OBJECTIVE: Real-world comparisons of rituximab biosimilars with originator rituximab have largely focused on oncology, with limited evidence in RA. This gap is particularly pertinent to the Veterans Health Administration (VHA), where patients with RA are older and have greater comorbidity. We evaluated the effectiveness and safety of rituximab-pvvr vs originator rituximab in veterans with RA by (i) describing treatment patterns, (ii) assessing effectiveness, (iii) evaluating adverse events and (iv) identifying reasons for switching back to the originator after initiating the biosimilar.

A 60-year-old man presented with refractory granuloma faciale. After unsuccessful treatment with dapsone, hydroxychloroquine, and radiotherapy, treatment with intralesional rituximab was initiated. This led to a steady improvement in the lesions without serious side effects. The case shows that rituximab can be a treatment option for refractory cases of granuloma faciale, especially in the context of IgG 4-related autoimmune disease.

Autoimmune blistering diseases are rare but potentially life-threatening conditions. Pemphigus foliaceus is one of these conditions, characterised by superficial erosions of the skin without mucosal involvement. Pediatric pemphigus vulgaris (PV) is a rare autoimmune bullous dermatosis that poses a diagnostic and therapeutic challenge. Rituximab, an anti-CD20 monoclonal antibody, is an important medication in adult PV but has rarely been used to treat pediatric disease. We describe successful rituximab therapy in a 4-year-old, the youngest patient with PV ever reported to receive rituximab. A live attenuated vaccination was later given without incident. We also review rituximab clinical outcomes, toxicity, dosing protocols, and relapse in children with PV.
