kidney. Histopathologic examination revealed focal lymphomatous infiltration and xanthogranulomatous pyelonephritis which is an atypical form of chronic renal infection. The case is discussed in relation to previons reports.

[Xanthogranulomatous pyelonephritis. Our experience and review of the literature].

BACKGROUND: Ten cases of xanthogranulomatous pyelonephritis (XGP) with diagnostic and therapeutic observation are described and other experiences reported in the literature are analyzed. METHODS: The clinical, laboratory and radiological features of 10 patients who had undergone nephrectomy in our Istitute, between October 1993 and March 2000, with a postoperative pathological diagnosis of XGP have been retrospectively re-evaluated. RESULTS: The most frequent symptom was flank pain. All the patients presented leucocytosis. Urine culture was positive for Proteus Mirabilis in four patients (40

Xanthogranulomatous pyelonephritis in childhood: report on an unusual case.

A case of localised xanthogranulomatous pyelonephritis (XPN) in a 4-year-old female with an unusual radiological picture is reported. The young girl was suspected to have a liver tumour because of a hypervascularised area found by arteriography in the hepato-diaphragmatic region. Laparotomy disclosed a large mass in the upper pole of the right kidney which was adherent to the liver and to the diaphragm. Excision of the large mass including upper pole of the kidney was carried out. Recovery was uneventful. Pathological examination revealed the XPN disease pattern.

Xanthogranulomatous pyelonephritis complicating sepsis and abdominal compartment syndrome.

INTRODUCTION AND IMPORTANCE: Xanthogranulomatous pyelonephritis (XGP) is a rare chronic kidney infection characterized by tissue destruction and lipid-laden macrophage infiltration. While sepsis is a known complication, abdominal compartment syndrome (ACS) is extremely uncommon. This case underscores the importance of early recognition and timely surgical intervention in managing life-threatening sequelae of XGP. PRESENTATION OF CASE: A 43-year-old woman with a history of substance abuse and recurrent urinary tract infections presented with septic shock and abdominal distension. Imaging showe

Robotic Heminephrectomy for Xanthogranulomatous Pyelonephritis in a Pediatric Patient.

Xanthogranulomatous pyelonephritis (XGP) is a rare, chronic, destructive inflammatory condition of the kidney characterized by progressive renal parenchymal loss and replacement with lipid-laden macrophages, granulomatous inflammation, and fibrosis. In adults, XGP most commonly affects middle-aged women and is typically associated with urinary tract obstruction, nephrolithiasis, and recurrent infection. In contrast, pediatric XGP is exceptionally uncommon, with reported cases largely limited to isolated case reports and small case series. Pediatric presentations are frequently atypical, often

Simple nephrectomy for Xanthogranulomatous pyelonephritis - Is it really simple?

We present a retrospective review of histopathologically confirmed cases of Xanthogranulomatous pyelonephritis (XGP), which is a rare condition associated with high morbidity and mortality. Simple nephrectomy, though technically not simple, remains the definitive treatment. It is essential pre-operatively to diagnose or have a high index of suspicion of XGP to ensure the best outcome for these patients.

Xanthogranulomatous Pyelonephritis in a Neonate Masquerading as Renal Mass.

Xanthogranulomatous pyelonephritis (XGP), a variant of chronic obstructive pyelonephritis, is a very rare diagnosis in neonates with three reported cases to date. It is often misdiagnosed in infancy as it mimics the features of renal mass. Herein, we report a case of 20-day-old neonate with XGP presenting as a renal mass.

Systemic Amyloid A Amyloidosis Secondary to Xanthogranulomatous Pyelonephritis.

The combination of systemic amyloid A (AA) amyloidosis and xanthogranulomatous pyelonephritis (XGP) resulting from a chronic urinary tract infection is extremely rare. We herein report a case of systemic AA amyloidosis secondary to XGP for which clinical remission developed after nephrectomy. To our knowledge, this is the first case report describing the clinical improvement of systemic AA amyloidosis secondary to XGP after nephrectomy in Japan. Clinicians should be aware of this uncommon combination and search for amyloid depositions in cases of XGP.

Nephrectomy for xanthogranulomatous pyelonephritis-a not-so-simple solution.

BACKGROUND: Xanthogranulomatous pyelonephritis (XGP) is a rare chronic inflammatory condition of the kidney, associated with high patient morbidity, often requiring targeted antibiotic therapy and surgical removal of the affected kidney. AIM: We report the outcomes of patients undergoing nephrectomy for XGP in our institution over a 12-year period. METHODS: Following ethical approval, a retrospective review of histological samples of renal tissue demonstrating features of XGP from June 2010 to 2022 was conducted. Laboratory, imaging, and clinical data of included participants were collected.
