Achievements in scalp reconstruction.

PURPOSE OF REVIEW: Reconstruction of scalp defects remains a challenge. This article reviews the reconstructive options and provides recommendations for scalp restoration based on current literature. RECENT FINDINGS: It is difficult to apply the standard reconstructive ladder to scalp defects due to the scalp's unique properties and paucity of adjacent tissue. Because of the frequency of large resections and the limited local tissue options microvascular free tissue transfer is a mainstay in scalp reconstruction and has been shown to be well tolerated and reliable with acceptable cosmetic and…

[Dermatologic surgery on the scalp].

Soft tissue defects of the scalp can be closed with varying techniques depending on size and depth of the defect. While small and superficial defects can be closed primarily or be left open for secondary intention healing, larger and deeper defects may need flaps or skin grafts. Extensive defects may require combined flaps or vacuum assisted closure techniques. Defects of the periosteum with denuded skull bone must be treated immediately to avoid bony complications. Usually, the tabula externa is fenestrated with holes or abraded totally to create better healing conditions. Granulation tissue …

[Eccrine porocarcinoma of the scalp].

OBJECTIVE: Eccrine porocarcinoma is a rare malignant tumor of eccrine sweat glands. It may arise de novo, but it usually develops in pre-existing eccrine poroma. Here we report a case of eccrine porocarcinoma in the grafted area on the scalp of a male patient who had recurrent basal cell carcinoma of the scalp, with clinical, histopathological and, immunohistochemical features. CASE: We report a case of eccrine porocarcinoma in 78-year-old male patient who had recurrent basal cell carcinoma of the scalp. The tumor was polypoid, granular and heterogenous colored. Microscopic examination reveal…

[Hydatid cyst of the scalp].

INTRODUCTION: Hydatid disease is a broad-based anthropozoonosis common to humans and several mammal species. The disease results from the development of Echinococcus granulosis in the body. CASE REPORT: We report a new case involving a 58-year old woman hospitalized for a subcutaneous mass in the scalp with no local inflammatory signs. Radiological examination was consistent with a subcutaneous cyst. Complete surgical resection of the mass was performed. Histopathological examination demonstrated hydatid cyst. DISCUSSION: Subcutaneous localization of hydatid cyst is uncommon even in endemic …

Giant trichoblastoma of the scalp.

BACKGROUND: Trichoblastoma is a rare and benign adnexial tumor with characteristic histological features. It occurs on any hair folliclebearing location, and usually presents as a solitary lesion most often less than 2 cm in size. Giant trichoblastoma has been rarely reported in the literature. AIM: To report a new case of giant trichoblastoma, misleading for malignancy. CASE REPORT: A 57-year-old woman presented with a 5 cm-solitary asymptomatic nodular lesion of the scalp, of 28 years. It had been previously excised with recurrence and progressive regrowth. On examination, it was a dome-sh…

Granuloma faciale of the scalp.

Granuloma faciale (GF) is an uncommon dermatosis with characteristic clinicopathological features. Extrafacial isolated GF is extremely rare. Pulsed dye laser (PDL) is a treatment option for GF to minimize the risk of scarring. We report a case of a 78-year-old male with an extensive GF of the scalp successfully treated with pulsed dye laser (PDL).

Idiosyncrasies of scalp melanoma.

OBJECTIVES/HYPOTHESIS: Examine the accuracy of sentinel lymph node biopsy (SNB) in scalp melanoma (SM), patterns of nodal metastases, patient outcomes, and the utility of immunohistochemistry (IHC) in SNB evaluation. STUDY DESIGN: Retrospective. METHODS: There were 22 patients, 4 females and 18 males. Sentinel lymph nodes (SLN) were localized via preoperative lymphoscintigraphy, intraoperative gamma probe, and Lymphazurin injection. SLNs were stained with hematoxylin-eosin, S-100, HMB-45, Melan-A, micropthalmia transcription factor, and tyrosinase. SLNs were grouped into cervical (levels 1-5…

Lipedematous alopecia of the scalp.

Lipedematous scalp is a rare disorder, mainly described in adult African-American females. We report 2 adult caucasian males with lipedematous scalp associated with androgenetic alopecia. Patients were studied by dermoscopy and histopathology; they were treated with finasteride 1 mg. In our patients, lipedematous scalp affected the occipital and the vertex areas and pathologically exhibited mild edema and thickening of the adipose subcutaneous layer. At videodermoscopy, lipedematous scalp areas showed linear areas of teleangiectasia within the scalp creases, possibly caused by compression of t…

Scalp and forehead reconstruction.

The reconstruction of defects that involve the scalp and forehead presents unique aesthetic and functional challenges. This article reviews the surgical anatomy of these regions and presents an algorithm for decision making in reconstructive surgery. Nonmicrosurgical techniques are briefly reviewed. The microsurgical reconstruction of scalp and forehead defects differs from the more common oropharyngeal reconstructions in several ways, including flap choices, choices for recipient vessels, and the opportunity to use conventional and microsurgical techniques simultaneously to improve outcomes. …

Congenital lipoblastoma of the scalp.

Lipoblastoma is a unique tumor of infancy and early childhood that can occur congenitally. It commonly occurs in trunk and extremities, but also rarely in head and neck. We have not encountered any report of congenital lipoblastoma of scalp in published literature. Here we describe the case of an infant who presented with a rapidly growing large scalp mass that measured 15 x 15 x 10 cm. At birth, the mass was located on the bregma but progressed to extend into the left upper eyelid and eyebrow. The mass was resected in two stages: the first stage consisted of resecting the scalp mass and the s…

Atypical fibroxanthoma of the scalp.

BACKGROUND: Atypical fibroxanthoma occurs most frequently in the head and neck region of the elderly. Previous reports have identified that the condition usually arises at the following sites: the nose, cheeks, forehead, and the ears; its development at other sites is unusual. METHOD: We report a series of 10 cases with lesions all occurring at an apparently unusual site, the scalp, over a 10-year period. We compared the clinical and histologic appearances and behavior of this series with the existing reports of these lesions elsewhere in the head and neck region to investigate whether there …

[Erosive lichen of the scalp].

BACKGROUND: We report an exceptional clinical presentation of erosive lichen planus of the scalp associated with hepatitis C and idiopathic CD4 lymphocytopenia. CASE REPORT: A 90-year-old woman was hospitalized in May 1995 for erosive dermatosis of the scalp and alopecia, associated with ungual lesions. Histology of the scalp lesions demonstrated lichen and the serology tests were positive for hepatitis C. The patient also had severe CD4 lymphocytopenia (290/mm3). The lesions regressed with clobetasol and occlusion and growth of scalp hair turned. DISCUSSION: Erosive lichen planus of the sca…
