OBJECTIVE: To study volume characteristics of the maxillary, sphenoid and frontal sinuses among healthy Caucasians adults, using computed tomography (CT) scans.
METHODS: A retrospective, case series study in a single academic center, CT scans of 201 consecutive adult subjects, performed between January and September 2014, were reviewed for the volume and dimensions of the paranasal sinuses. Patients with documented sinus pathology or lack of pneumatization were excluded. The study population was subdivided by gender and age (50 men age 25-64; 51 men age ≥65; 50 women age 25-64, and 50 women a

An inflammation or injury to optic nerve anywhere its pathway by extrinsic lesions causes the optic neuropathy. These lesions are more effective in confined spaces like optic canal, orbital apex. We present a 61 year old woman with optic neuropathy due to lesion in an Onodi cell. In this case patient presented with progressive loss of vision in her right eye within a few days. Computed tomography (CT) revealed mucosal thickening and inflammatory signs in right sphenoid sinus and presence of Onodi cell in that side. Magnetic Resonance Imaging confirmed compression to the optic nevre. Systemic a

A fungal ball is most frequently found in the maxillary sinus, though it can also occur in the sphenoid, ethmoid, and frontal sinuses. This study aims to demonstrate the pathological conditions and clinical features of a patient with unilateral left maxillary and sphenoid sinus fungal balls. A 52-year-old woman arrived with complaints of headache, facial pain, and post-nasal discharge. An endonasal endoscopic examination was unremarkable. However, the plain paranasal sinuses computerized tomography scan (CT scan PNS) of the patient revealed two simultaneous opacities occupying both the maxilla

Our objective was to establish the age-related 3D size of maxillary, sphenoid, and frontal sinuses. A total of 179 magnetic resonance imaging (MRI) of children under 17 years (76 females, 103 males) were included and sinuses were measured in the three axes. Maxillary sinuses measured at birth (mean+/-standard deviation) 7.3+/-2.7 mm length (or antero-posterior)/4.0+/-0.9 mm height (or cranio-caudal)/2.7+/-0.8 mm width (or transverse). At 16 years old, maxillary sinus measured 38.8+/-3.5 mm/36.3+/-6.2 mm/27.5+/-4.2 mm. Sphenoid sinus pneumatization starts in the third year of life after convers

OBJECTIVES: This study aimed to evaluate linear measurements of the frontal sinus (FS) and sphenoid sinus (SS) for sex identification on cone beam computed tomography (CBCT) images.
METHODS: A comparative CBCT analysis was conducted on 200 full field of view (FOV) scans taken as part of routine dental investigations. Dimensions of the bilateral frontal and sphenoid sinuses were measured. Intra- and interobserver reliability were calculated. Independent t tests were used to compare the various parameters between sexes. Stepwise discriminant function analysis was used to determine sex. Addition

PURPOSE: Vidian neurectomy is a surgical procedure applied to different pathological conditions, including chronic rhinitis and sphenopalatine neuralgia. The choice of the correct surgical approach depends upon the possible protrusion of Vidian nerve into the sphenoid sinuses. The present study analyzes the possible relationship between protrusion of Vidian nerve and volume of sphenoid sinuses.
METHODS: In total, 320 maxillofacial CT-scans were retrospectively assessed. Subjects equally divided among males and females (age range 18-94 years) were divided into three groups according to the pro

Nocardiosis is an infectious disease caused by filamentous, saprophytic bacteria that classically affects the lungs, central nervous system, and skin as an opportunistic infection in immunocompromised patients. Although it can affect any organ system in general, osteomyelitis is not commonly seen and even more rarely involves the skull. We report the uncommon case of a clival osteomyelitis secondary to isolated sphenoid sinusitis caused by Nocardia veterana in an 85-year-old immunocompetent female who presented with right-sided facial pain, abducens nerve palsy, and eventually loss of vision.

BACKGROUND: Unilateral isolated sphenoid sinus disease (ISSD) is a rare diagnosis of the paranasal sinuses that can be associated with complications involving vascular, neurologic, and optic structures in close proximity.
CASE PRESENTATION: A 62 year old female presented to a hospital in Georgetown, Guyana with right lateral rectus palsy, diplopia, and a severe progressively worsening headache. CT scan revealed an opaque left sphenoid sinus consistent with unilateral ISSD. A transnasal sphenoidotomy was performed without complication under local anesthetic in the absence of endoscopic guidanc

BACKGROUND: Mucormycosis is a fungal infection with the following 5 classic forms: cutaneous, pulmonary, gastrointestinal, disseminated, and rhinocerebral. The rhinocerebral form can be rapidly progressive and invasive with a high mortality rate. We present a case of a 38-year-old man with invasive mucormycosis that led to a basilar artery territory stroke. Rhinocerebral mucormycosis is an unusual cause of stroke.
CASE REPORT: A 38-year-old man with a past medical history of diabetes mellitus presented with altered mental status. A lumbar puncture revealed eosinophilic pleocytosis with a mild

Isolated sphenoiditis is an uncommon disease, particularly in children. Immediate antibiotic therapy and/or endoscopic transnasal sphenoidotomy should be performed to avoid fatal complications. However, the commonly used transnasal route is not always available in children with a cleft lip and deviated nasal septum. We report a case of acute isolated sphenoid sinusitis in a 12-year-old boy with a cleft lip and deviated nasal septum. This patient underwent combined endoscopic transnasal and intrasphenoidal septal surgery and improved without septoplasty, which may affect the later growth of the…

INTRODUCTION: Conventional surgical approaches to the lateral aspect of a well-pneumatized sphenoid sinus are associated with significant surgical morbidity. Transorbital endoscopic approaches have recently gained favor as an alternative approach to the skull base. We describe the use of a contralateral precaruncular endoscopic approach to provide a surgical pathway to the lateral aspect of the sphenoid sinus, allowing for improved and direct visualization of the surgical field, with limited morbidity.

Isolated sphenoid sinusitis is an uncommon entity, accounting for a small proportion of paranasal sinus disease. Its deep anatomical location and proximity to the optic nerve, cavernous sinus, and orbital apex often result in delayed diagnosis, particularly when typical nasal symptoms are absent. In poorly controlled diabetics, fungal infection of the sphenoid sinus can progress rapidly, presenting initially with ophthalmic deficits rather than sinonasal complaints. This series describes three female patients between 50 and 53 years of age, all with long-standing type 2 diabetes mellitus with …

Background: Primary ciliary dyskinesia (PCD; MIM 242650) is a rare genetic disorder characterized by malfunction of the motile cilia resulting in reduced mucociliary clearance of the airways. Together with recurring infections of the lower respiratory tract, chronic rhinosinusitis (CRS) is a hallmark symptom of PCD. Data on genotype-phenotype correlations in the upper airways are scarce. Materials and methods: We investigated the prevalence, radiologic severity, and impact on health-related quality of life (HrQoL) of CRS in 58 individuals with genetically confirmed PCD. Subgroup analysis was p

This work was designed to carry out the postmortem evaluation of the potential of using a multi-angle endoscope for endonasal surgical interventions on the ethmoidal and sphenoidal sinuses. The study was performed on 5 fresh cadavers (10 sides). It was shown that the range of the angles and directions of view during dissection of ethmoidal sinuses is 10-50 degrees and 360 degrees respectively. The use of an Acclarent Cyclops endoscope made it possible to visualize all walls of the sphenoidal sinus through the enlarged aperture which confirms the higher effectiveness of this instrument compared
