Torsion of the wandering spleen.

A case of torsion of the wandering spleen with splenic infarction is reported. The medical imaging studies performed with typical findings are described. Case reports have been reviewed. Although rare, torsion of the spleen remains an important differential diagnosis in patients presenting with acute abdominal pain. Early intervention is necessary to reduce the risk of splenic infarction and other complications, and an increased awareness of the condition together with the use of appropriate medical imaging can lead to the correct diagnosis more readily.

[Epidermoid cyst of the spleen].

Splenic cysts are rarely found or diagnosed. Excluding cases of trauma, the events preceding their development have not been fully understood. We describe a 22-year-old woman in her 34th week of pregnancy in whom ultrasound revealed a cystic lesion 8 x 10 cm. in diameter in the left upper abdomen. Further imaging tests followed by laparotomy confirmed the splenic origin of the cyst. Splenectomy was performed and the lesion was histopathologically defined as an epidermoid cyst.

[Therapeutic embolization of the spleen].

Spleen embolization is a method of percutaneous occlusion of instrasplenic vascular network, by which is achieved a partial embolization of arterial branches with the convenience of controlled circulation reduction. Reduction achieved in this way corrects clinical and laboratory symptoms of hypersplenism and improves hematologic status in the diseases of lymphopoietic tissues where splenectomy is otherwise the therapeutic solution. This method had been applied experimentally, and afterward clinically since 1973, but with numerous complications and incidents. The authors had discussed those com…

[Fibronectin and the human spleen].

Fibronectins are glycoproteins with a function of molecular glue. The aim of the study is to encourage interest in processes in extracellular matrix of the spleen. Undigested cryostat or formol-paraffin sections and commercially available antibodies were used. We were mostly successful in extracellular localization of fibronectin in trabeculae, vessel walls and lamellae of circumferent reticulum of periarterial lymphatic sheaths and in the ring fibres of the sinuses. Changes in the amount of fibronectin occurred, for example, in circulation disturbances. In hereditary spherocytosis the ring fi…

The 'small spleen' in malaria.

A cardinal feature of malaria, splenomegaly, is usually absent in adult patients who have already suffered from falciparum malaria or who are natives of an endemic falciparum zone. This is an attribute of the past episode of clinical or sub-clinical malaria which usually results in regression of the splenic size to below-normal. An ultrasonographic evaluation of spleens was done in 90 healthy adult males, who had suffered from vivax (n = 28) or falciparum (n = 25) malaria in the past, except the controls (n = 22) and natives from an endemic falciparum area (n = 15) who never suffered from mala…

Bacillary angiomatosis of the spleen.

Bacillary angiomatosis is a recently described vasoproliferative lesion associated with infection by a newly characterized rickettsial organism, Rochalimaea henselae. Most previous reports have described skin lesions in immunocompromised patients infected with human immunodeficiency virus. This is the first case report detailing the features of bacillary angiomatosis of the spleen occurring in a patient undergoing cytotoxic chemotherapy for disseminated ovarian carcinoma.

[Epidermoid cyst of the spleen].

A 33-year old woman with 6 x 6 cm splenic cyst of the lower pole of a moderately enlarged spleen, found during ultrasonographic examination for epigastric and left subcostal abdominal pain, is presented. At operation the spleen of 900 grams with a subcapsular cyst in pedunculated extension of the lower splenic pole, ie. accessory lobe, was removed. No other abdominal disease was found. Postoperative recovery was uneventful. Pathohistological examination revealed a cyst, 6 cm in diameter, with whitish wall filled with dark-red turbid fluid containing yellow flocules. The wall of the cyst consis…

Primary lymphoma of the spleen.

Primary lymphoma of the spleen is a relatively rare clinical entity. The majority of patients reported with this entity not only have splenic involvement but also have had lymph node and/or liver involvement. The patient reported presented with splenomegaly and was otherwise asymptomatic. Only splenic involvement was noted at operation and no other manifestation of the disease has become apparent, some 24 months post-splenectomy.

Epidermoid cyst of the spleen.

Epidermoid cysts of the spleen are extremely rare, occurring mainly in people under 20 years of age. They are thought to originate from an abnormality in the development of the spleen during the seventh week of embryological life, when the spleen is close to the mesonephric tissues. Cysts present as an asymptomatic abdominal mass or with pain in the left upper quadrant and/or the left shoulder. In the past the recommended treatment has been splenectomy, but with the changing attitudes towards splenic surgery a more conservative approach is now employed. Drainage under radiographic guidance, de…

[Fat embolism in the spleen].

A fat embolism is seldom seen in the spleen. In some cases, fat globules lie in the central arteries and capillaries of the Malpighian bodies only and in other cases in the red pulp in capillaries or sinus. Part of the fat droplets has been phagocytosed by mononuclear cells, especially when patients die some days after injury and the beginning of fat embolism in the systemic circulation. The distribution of fat emboli described can been explained by the peculiarities of vascular structures in form of sheathed capillaries, which arise from pencil arterioles and probably do not permit fat globul…

Inflammatory pseudotumor of the spleen.

Two examples of an unusual splenic inflammatory pseudotumor are reported. Splenectomy followed detection of a mass lesion by CT scan in one case, and in the other, the pseudotumor was an unexpected and incidental pathologic finding. Despite the alarming gross appearance of these lesions, microscopic features were unequivocally inflammatory and benign. To our knowledge, this entity has not been described previously in the spleen.

Traumatic rupture of the spleen.

Sixty-three cases of traumatic rupture of the spleen were analyzed. The patients were admitted to our hospitals during the last 24 years and there has been a steady increase in the numbers seen. The great majority of these cases resulted from road traffic accidents, and 33% of the patients had multiple injuries. Most were in the second and fourth decades; males outnumbered females by nearly four to one. Associated injuries, which occurred in 33% of cases, significantly raised the mortality. The mortality was also higher in the older age group and in those patients who had multiple injuries and…

Hematopoiesis in the human spleen.

To assess the capacity of the human spleen to function as a hematopoietic organ, spleen mononuclear cells from patients undergoing splenectomy were cultured to assay CFU-E, BFU-E, and CFU-C. Although the peripheral blood from cases of hereditary spherocytosis, Gaucher disease, Thalassemia major, and idiopathic thrombocytopenic purpura yielded 3-20 BFU-E/5 X 10(5) cells plated and 1-7 CFU-C/5 X 10(5), suspensions of spleen cells failed to grow colonies. Histologically these spleens did not show extramedullary hematopoiesis. In contrast, peripheral blood from a patient with osteopetrosis (marble…

Computed tomography of the spleen.

Computed tomography (CT) has proved to be valuable in the diagnosis of a wide variety of congenital, inflammatory, traumatic, and neoplastic lesions of the spleen, in addition to identifying certain normal variants which may present problems. A comprehensive review of the CT characteristics of splenic lesions and variants is provided, and CT is compared with other imaging modalities, principally radionuclide imaging.
