Pediatric rapid fluid resuscitation.

PURPOSE OF REVIEW: Intravenous and enteral fluid resuscitation are frequently used therapies in the management of pediatric patients in emergency departments and critical care settings. Any state of intravascular fluid deficit, ranging from mild dehydration due to gastroenteritis to fulminant septic shock, requires careful assessment and early restoration of hemodynamic stability. Rapid fluid resuscitation has gained increased recognition since the most recent pediatric shock management guidelines. We sought to review the evidence for rapid fluid resuscitation and to outline its clinical indic…

Intrauterine resuscitation during labor.

Intrauterine resuscitative measures are commonly initiated during labor when the fetal heart rate (FHR) pattern is indeterminate or abnormal. The most effective use of these measures is directed at the presumed underlying cause. However, some FHR patterns are nonspecific, while others are such that intrauterine resuscitation will not remedy the situation. The goals of intrauterine resuscitation during labor are, at its best, to reverse any hypoxia that might lead to further deterioration, and at the very least to avoid prolonged periods of indeterminate or abnormal FHR patterns, which may caus…

Cardiopulmonary resuscitation in children.

PURPOSE OF REVIEW: To summarize recent advances in pediatric cardiopulmonary arrest prevention, resuscitation and postresuscitation management. RECENT FINDINGS: Pediatric cardiac arrest has traditionally been considered a futile medical condition with dismal outcomes. Data in the 21st century indicate that more than 25% of children treated for in-hospital cardiac arrests survive to hospital discharge and more than 10% of children older than 1 year treated for out-of-hospital cardiac arrests survive to hospital discharge. These data establish that children are more likely to survive to hospita…

Cardiopulmonary resuscitation in children.

PURPOSE OF REVIEW: To summarize current opinion and advances in pediatric cardiopulmonary resuscitation, including etiology, pathophysiology, rationale for interventions, and postresuscitation management. RECENT FINDINGS: Cardiac arrest and ventricular fibrillation in children are not as uncommon as previously reported. Out-of-hospital cardiac arrests occur in 8-20 children/100,000/year, and in-hospital arrests occur in 2-6% admitted to a pediatric intensive care unit. Most pediatric arrests are precipitated by asphyxia or circulatory shock, but approximately 10% are precipitated by ventricul…

Pitfalls in neonatal resuscitation.

Ten percent of all newborns require resuscitation at birth. The Neonatal Resuscitation Program establishes the authoritative technique of newborn resuscitation. Errors continue to occur that are related to the use of unskilled resuscitators; intubation; inadequate suctioning of meconium; and the postresuscitation problems of hypoglycemia, hypocarbia, and hypotension. Specific recommendations are offered to avoid these pitfalls of neonatal resuscitation.

Pitfalls in surgical resuscitation.

Timing of diagnosis and intervention, together with understanding of physiology and knowledge of anatomy, are essential for success in the practice of surgery in general, and emergency surgery in particular. Either excess of treatment, for example fluid overload in tamponated haemorrhage such as a ruptured retroperitoneal Abdominal Aortic Aneurysm, or defect of treatment such as delayed arrest of life threatening bleeding from a transected Superficial Femoral Artery above the knee, can both lead to disastrous outcome or death. Proximal control of damaged arteries is an obligatory step before r

Update on neonatal resuscitation.

Pediatricians and other practitioners who attend deliveries maintain resuscitation skills throughout their careers. NRP certification and continuing education programs (PCEP) have done much to standardize the care a newborn receives in the first minutes after birth. Neonates have benefited tremendously from strong national programs (NRP and PCEP) with great local execution of training modules.

Resuscitation from traumatic shock.

Shock is the body's response to decreased cellular perfusion. It can begin with hemorrhage, mechanical obstruction of the circulation, cardiac dysfunction, central nervous system injury, or sepsis. Once triggered, shock is perpetuated by the release of toxic compounds from ischemic cells. The treatment of shock consists of the removal or correction of the triggering pathology, followed by resuscitation back to the normal state. Clinical research in shock resuscitation in the past year has focused on recognizing the presence of shock in patients at risk, particularly those with normal vital sig…

Patient assessment and resuscitation.

The patient with acute variceal bleeding requires prompt attention and is best served by an organized approach to assessment and resuscitation. Knowledge of the differential diagnosis for acute bleeding from portal hypertension is essential. Special attention is needed during resuscitation regarding endotracheal intubation, intravenous resuscitation, and management of coagulopathy and thrombocytopenia. Carefully selected patients may be triaged into outpatient, overnight observation, routine inpatient, and intensive care settings.

Hypovolemic shock and resuscitation.

Hypovolemic shock is the manifestation of hypoperfusion from inadequate intravascular volume resulting in cellular hypoxia. Early, effective treatment tailored to the individual patient will minimize morbidity and mortality. The causes and end-organ responses can differ with each patient, requiring an understanding of the underlying physiology and pathophysiology. Treatment always consists of oxygen and isotonic crystalloids and may require the use of colloids, corticosteroids, antibiotics, and vasomotor drugs.

Fluid resuscitation in pediatrics.

Early recognition of hemodynamic instability in the pediatric patient, followed by prompt intervention, is the key to successful resuscitation. Fluid management for patients in shock must be adjusted according to the patient's age and size and the pathophysiology of the underlying condition. The identification, evaluation, and fluid therapy for children presenting with shock are reviewed.

[Pharmacology of cardiopulmonary resuscitation].

Among the adrenergic receptor agonists, epinephrine, at alpha and beta stimulating doses, remains the drug of choice for cardiopulmonary resuscitation. However, alpha adrenergic agonists such as phenylephrine, methoxamine and dopamine (at alpha stimulating doses) result in similar success rates of resuscitation as epinephrine. In the opposite, beta adrenergic agonists, without or with only low alpha stimulating effect, such as isoproterenol or dobutamine are significantly less efficient. There are few data indicating that sodium bicarbonate improves outcome. Moreover it carries the risk of adv…

Perspectives on brain resuscitation.

Advances in critical care practice have created entire groups of patients that have never before survived. Regardless of the subspecialty of critical care in which you practice, the concept and knowledge of the principles of brain resuscitation must be a core component of critical care practice. The concept of brain resuscitation extends beyond subspecialty boundaries and the boundaries of cardiopulmonary arrest, to include all states of no flow or low flow, which threaten the integrity of neurons.

Brain resuscitation and protection.

No drug that is used for brain protection after global brain ischaemia as a result of cardiac arrest has been shown to be of benefit. Barbiturate agents have been proved not to be beneficial whereas studies of calcium-channel blocking drugs are inconclusive. Hypothermia, haemodilution and mechanical hyperventilation are not of proven benefit. Immediate defibrillation with rapid restoration of blood pressure is the best method to improve the neurological outcome after a cardiac arrest. After severe head injury, prompt emergency care to restore ventilation, oxygenation and blood pressure improve

Resuscitation in late pregnancy.

This paper considers cardiopulmonary resuscitation in obstetric patients at term and the influence of aortocaval compression on the outcome. The maximum chest compression force produced by eight physicians was measured as a function of angle of inclination using an inclined plane. The compression force at an angle of 27 degrees is 80% of that in the supine position and the Cardiff resuscitation wedge, designed to prevent aortocaval compression, is described with this inclination. Midwives' expertise in basic life support 6 months after instruction was assessed using a manikin simulator. The ma…
