General surgery problems in patients with spinal cord injuries.

Twenty-four patients with spinal cord injuries were studied to correlate their responses to intra-abdominal disease with the level and completeness of the cord lesion. Patients with complete cervical lesions and lesions of the upper part of the thoracic region (C-4 to T-6) usually responded by early noniocalized abdominal pain associated with signs of autonomic dysreflexia. As the disease progressed to involve the parietal peritoneum, these patients were more capable of localizing pain to the corresponding dermatome, whereas patients with incomplete lesions were able to localize their pain ear…

Spinal cord injury: preventing secondary injury.

Spinal cord injury is devastating to the victim, as well as being costly in terms of medical expenses, lost wages, and lost independence. The initial damage to the spinal cord results from several mechanisms of injury--flexion, extension, compression, penetration, rotation, and the disease process. When the spinal cord is injured and there is necrosis of the nervous tissue, no regeneration of that tissue occurs. Unlike in the peripheral nervous system, where regeneration is possible, the spinal cord is part of the central nervous system, as is the brain. The spinal cord extends from the base o

Exosome-Loaded Bioscaffolds for Spinal Cord Injuries: A Review.

Exosomes are naturally occurring cellular products released by various cell types in the body. Their composition is similar to that of human tissues, which reduces the risk of immune rejection. As critical mediators of intercellular communication, exosomes transmit signals and information that regulate the physiological states of surrounding tissues. Depending on their cellular origin and molecular content, exosomes can either promote nerve regeneration and functional recovery at the site of spinal cord injury (SCI) or exacerbate the local injury microenvironment. However, as a cellular produc

Recent advances in nanotechnology for repairing spinal cord injuries.

Spinal cord injury (SCI) remains a formidable clinical challenge with limited therapeutic options. Recent advances in nanotechnology have introduced paradigm-shifting strategies that transcend the limitations of traditional treatments by offering precision, controllability, and multifunctionality in modulating the hostile post-injury microenvironment. This review systematically summarizes nanotechnology-based therapeutic approaches for SCI, including cell-based nanotherapeutics, nanogels/hydrogels, nano-engineered materials, and combinatorial strategies. We emphasize the synergistic design of

Delayed diagnosis of spinal cord injuries in Huntington's disease.

Huntington's disease is a neurodegenerative disorder, characterised by progressive cognitive, motor and psychiatric symptoms. Patients with advanced disease presenting to emergency medical services can pose a diagnostic and management challenge for physicians unfamiliar with the condition. We describe two patients with Huntington's disease in whom the diagnosis of traumatic spinal cord injury was delayed, discuss the role that cognitive bias and other factors played in this delay, and the lessons we can learn.

Mechanisms of Stem Cell Therapy in Spinal Cord Injuries.

Every year, 0.93 million people worldwide suffer from spinal cord injury (SCI) with irretrievable sequelae. Rehabilitation, currently the only available treatment, does not restore damaged tissues; therefore, the functional recovery of patients remains limited. The pathophysiology of spinal cord injuries is heterogeneous, implying that potential therapeutic targets differ depending on the time of injury onset, the degree of injury, or the spinal level of injury. In recent years, despite a significant number of clinical trials based on various types of stem cells, these aspects of injury have n…

Cognitive Dysfunction in Persons with Chronic Spinal Cord Injuries.

Cognitive dysfunction (CD) is pervasive in individuals who have chronic spinal cord injuries (SCI). Although classically associated with concomitant traumatic brain injuries, many other causes have been proposed, including premorbid neuropsychological conditions, mood disorders, substance abuse, polypharmacy, chronic pain and fatigue, sleep apnea, autonomic dysregulation, post-intensive care unit syndrome, cortical reorganizations, and neuroinflammation. The consequences of CD are likely widespread, affecting rehabilitation and function. CD in those with SCI should be recognized, and potential…

The perimenopause experience for women with spinal cord injuries.

When health questions or concerns arise, nurses are often the first care providers contacted by women with spinal cord injuries (SCIs). This highlights the importance of nurses' understanding of specific health issues such as the perimenopause transition, along with understanding the symptoms and changes that women with SCI may experience throughout the conversion into menopause. Women aged 35 to 55 years old, with an SCI of any level or degree of completeness, no history of salpingo-oophorectomy or hysterectomy, and who receive care at a SCI model system in the mountain states, were mailed a …

Spinal cord injuries and attempted suicide: a retrospective review.

STUDY DESIGN: A retrospective review examining the cases of 137 individuals with spinal cord injury (SCI) as a result of a suicide attempt between 1951 - 1992. OBJECTIVE: To ascertain demographic details of this participant sample, explore and identify the type of psychiatric condition evident around the time of injury, and to review outcome information of this sample with specific focus on mortality, especially further evidence of deliberate self harm. SUMMARY OF BACKGROUND DATA: Research examining suicide rates in SCI populations has found such numbers to be significantly higher than in th

Traumatic spinal cord injuries in Jordan--an epidemiological study.

To survey the situation of traumatic spinal cord injuries (SCI) in Jordan and for a future nationwide epidemiological survey, a retrospective study was conducted at the Royal Jordanian Rehabilitation Centre (RJRC) King Hussein Medical Centre (KHMC) Amman-Jordan, where all traumatic cases within this centre in addition to a few nontraumatic spinal injury patients are referred to the spinal unit which has a capacity of 30 beds. 151 traumatic SCI patients who were admitted to the spinal unit at RJRC during the period January 1988 to December 1993 were reviewed. The estimated annual incidence was …

Spinal cord missile injuries during the Lebanese civil war.

BACKGROUND: Spinal cord injuries due to penetrating wounds are not uncommon. The management of these injuries remains controversial especially with regard to the effect of laminectomy on the neurological outcome. METHODS: Between 1980 and 1989, 64 patients injured by bullets and shell fragments to the spinal cord were reviewed. There were 58 males and 6 females: 24 injuries (37.5%) involved the cervical spine, 37 (57.8%) the dorsal spine, and 3 (4.7%) the lumbar spine. One group (group I) consisted of 47 patients (73.4%) who had immediate and complete sensorimotor loss of function. Another gr…

Cervical spinal cord injuries in patients with cervical spondylosis.

Eighty-eight patients over age 40 with traumatic cervical spinal cord injuries were clinically and radiographically evaluated, and comparison was made with 35 spinal cord injury patients under age 36. While most older patients sustained obvious bony and/or ligamentous damage commensurate with their neurologic findings, 25 (28%) of the 88 patients had no demonstrable bony abnormalities and 17 (20%) of the 88 patients had only minimal evidence of bony injury. Of particular interest are the patients with severe cord injuries, yet no bony abnormalities, who seem to form a distinct subgroup of the …

Rehabilitation after spinal cord injuries complicated by previous lesions.

Since Sir Ludwig Guttmann introduced the comprehensive modern clinical approach to the rehabilitation of spinal cord injured patients much experience has been accumulated. Modern spinal cord injuries centres have opened throughout the world, treating mainly acute spinal cord injuries in young patients. At Stoke Mandeville Hospital we have had the opportunity to treat several spinal patients who had suffered previous neurological or orthopaedic disorders. These complicated combinations affected the rehabilitation process and the presentations show clearly different aspects of the problem.

Five-year review of spinal cord injuries in motorcyclists.

One hundred and twenty motorcyclists with significant spinal cord injuries, who were admitted to hospital over a five-year period, have been reviewed. Fifty-one per cent of them suffered direct injuries to the vertebral column and spinal cord, the majority remaining permanently paraplegic. Thirteen of the 25 riders who became tetraplegic were wearing open-face helmets, and 11 of these remained permanently paralysed. The survey suggests that the open-face helmet offers less protection to the cervical spinal cord than the full-face helmet. There is an urgent need to review standards for the desi…

Spinal Cord Blood Flow, Metabolism, and Neurological Outcome in Patients with Acute, Severe Traumatic Spinal Cord Injuries.

BACKGROUND: We characterized, in patients with severe acute traumatic spinal cord injuries, the relationships between intraoperative spinal cord blood flow (SCBF) and postoperative injury-site metabolism and physiology, preoperative magnetic resonance imaging (MRI) features, and neurological outcome. METHODS: Twenty-six adults with severe, acute traumatic spinal cord injuries (American Spinal Injury Association Impairment Scale, grades A-C) had surgery within 72 h of injury. All had preoperative spine MRI and intraoperative laser speckle contrast imaging of SCBF. For four days after operation…
