Pericarditis and Sacroiliitis in a World Traveler.

A 65-year-old immunocompromised woman presented with progressive dyspnea and sacroiliac joint pain. Cardiac magnetic resonance showed abnormal right ventricular filling with septal bounce and abnormal pericardial enhancement, suggestive of constrictive pericarditis. Cultures from pericardium following pericardiectomy grew Coccidioides immitis. She was diagnosed with coccidioidomycosis and responded to pericardiectomy and amphotericin. (Level of Difficulty: Intermediate.).

[Tuberculous sacroiliitis: report of a case].

Our case concerns a 32-year old Cameroonian male presenting with tuberculous sacroiliitis. Diagnosis was made on the basis of a positive abscess needle aspirate. Tuberculous sacroiliitis is rare and generally an isolated phenomenon. Its insidious presentation leads to delayed diagnosis. The affection is often overlooked due to lack of awareness of the clinicians, the usually good condition of the patient and minimal signs of sacroiliac joint infection. Haematological data are frequently non contributory. High risk groups include developing countries immigrants, immunodepressed patients and low…

Behçet's disease and sacroiliitis in a child.

Abstract We describe the case of a Japanese girl who suffered from both Behçet's disease (BD) and bilateral sacroiliitis. This patient was diagnosed with BD at 8 years of age, and began to complain of buttock pain when she was 10 years old. X-rays of her pelvis showed bilateral grade 3 sacroiliitis, and bone scintigraphy showed an increased uptake in the sacroiliac joints. Movements of the spine and chest expansion were not restricted. HLA typing was positive for B44 and DR4, but negative for B27. The patient was not diagnosed as having seronegative spondyloarthropathy. This case indicates tha…

["Sacroiliitis" as the initial manifestation of hepatocarcinoma].

The hepatocellular carcinomas rarely develop in absence of a previous hepatopathy and their first clinical manifestations are not related, in general, to the metastasis located at the bones of the pelvic girdle. We present a multifocal hepatocarcinoma in a 17-year-old woman without known risk factors, whose initial manifestation was a clinical radiological picture suggesting left sacroiliitis. We stress this rare manifestation preceding the diagnosis of neoplasia two years later.

Sacroiliitis in the Old Order Amish.

In a study designed to clarify the relationship of HLA-B27 and sacroiliitis, 45 members of seven Old Order Amish families were thoroughly evaluated for sacroiliitis. The primary study group was three sibs who carried HLA-B27 and three of their sibs who inherited the alternate non-B27 haplotype, their spouses and offspring. The seventh B27 positive family was more distantly related. The study was undertaken with no prior knowledge of disease in these families. These families represented all the B27-positive subjects who had been ascertained up to the time the study began. In these selected fami…

[Brucella sacroiliitis in a West African].

A brucelian sacro-iliitis is reported in a 33-year-old West African man recently arrived from Mali. Thus, clinical, biological, radiological, and isotopic aspects could be precisely observed, and were found very similar to brucelian sacroiliitis in Europeans. Major information is required about the interest of technetium scintigraphy. The treatment included Rifampicin, in association with Doxycyclin and the theoretical bases of this choice are explained. Tolerance was good and a complete recovery was obtained after three months.

Utility of CT imaging in differentiating sacroiliitis associated with spondyloarthritis from gouty sacroiliitis: a retrospective study.

Sacroiliitis is one of the criteria for classification as spondyloarthritis (SpA), though not unique to SpA. Other conditions including gout may be erroneously diagnosed as SpA due to sacroiliitis. The objective was to identify specific CT findings in sacroiliitis associated with SpA and gout. In this retrospective study, CT images of patients with sacroiliitis and clinical diagnosis of gout or SpA from 2010 to 2015 were independently reviewed by two radiologists, blinded to diagnosis. Axial and coronal oblique images were analyzed for characteristics of erosions. The receiver operator charact…

The frequency of sacroiliitis in familial Mediterranean fever and the role of HLA-B27 and MEFV mutations in the development of sacroiliitis.

The objective of this study was to investigate the frequency of sacroiliitis in familial Mediterranean fever (FMF) patients and the role of HLA-B27 and MEFV mutations in the development of sacroiliitis. The study group consisted of 256 FMF patients (male 128, female 128, mean age 27.2 +/- 6.3 years). After evaluation of the medical records, 70 patients (27.4%) were determined to have one or more of musculoskeletal manifestations. Sacroiliitis was determined in 18 (32.7%) FMF patients. The frequency of sacroiliitis among all FMF patients was found to be 7%. HLA-B27 was 47% and 6.3% in FMF patie…

Mycobacterium tuberculosis concealed by enterococcal sacroiliitis.

A 77-year-old African American female with rheumatoid arthritis presented with fever and unsteady gait. She was started on broad-spectrum antimicrobials due to CT evidence for sacroiliitis and psoas abscess and underwent partial excision of her sacroiliac bone and drainage of the abscess. One of four blood cultures grew Enterococcus faecalis and the patient was sent home with intravenous ampicillin for 6 weeks. Two days after antimicrobial completion, the patient presented with night sweats and weakness. Chest x-ray revealed new right upper lobe pulmonary infiltrates, and the AFB culture sent …

Sacroiliitis and polyneuropathy during isotretinoin treatment.

Isotretinoin, a medication for acne, has been reported to cause a variety of side effects on the musculoskeletal system. We present a case of sacroiliitis (a relatively uncommon feature) and sensorimotor demyelinating polyneuropathy, which has been reported previously in only a few cases during isotretinoin therapy. Clinical symptoms were improved after the withdrawal of isotretinoin and the follow-up electrophysiological study performed 2 years after the initial diagnosis of polyneuropathy showed mild improvement. Dermatologists are advised to be alert to symptoms of polyneuropathy and sacroi…

Acute pyogenic sacroiliitis without predisposing conditions.

STUDY DESIGN: Clinical and imaging findings of patients diagnosed with pyogenic sacroiliitis were retrospectively analyzed. OBJECTIVES: To characterize the clinical and magnetic resonance features of patients with acute pyogenic sacroiliitis without the usually known predisposing conditions and to investigate the diagnostic value of magnetic resonance imaging in detecting the early stages of pyogenic sacroiliitis. SUMMARY OF BACKGROUND DATA: Pyogenic sacroiliitis is relatively uncommon, and accurate diagnosis is frequently delayed due to lack of awareness by clinicians and nonspecific clinic…

Magnetic resonance imaging of septic sacroiliitis.

Five cases of septic sacroiliitis diagnosed by magnetic resonance imaging (MRI) are presented. Imaging was performed between 2 and 14 days after onset of symptoms and consisted of varying combinations of coronal short tau inversion recovery (STIR), axial T2-weighted spin echo (SE), and coronal and axial pre- and postcontrast T1-weighted SE scans. Abnormalities included demonstration of sacroiliac joint effusions, bone oedema and adjacent inflammation as high signal on STIR and T2-weighted SE scans, and identification of abscesses in two cases as rim-enhancing lesions anterior to the joint on g…
