Hyper-reactive malarial splenomegaly syndrome (HMSS) is a massive enlargement of the spleen due to an exaggerated immune response to repeated attacks of malaria. Tropical splenomegaly syndrome (TSS) is the most frequent cause of massive tropical splenomegaly in malarious areas [1-2]. It is seen more commonly among residents of endemic areas of malaria. It occurs mainly in tropical Africa, but also in parts of Vietnam, New Guinea, India, Srilanka, Thailand, Indonesia, South America, and the Middle East. TSS is characterized by massive splenomegaly, hepatomegaly, marked elevations in levels o…

Sub acute left-sided bacterial endocarditis is a serious condition that may be overlooked due to highly variable clinical manifestations. We present the case of a 45-year-old man who presented with complaints of fullness in his abdomen and splenomegaly that referred to the surgery clinic. He underwent diagnostic splenectomy. 3 month after splenectomy endocarditis was diagnosed. We recommend echocardiography in the work up of isolated splenomegaly.

Symptomatic splenomegaly, a frequent manifestation of myelofibrosis (MF), represents a therapeutic challenge. It is frequently accompanied by constitutional symptoms and by anemia or other cytopenias, which make treatment difficult, as the latter are often worsened by most current therapies. Cytoreductive treatment, usually hydroxyurea, is the first-line therapy, being effective in around 40% of the patients, although the effect is often short lived. The immunomodulatory drugs, such as thalidomide or lenalidomide, rarely show a substantial activity in reducing the splenomegaly. Splenectomy can…

An enlarged spleen is a frequent and important clinical sign. It may be a presenting or dominant feature of a number of primary blood diseas … amination and laboratory routine test results as proposed approach to the differential diagnosis of splenomegaly. Progress and changes in management of blood diseases resulted in reappraisal of indications for splenectomy. Progress in surgery has made it possible to perform splenectomy under videolaparoscopic control avoiding laparotomy.

Follow-up of 33 patients with idiopathic splenomegaly, 25 for a period ranging from 14 to 80 months after starting treatment with proguanil 100 mg. daily, showed that there was an excellent response of the splenomegaly, anaemia, and hepatomegaly, together with a definite gain in weight. Every patient improved, though a maximum result was not attained until after at least one year's treatment.Therapy with proguanil is considered superior to and safer than splenectomy. Malaria seems unlikely to have a causal role in the aetiology of the disease, which is probably a manifestation of a disorder of…

We examined a 22-year-old woman who was admitted to our hospital with abdominal distention. At 19 years of age, the patient presented with hepatosplenomegaly. She was examined several times in another hospital; however, the cause was unidentified. Our evaluation showed severe pancytopenia and a spleen 13×24 cm in size. The serum levels of angiotensin-converting enzyme and lysozyme were elevated. She was diagnosed with liver sarcoidosis based on non-caseating epithelioid granuloma in liver biopsy tissue. To improve the symptoms, splenectomy was performed, and her pancytopenia and symptoms impro…

Therapeutic splenic embolization has been commonly performed for hypersplenism, tumor palliation, and other indications, but has not been reported as a therapy specifically for pain relief. We describe 1 patient with chronic pain secondary to severe splenomegaly in whom surgery was contraindicated. Selective partial splenic embolization achieved a pain-free state for the patient without significant complications.

The study of the evolution of 168 cases of idiopathic myeloid splenomegaly allowed to point out the prognostic value of the clinical, hematologic, isotopic and radiological parameters. The correlation of these data with the histological type of the osteomedullary lesions at the time of the diagnosis, allows to confirm the long survival of the hyperplastic formes (type I), and the more reserved prognosis in advanced myelofibroses (type II and type III together) without it being possible to bring out a significant difference between the average presumption of survival of type II and type III con…
