The intelligibility of sentences in quiet and in noise in aged listeners.

The monaural speech-reception threshold (SRT) for sentences was investigated in quiet and under four noise conditions for 80 male subjects (age 60-90) and 30 female subjects (age 71-89). The noise levels used were 28, 43, 58, and 73 dBA. The noise had the long-term average spectrum of speech. It is shown that a model developed by Plomp [J. Acoust. Soc. Am. 63, 533-549 (1978)], which interprets any hearing loss for speech (SHL) as a combination of a loss of class A (attenuation of both speech and noise) and a loss of class D (distortion of the sound signals), enables one to make an accurate des

[Renewal of a hearing aid is a medical task].

A 67 year-old man was known to have a right-sided hearing loss with a normal discrimination score (DS) and was treated with a hearing aid (HA) in the right ear. An MRI-scan was normal. In 2006 and 2010 the HA was renewed at a private hearing aid clinic. No DS was performed. At the audiology clinic we found a masked DS of 0% and progression of the hearing loss. An MRI-scan revealed a vestibular schwannoma (VS) on the right side. The patient was fitted with an HA in the left ear and MRI-scans every six months. A masked DS could have revealed the VS and correct treatment thus could have been init

Analysis of glottal source parameters in Parkinsonian speech.

Diagnosis and monitoring of Parkinson's disease has a number of challenges as there is no definitive biomarker despite the broad range of symptoms. Research is ongoing to produce objective measures that can either diagnose Parkinson's or act as an objective decision support tool. Recent research on speech based measures have demonstrated promising results. This study aims to investigate the characteristics of the glottal source signal in Parkinsonian speech. An experiment is conducted in which a selection of glottal parameters are tested for their ability to discriminate between healthy and Pa

Comparison between central tonal tests and central speech tests in elderly subjects.

A battery of tonal tests investigating the performance of the central auditory pathways (auditory lateralization, temporal order, auditory pattern) has been applied, together with a battery of central speech tests (sensitized speech and synthetic sentences), to a group of 50 subjects aged from 60 to 80 years, affected only with presbycusis. The results demonstrate that the speech tests undergo a considerable reduction of the performance-intensity function with age whereas the tonal tests remain within the values obtained in younger subjects. The topodiagnostic implications of these data are di

Evaluation of the speech perception in noise (SPIN) test.

Several investigations were perfomed with normal hearing subjects to determine the effects of presentation level and signal-to-babble ratio on the speech perception in noise (SPIN) test. The SPIN test contains sentences that simulate a range of contextual situations encountered in everyday speech communication. Findings from several representative patients with sensorineural hearing loss demonstrate the possible clinical utility of the test to measure the effects of context on speech discrimination.

[Pure word deafness after cerebral hemorrhage in the left temporal lobe: a case report].

We report a patient with pure word deafness after subcortical hemorrhage in the left temporal lobe. Repetition and auditory comprehension were severely impaired, while reading and visual comprehension of the same material were almost normal. He did not show hearing loss, but speech discrimination and melody recognition was poor. On the speech discrimination test, his score was low especially in the right ear. The threshold on the directional hearing test was mildly elevated. There was no temporal summation by click sounds. CT and MRI disclosed a subcortical hematoma in the left superior tempor

The effect of practice on low-level auditory discrimination, phonological skills, and spelling in dyslexia.

Phonological awareness is believed to play a major role in the auditory contribution to spelling skills. The previous paper reports low-level auditory deficits in five different subdomains in 33-70% of the dyslexics. The first study of this paper reports the results of an attempt to improve low-level auditory skills by systematic daily practice of those tasks that had not been passed in previous diagnostic sessions. The data of 140 dyslexics indicate that the average number of unsolved tasks can be reduced from 3 of 5 to 1 of 5. The success rates have values of 70-80% for intensity and frequen

Recognition of plosive syllables in noise: comparison of an auditory model with human performance.

Some of the effects of noise on the perception of plosive-vowel syllables have been investigated. It was found that noise added to the syllables for the duration of the speech had a more deleterious effect on perception than noise of the same intensity played continuously. Physiological experiments have shown that the response thresholds of cochlear nerve fibers to tones are raised by continuous background noise but not by short bursts of noise. It is suggested that this may be responsible for the speech perception results. In order to investigate this, an auditory model was developed which in

Speech perception in adult subjects with familial dyslexia.

Speech perception was investigated in a carefully selected group of adult subjects with familial dyslexia. Perception of three synthetic speech continua was studied: /a/-/e/, in which steady-state spectral cues distinguished the vowel stimuli; /ba/-/da/, in which rapidly changing spectral cues were varied; and /sta/-/sa/, in which a temporal cue, silence duration, was systematically varied. These three continua, which differed with respect to the nature of the acoustic cues discriminating between pairs, were used to assess subjects' abilities to use steady state, dynamic, and temporal cues. Comparison of small fenestra and total stapedectomy.

A biostatistical study of the results of 280 consecutive operations for clinical otospongiosis are analyzed over a seven-year period. One hundred forty-one (141) operations were done utilizing the "total stapedectomy" technique, and 139 were done utilizing the "small fenestra" technique. The auditory acuity of the two groups is compared with reference to conductive and sensorineural performance. All the surgery was performed by the authors, the data was collected by a third party and analyzed by a statistician. All audiograms were performed by the same audiologist under the same conditions. Early speech changes in children with multichannel cochlear implants.

A number of studies have demonstrated that cochlear implants provide an improved auditory signal and enhance the development of speech-perception and production skills for profoundly deaf children. However, exactly when these early speech skills begin to develop remains unclear. To explore this issue, we observed, for a 1-year period, four prelingually deaf children who underwent implantation consecutively within 1 month of each other, and we paid particular attention to the first few months of rehabilitation. We found immediate speech scores as early as the first day of implant tune-up. Speec

Hearing Aid Benefit in Auditory Neuropathy Due to Friedreich Ataxia: A Case Report.

PURPOSE: Friedreich ataxia is one of the most common recessively inherited ataxias, with hearing loss due to auditory neuropathy being a recognized complication. However, there are limited data on the best interventions for hearing loss in these patients. We report the significant behavioral and subjective benefits of binaural amplification in a patient with auditory neuropathy due to Friedreich ataxia. METHOD: Auditory function was assessed in a 44-year-old woman with genetically confirmed Friedreich ataxia using pure-tone audiometry, speech audiometry, and immittance testing, along with aud

Telephone speech perception by Mandarin-speaking cochlear implantees.

OBJECTIVE: To evaluate Mandarin-speaking cochlear implant patients' understanding of telephone speech. DESIGN: Telephone speech was simulated by band-limiting broadband speech stimuli (300-3200 Hz) and adding Gaussian noise (35 dB signal-to-noise ratio). Recognition of multitalker vowels, consonants, voice gender, and Chinese tones was measured for both simulated telephone speech and broadband speech in fifteen Mandarin-speaking cochlear implant patients. RESULTS: Results showed no significant difference in Chinese tone recognition scores between broadband and telephone speech. However, mean

Clinical manifestations of otological syphilis.

One patient with early otological syphilis and five patients with late otological syphilis are described including audiological and serological findings. In patients with late syphilis poorer than expected speech discrimination scores relative to the pure tone audiograms were noted. Profound hypo-activity of vestibular responses to caloric stimulation was present in the cases of late otological syphilis. Routine non-treponemal serological tests were not sensitive enough to establish the diagnosis. It is recommended that more sensitive treponemal serological tests such as the FTA-ABS should be

Processing of temporal fine structure as a function of age.

OBJECTIVES: The purpose of this study was to determine whether the processing of temporal fine structure diminishes with age, even in the presence of relatively normal audiometric hearing. Temporal fine structure processing was assessed by measuring the discrimination of interaural phase differences (IPDs). The hypothesis was that IPD discrimination is more acute in middle-aged observers than in older observers but that acuity in middle-aged observers is nevertheless poorer than in young adults. DESIGN: Two experiments were undertaken. The first measured discrimination of 0- and π-radian inte

Identifying language impairment in children: combining language test scores with parental report.

BACKGROUND: Children who meet language test criteria for specific language impairment (SLI) are not necessarily the same as those who are referred to a speech and language therapist. AIMS: To consider how far this discrepancy reflects insensitivity of traditional language tests to clinically important features of language impairment. METHODS & PROCEDURES: A total of 245 twin children, 52 of whom had been referred to a speech and language therapist for assessment or intervention, were studied. They were given a battery of language tests and their parents completed the Children's Communication

The effect of talker and intonation variability on speech perception in noise in children with dyslexia.

PURPOSE: In this study, the authors aimed to determine whether children with dyslexia (hereafter referred to as "DYS children") are more affected than children with average reading ability (hereafter referred to as "AR children") by talker and intonation variability when perceiving speech in noise. METHOD: Thirty-four DYS and 25 AR children were tested on their perception of consonants in naturally produced CV tokens in multitalker babble. Twelve CVs were presented for identification in four conditions varying in the degree of talker and intonation variability. Consonant place (/bi/-/di/) and
