Screening for sickle cell trait: the Veterans Administration National Sickle Cell Program.

Results of the Veterans Administration Sickle Cell Program for a period of 10 years are presented. We screened 370,250 patients; 404,341 attended educational sessions, and 38,347 had individual counseling sessions. Sickle cell trait was present in 6.4% of patients, and HbC trait was present in 1.8%. The clinically significant disorders HbSC disease, sickle cell anemia, and sickle beta thalassemia were present in 0.41% of individuals screened. A large number of uncommon variants were detected. The program enhanced the awareness of and the approach to evaluation of hemoglobinopathies.

Sickle Cell Trait: Is It Always Benign?

Sickle cell disease is a well-known homozygous inherited hemoglobinopathy that causes vaso-occlusive phenomena and chronic hemolysis. Vaso-occlusion results in sickle cell crisis and can eventually lead to complications involving multiple organ systems. However, the heterozygous counterpart, sickle cell trait (SCT) has less clinical significance as these patients are generally asymptomatic. This case series examines three unrelated patients with SCT ranging from the age of 27 to 61 years, who presented with pain in multiple long bones. Hemoglobin electrophoresis confirmed a diagnosis of SCT. R…

Sickle Cell Trait Causing Splanchnic Venous Thrombosis.

Sickle cell trait is considered as a benign condition as these individuals carry only one defective gene and typically have their life span similar to the normal population without any health problems related to sickle cell. Only under extreme conditions, red cells become sickled and can cause clinical complications including hematuria and splenic infarction. Although twofold increased risk of venous thrombosis has been described in African Americans, there is no data available from Indian population. We here report a case of sickle cell trait from India whose index presentation was thrombosis…

Exercise-related complications in sickle cell trait.

This review presents the epidemiological data regarding the exercise-related complication in exercising sickle cell trait carriers, and focuses on the different potential mechanisms that could be involved in these adverse events, such as hemorheological alterations, inflammation, vascular adhesion of circulating blood cells, oxidative stress and impaired nitric oxide metabolism. We also discuss the effects of different modulating factors such as vascular function, environment (hot temperature), hydration status, physical fitness, exercise intensity and genetic factors.

Retinopathy in patients with sickle cell trait.

BACKGROUND: Sickle hemoglobinopathies are among the most prevalent genetic disorders in the United States. Sickle cell trait (hemoglobin AS) is the most common genotype and has traditionally been considered a benign condition. Systemic and ocular complications are seen infrequently in patients with sickle cell trait. In the presence of concomitant systemic diseases or trauma, however, marked retinopathy can occur. The presence of retinopathy in patients with sickle cell trait necessitates a comprehensive medical workup, including serologic testing to rule out systemic conditions. CASE REPORTS…

Subcortical cerebral infarctions in sickle cell trait.

At necropsy, two patients with sickle cell trait and progressive motor and visual deficits, lethargy and coma showed infarctions of the deep cerebral white matter and brain stem. The findings in these patients and another reported in the literature suggest that subcortical infarctions may be more common in sickle cell trait than has been recognised and should be suspected in any patient with sickle cell trait who presents with an unusual neurological illness.

Maternal death associated with sickle cell trait.

A case report is presented of a patient with sickle cell trait who died an abrupt intrapartum death precipitated by probable peripartal cardiomopathy and apparently aggravated by intravascular sickling. Notable autopsy findings included massive sicklemia in all organ systems.

Pregnancy in women with sickle cell trait.

The complications and outcome of 334 pregnancies in women with sickle cell trait between 1975 and 1981 are reviewed. The perinatal mortality rate was 6 per 1000. The main maternal complications were recurrent urinary tract infections (6%) and haematuria (16.5%). Fetal distress in labour was seen in 17% and 11.5% were delivered by emergency caesarean section. Compared with a comparable group of women without sickle cell trait, the mean birthweight of the babies (3202 g) was not reduced. The mean age of the mothers (25 years), their mean parity (1.46) and the percentage who were para 4 or more (…

Multifocal Best's disease and sickle cell trait.

Ophthalmoscopy performed during a routine eye checkup on a 22-year-old black man revealed multiple intraretinal, cyst-like lesions scattered throughout the macular and extramacular areas in both eyes. Based on clinical findings, morphologic appearance, and an abnormal electro--oculogram, the diagnosis of multifocal Best's macular dystrophy was made. Hemoglobin electrophoresis showed an AS pattern confirming the presence of sickle cell trait. This is the first case of the coexistence of these two genetically transmitted diseases, Best's disease and sickle cell trait.

Sickle cell trait and transient monocular blindness.

Five episodes of transient monocular blindness during a six-month period occurred in a 42-year-old black man who had sickle cell trait. Other than the hemoglobin abnormality no cause for the transient monocular blindness was determined.
