Mitochondrial ROS regulate thermogenic energy expenditure and sulfenylation of UCP1.

Brown and beige adipose tissues can dissipate chemical energy as heat through thermogenic respiration, which requires uncoupling protein 1 (UCP1). Thermogenesis from these adipocytes can combat obesity and diabetes, encouraging investigation of factors that control UCP1-dependent respiration in vivo. Here we show that acutely activated thermogenesis in brown adipose tissue is defined by a substantial increase in levels of mitochondrial reactive oxygen species (ROS). Remarkably, this process supports in vivo thermogenesis, as pharmacological depletion of mitochondrial ROS results in hypothermia

Cellular hypertrophy in cardiomyopathic patients is associated with lower creatine-stimulated mitochondrial respiration.

The mitochondrial respiration rate and morphometric indices in endomyocardial biopsy samples were measured in 43 patients with dilated cardiomyopathy selected in accordance to WHO criteria by endomyocardial biopsy studies after excluding of various forms of myocarditis, alcoholic cardiomyopathy and other specific diseases of the heart. A group of 13 patients with unusually high mean myocyte diameter, 30 +/- 4 microns, and nuclear size, 57 +/- 5 microns, was selected. The remainder of patients (n = 30) had significantly lower mean myocyte diameter and nuclear size, 23 +/- 3 and 42 +/- 6 microns

Intracranial pressure during recovery from nitrous oxide and halothane anaesthesia in neurosurgical patients.

Seven patients with intracranial disorders were studied during recovery from anaesthesia with nitrous oxide and halothane. Arterial, intracranial, and central venous pressure, and arterial carbon dioxide tension were measured and compared with the patient's clinical state. No patient had evidence of increased brain volume when the dura was closed. All had been hyperventilated during the surgical procedure. Cessation of hyperventilation and the continued administration of anaesthetics was followed by a moderate increase in intracranial pressure and a reduction in cerebral perfusion pressure, bu

Peak high-frequency HRV and peak alpha frequency higher in PTSD.

Posttraumatic stress disorder (PTSD) is difficult to treat and current PTSD treatments are not effective for all people. Despite limited evidence for its efficacy, some clinicians have implemented biofeedback for PTSD treatment. As a first step in constructing an effective biofeedback treatment program, we assessed respiration, electroencephalography (EEG) and heart rate variability (HRV) as potential biofeedback parameters for a future clinical trial. This cross-sectional study included 86 veterans; 59 with and 27 without PTSD. Data were collected on EEG measures, HRV, and respiration rate du

Investigating neural impairments in psychotic disorders using electroencephalography and cortical spheroids.

Synaptic dysfunction is a candidate mechanism in psychotic disorders, yet the precise underlying substrates remain elusive. We investigated how combining in vivo electroencephalography (EEG) and in vitro human cortical spheroid (hCS)-based methods can further our understanding of psychosis pathophysiology during fetal stages of neurodevelopment. Ten individuals with schizophrenia (SZ) or bipolar disorder (BD; 5 males and 5 females) and five controls (CTRL; 3 males and 2 females) underwent EEG assessments, including long-term potentiation (LTP)-like cortical plasticity and mismatch negativity (

Proper positioning eliminates patient injury.

1. Pre-existing disorders such as poor nutritional state, diabetes, respiratory disorders arthritis, and cardiac status compound the effects of positioning. 2. Under anesthesia, the patient may be placed in a potentially injurious position that he could protect himself against otherwise. 3. Position influences respiration by mechanical restriction of the ribs and sternum, and by reduced ability of the diaphragm to push down against the abdominal contents. 4. Informed involvement of the operating room nurse will minimize risk to the patient who is defenseless under anesthesia.

Sleep respiratory arrhythmias in control subjects, narcoleptics and non-cataplectic hypersomniacs.

Normal subjects may present central-type apneas or periodic respiration during sleep (stages I and II and paradoxical sleep). The importance of these respiratory disorders increases with age. Hypersomniac patients can manifest either similar or more significant sleep respiratory disorders than normal subjects. The presence of cataplexy or obesity does not permit the prediction of the existence of respiratory arrhythmias or of their type. Sleep respiratory arrhythmias of central type are not likely to cause hypersomnia; however, an aggravating role may be played by obstructive apneas.

From antioxidant hype to mitochondrial reality: Quercetin modulates cardiac redox balance but fails to restore mitochondrial respiration parameters in Zucker diabetic fatty rats.

Cardiovascular diseases are strongly associated with metabolic disorders; particularly type 2 diabetes mellitus (T2DM), which profoundly alters cardiac energy metabolism. Quercetin (QCT), a dietary polyphenol with antioxidant and cardioprotective properties, has been shown to improve mitochondrial function in various disease settings. This study evaluated the effects of chronic QCT supplementation (20 mg/kg/day for 6 weeks) on antioxidant capacity and bioenergetic parameters in cardiac tissue of 6-month-old male Zucker Diabetic Fatty (ZDF) rats. As expected, T2DM caused significant hyperglycae

Needle electromyography of the diaphragm.

A method is described for performing needle electromyography of the diaphragm which is safe and causes little discomfort. It provides valuable information concerning neuropathies and myopathies which may affect the diaphragm, and complements information derived from phrenic nerve conduction studies. Firing patterns of motor unit potentials during spontaneous respiration identify upper motor neurone disorders causing respiratory insufficiency.

The English bulldog: a natural model of sleep-disordered breathing.

To establish a natural model of sleep-disordered breathing, we investigated respiration during wakefulness and sleep in the English bulldog. This breed is characterized by an abnormal upper airway anatomy, with enlargement of the soft palate and narrowing of the oropharynx. During sleep, the animals had disordered respiration and episodes of O2 desaturation. These were worst in rapid-eye-movement (REM) sleep, with most bulldogs having O2 saturations of less than 90% for prolonged durations. In contrast, control dogs never desaturated. In REM sleep, the bulldogs had episodes of both central and

Leucine inhibits degradation of outer mitochondrial membrane proteins to adapt mitochondrial respiration.

The mitochondrial proteome is remodelled to meet metabolic demands, but how metabolic cues regulate mitochondrial protein turnover remains unclear. Here we identify a conserved, nutrient-responsive mechanism in which the amino acid leucine suppresses ubiquitin-dependent degradation of outer mitochondrial membrane (OMM) proteins, stabilizing key components of the protein import machinery and expanding the mitochondrial proteome to enhance metabolic respiration. Leucine inhibits the amino acid sensor GCN2, which selectively reduces the E3 ubiquitin ligase cofactor SEL1L at mitochondria. Depletio

[A case of emergency admission for CO2 narcosis in a patient with amyotrophic lateral sclerosis].

A 68-year-old man with severe dyspnea was admitted as an emergency case. He had no past history of any respiratory or neuromuscular diseases. Immediately after insufflation of oxygen, respiratory arrest occurred. The blood gas analysis showed hypoxemia and severe hypercapnia (PaO2; 32 mmHg, PaCO2; 127 mmHg). We diagnosed as CO2 narcosis, and he was treated with a respirator in the ICU. He showed nonflaccid bilateral diaphragmatic paralysis and muscle atrophy of the upper extremities. As the EMG showed giant spikes of neurogenic pattern, he was diagnosed as ALS. Weaning from the respirator fail

CLINICAL CASE OF THE INNATE CENTRAL HYPOVENTILATION SYNDROME IN THE PARTURIENT.

The syndrome "of the curse of Undina" or the innate central hypoventilation syndrome with the second disorder of cen- tral nervous system is the result of the disrupted vegetative control of the respiration in the absence of neuromuscular diseases and disturbances of the mechanics of respiration. In the clinical practice diagnostics of this state is complex, frequently the cases remain not diagnosed. Taking into account clinical manifestations and depending on the degree of their intensity, early diagnostics, which warns the undesirable consequences ofthe episodes ofhypoxia and hypercapnia, th

Intrinsic factors contributing to elevated intra-abdominal pressure.

Pelvic floor disorders affect 24% of US women, and elevated intra-abdominal pressure may cause pelvic injury through musculoskeletal strain. Activity restrictions meant to reduce pelvic strain after traumatic events, such as childbirth, have shown little benefit to patients. Reported high variability in abdominal pressure suggests that technique plays a substantial role in pressure generation. Understanding these techniques could inform evidence-based recommendations for protective pelvic care. We hypothesized use of a motion-capture methodology could identify four major contributors to elevat

Adult laryngotracheal surgery.

The human larynx plays a pivotal role in airway protection, respiration, and phonation. Laryngeal disorders can be divided into two categories, benign lesions and malignant lesions. Most benign lesions are treatable with surgery and speech therapy, whereas the malignant lesions require more invasive surgery as well as radiation and chemotherapy. Preoperative assessment and anesthesia management for adult laryngotracheal surgery are reviewed.

[Respiratory function diagnosis during sleep: possibilities and limitations].

Diagnosis of the respiratory function during sleep is based on a continuous recording of all necessary signals. These are basically respiratory flow, respiratory effort and blood gases. In addition, sleep stages, body position and cardiovascular function have to be monitored to distinguish the different forms of sleep related breathing disorders as there are obstructive, mixed and central apnea and hypoventilation. The main evaluation is a simple counting of events and the calculation of an index related to total sleep time. These indices are of limited value because all information about the

Pearls and oy-sters: reversible Ondine's curse in a case of lateral medullary infarction.

Ondine's curse is an eponym that refers to central alveolar apnea/hypopnea observed among patients with acquired or congenital brainstem disorders. This condition results in loss of automatic and/or voluntary respiration with characteristic polysomnographic finding of impaired ventilator responses to hypercapnia and sleep apnea, which are more pronounced during non-REM sleep, less in REM sleep, and least during wakefulness.
