OBJECTIVE: To introduce the Intersegmental Pudendal Reflex (IPR) and evaluate its potential as a surrogate functional marker for S2-S4 integrity and for extended suprasegmental levels during Intraoperative Neurophysiological Monitoring (IOM).
METHODS: Observational retrospective study involving twenty patients undergoing spinal surgery. Pudendal nerve stimulation elicited sacral and suprasacral responses. Reflexive responses were recorded simultaneously in the external anal sphincter (bulbocavernosus reflex, BCR) and the abdominal oblique muscles (IPR).

We report on a 35-year old women with severe abdominal pain, cramps, diarrhea with blood and a palpable paraumbilical resistance. The symptoms appeared a few days before admission and were recurrent within the last two years. The abdominal ultrasound showed a target sign with a thickened wall from the right to the left colon flexure with inhomogeneous reflexes. This paper describes an abomasal body-antral excitatory reflex in acute experiments in sheep anaesthetized with chloralose. The abomasum was either surgically intact or acutely transected to form separate compartments of the abomasal body and antrum. Inflation of a balloon situated in the abomasal body and active contraction of the abomasal body increased the amplitude of antral contraction. Percutaneous endoscopic gastrostomy (PEG) was performed in 33 patient with head injury, one with laryngeal cancer, and one with gastric volvulus. The gastrostomy tube was prepared from 20 F Foley catheter and a plastic micropipette tip. The complications encountered included peritubal leak in three patients (9%) and abdominal wall hematoma in one patient (3%). There was no procedure-related mortality. We recommend PEG for tube enteral feeding in patients who have lost the swallowing reflex.

Twenty one cases of delayed diagnosis of ruptured diaphragm caused by closed trauma are reported: 14 on the left side and 7 on the right side. The clinical signs and the modes of presentation are non-specific. Although the history of thoraco-abdominal trauma and the chest x-ray are sufficient to establish the diagnosis of rupture of the left hemidiaphragm, they can only suggest the diagnosis in cases of righ-sided rupture. The mechanism of rupture is more often due to sudden reflex contraction of the diaphragm against a closed glottis than to excessive abdominal pressure caused by the trauma.

SUMMARY: Hepatojugular reflux is an important, prognostic clinical sign in the field of medicine. The majority of medical students or clinicians may attempt to elicit this sign during abdominal or cardiovascular examinations. However, some individuals may incorrectly call it the hepatojugular 'reflex' rather than a 'reflux'.
