OBJECTIVE: To analyze the therapeutic approach in epidermoid or squamous cell carcinoma of the scrotum, a rare tumor that is of interest for clinical and historical reasons.
METHODS/RESULTS: A case of epidermoid carcinoma of the scrotum is described. The patient had undergone urethroplasty in 1972, using preputial free graft, according to the technique of Ben Johnson. Right scrotectomy and orchidectomy were performed and partial urethrectomy with inguinal lymphadenectomy 6 weeks later. At 24 months' follow-up, the patient has no tumor recurrence.

Aggressive angiomyxoma is a rare, locally aggressive but non-metastasizing soft tissue tumour which occurs almost, exclusively in female pelvic soft tissues. We report on a tumour occurring in the scrotum of the adult male. Despite a histologically benign picture, wide excision and strict postoperative control are recommended because of the risk of tumour recurrence.

Torsion of the spermatic cord is the most common etiology of acute scrotum in children. Children with torsion usually present with acute scrotal pain, nausea and vomiting, and only early surgical treatment, within 6 hours of the onset of symptoms, may ensure the preservation of the testis. For that reason any acute scrotal pain with edema requires urgent specialized evaluation. Other etiologies include less severe diseases as torsion of an appendage, epididimytis, trauma, and other rare conditions such as acute idiopathic scrotal edema.

Pyogenic granuloma is a benign, vascular, over-exuberant, inflammatory, solitary skin tumor common in infants and children. It is a rapidly growing solitary, sessile, or polypoidal vascular nodule that will bleed easily. The exact pathogenesis of this condition is unknown. Various predisposing factors, such as trauma, hormones, infection, viral oncogenesis, microscopic anastomosis, and others may be responsible for its growth.

A 65-year-old man presented with a 3.5 cm scrotal mass. Ultrasound showed a solid mass of indeterminate aetiology within the skin. Excisional biopsy revealed a benign chondroid syringoma. This is the first report of chondroid syringoma involving the scrotum.

Hemangiomas of the genitalia are extremely rare. A hemangioma is self-limited and usually resolves spontaneously, therefore conservative treatment is generally recommended. However, genital hemangiomas may require surgical treatment if they become symptomatic. Herein we report a case of hemangioma of the penis and scrotum requiring surgical excision. The natural history and alternative treatments of hemangiomas are described.

Paratesticular sarcomas are rare, especially the malignant mesenchymoma. To our knowledge only four cases of paratesticular malignant mesenchymoma have been described previously. All were localized to the spermatic cord. We present a case of malignant mesenchymoma in the scrotum free of the spermatic cord.

The pediatric acute scrotum can be a diagnostic dilemma. The history, physical examination, and appropriate tests are important. The correct diagnosis will be arrived at if the systematic approach is remembered (Figure 14). A case of idiopathic calcinosis of the scrotum is presented. This term has been applied to an uncommon disorder characterized by solitary or multiple, firm painless papules or nodules of the scrotum that usually appear during childhood or early adult life. The lesions are skin colored to white-yellow, increase in size and number, may itch or drain a chalky white material. X-ray film reveals calcified shadows. Biopsy specimens of the lesions show masses of calcium deposits in the dermis without evidence of an epithelial lining.

Ultrasound is a proved, safe diagnostic procedure. Its efficacy in the evaluation of 54 cases of scrotal pathology is reviewed.
