[Stenosis of the cervical spinal canal: acquired? congenital].

The authors show that it is impossible to describe congenital stenosis of the cervical rachidian canal with a particular morphology that would make it possible to recognize the condition before the appearance of neurological signs. They do not deny its existence, but at the moment there is no proof, and hope of a prophylaxis for the neuropathy is not justified. On the other hand, there is clearly a possibility of an acquired stenosis. The term arthrosis is so generalized that it has lost nearly all its meaning. The rheumatologist knows the complex lesions comprise osteophytes and thickening of

Morphometric analysis of the cervical spinal canal on MRI.

Two useful numerical values, called the Torg ratio and the spinal canal diameter (SC diameter) are widely accepted as reliable morphometric determinants of spinal stenosis. The aims of the study were to examine morphometric determinants of the cervical spinal canal on MRI in both sexes and analyse them as reliable indicators of spinal stenosis. Measurements were made on 50 MR images (sagittal T2 weighted images from C3 to C7) of the cervical spine of patients from the Emergency Centre who had undertaken MRI of the cervical spine in addition to CT for various diagnostic indications. Torg ratio,…

The peridural membrane of the spinal canal: a critical review.

There exists substantial evidence that a peridural membrane (PM) is present in the spinal canal of humans and, like the pleura and peritoneum, has one or more physiologic functions. Innervation of the PM suggests that it may become a source of pain if injured. Although debated, the physiology of this structure has important implications with respect to neuraxial distribution of drugs and for back and radiating pain. This review, separated into embryological, anatomic, and physiologic discussions, provides an in-depth summary of the observations of this connective tissue. The discrepancies betw…

Enterogenous cysts of the spinal canal and cerebellopontine angle.

Enterogenous cysts are rare, benign lesions involving the spinal canal or the cerebellopontine angle. Typically they present with compression of the spinal cord or cranial nerves. They are usually 1 cm or less in size and are typically lined by columnar mucin-producing epithelium resembling enteric epithelium or columnar ciliated epithelium resembling respiratory epithelium. The case records of the Division of Neuropathology at Indiana University were reviewed for cystic lesions of the spinal canal and cranial cavity for a 26-year period. Seven surgically resected enterogenous cysts were ident…

[Congenital defects of the spinal cord and canal in neuroimaging].

Authors present the clinical classification of spinal dysraphisms and discuss all imaging modalities useful in the diagnosis: conventional X-rays, myelography, computed tomography, myelo-CT and magnetic resonance imaging. They precise the place of ultrasound in the prenatal and postnatal imaging of spinal dysraphisms.

Baricity and the distribution of lidocaine in a spinal canal model.

The role of the baricity of local anaesthetic solutions in determining the distribution of local anaesthetics injected into the subarachnoid space (and hence the level of anaesthesia) has been challenged. A recent study found no difference in the extent of cephalad spread of hyperbaric and isobaric solutions and concluded that density had no effect on the spread of local anesthetics. The present study, to determine the validity of this conclusion, utilized a spinal model filled with a "cerebrospinal fluid equivalent." Following the injection of hyperbaric lidocaine, the local anaesthetic was m

[Sciatica caused by lateral stenosis of the spinal canal].

Eighty-eight patients (52 men, 36 women) were operated on for lateral recess syndrome, during the 1970-1986 period. 7 had priorly a disc excision; in 11 a chemonucleolysis had been performed. The pain affected one nerve root in 41 cases, two in 8, three in 1. In 33 patients the pain pattern was incomplete. Myelography was used in 77 cases, and was positive in 68. CT scan was used in 43 cases. A posterolateral fusion was added to the unroofing of the nerve in 9 patients, who had severe low back pain before surgery. The results were assessed with a special rating scale of 20 points. They were co

[Complex indications in surgery of the lumbar spinal canal].

Opinions on the treatment of disc herniation are very controversial and the results reported vary, very much indeed. Most often, one decided … % of those who had also been suffering from additional causes of lumbalgia like a relatively narrow spinal canal, spondylolisthesis, spondylolysis or nearthrosis for instance. This finding is of paramount importance when attempting to improve the overall results. It is very necessary to separate the complex cases from the "simple" disc herniation ones. At the …

Developmental narrowing of the spinal canal in the lumbar region.

Developmental narrowing of the spinal canal in the lumbar region is one of the most controversial subjects from the point of view of the significance of the neurological syndrome, the value of spondylometric assessment and methods of treatment. Among 1,196 patients operated on in our neurosurgical centre during the last 18 years because of symptomatic sciatica the authors diagnosed developmental narrowing of the spinal canal in 29 cases (2.4%). The diagnosis was made mainly on routine X-ray plates, functional X-rays, X-ray contrast examination and tomograms with indicator of known size placed …

Stenosis of the lumbar spinal canal in Forestier's disease.

Hyperostosis of the spine, Forestier's disease, is a rare, but well defined entity which results from silent, extensive paraspinal calcification. It has been regarded as an asymptomatic ankylosis requiring no medical or surgical treatment. Two patients with Forestier's disease are presented who show severe stenosis of the lumbar canal with early symptoms of neurological compression.

Percutaneous Endoscopic Lumbar Decompression for Lumbar Lateral Spinal Canal Stenosis: Classification of Lateral Region of Lumbar Spinal Canal and Surgical Approaches.

PURPOSE: The purpose of the present study was to provide a new classification of the lateral region of the lumbar spinal canal (LLSC) and evaluate the clinical outcomes of surgical treatment of LLSC stenosis guided by the classification. METHODS: We developed a new functional classification of the LLSC by dividing the region into 5 zones. Thirty random lumbar imaging studies from patients with single-level LLSC stenosis in our department from January 2015 to June 2015 were evaluated by 3 examiners to validate its reliability. Subsequently, a prospective clinical outcome study of 76 patients w…

Misplacement of a vena cava filter into the spinal canal.

We report the case of a 70-year-old male with a complication of misplacement of a vena cava filter into the spinal canal. This likely happened as a result of penetration of the wire and filter sheath through the iliac vein or vena cava into the retroperitoneum, vertebral foramina, and spinal canal at the level of L2 and L3. Due to the patient's condition, the filter was not removed and no neurologic symptoms have occurred. This represents the first reported case of a filter deployment into the spinal canal. Although placement of vena cava filters is a relatively safe procedure, complications a…

The effect of translation of the C1-C2 on the spinal canal.

The stability of the atlantoaxial articulation depends fundamentally on the integrity of the odontoid process and the ligaments. Ligament stability mostly is maintained by two ligaments: the transverse ligament and the alar, apical ligaments. Failure of the transverse ligament can result in anterior translation of the atlas on the axis. The anteroposterior diameter of the ring of the atlas is approximately 3 cm. The spinal cord and the odontoid process are each approximately 1 cm in diameter, approximately 1/3 the diameter of the ring. According to Steel's rule of thirds, the remaining centime

Restoration of the spinal canal by the internal fixator and remodeling.

It has been observed that after reduction of an unstable vertebral fracture, treated either conservatively or operatively, the encroachment of the spinal canal remaining after reduction can decrease considerably with time. A series of 31 cases, each with a set of CT scans taken preoperatively, immediately after surgery and after implant removal respectively, was assessed using computer-aided planimetry. The mean initial narrowing of the spinal canal was 44.6%. Through surgery this was decreased by 20.4%, leaving a remaining deficit of 24.2%. After implant removal, 15 months after trauma and su…
