Staphylococcal septic synovitis of the sternoclavicular joint with retrosternal extension.

Bacterial arthritis of the sternoclavicular joint is an uncommon disorder caused by a variety of microorganisms. Both Gram-positive and Gram-negative bacteria have been identified as etiologies of an acute suppurative arthritis, whereas a few other bacteria such as mycobacteria and treponemes have been incriminated in chronic disease of the sternoclavicular joint. We recently treated a patient with staphylococcal synovitis of the sternoclavicular joint, which is the 24th recorded in the literature. His illness was complicated by a retrosternal abscess, soft tissue abscess of the chest, septic …

Sternoclavicular joint involvement in rheumatoid arthritis: clinical and ultrasound findings of a neglected joint.

OBJECTIVE: To describe the prevalence of sternoclavicular (SC) joint involvement and the relationship between clinical and ultrasound (US) findings in patients with rheumatoid arthritis (RA). METHODS: One hundred three consecutive patients with RA and 103 age- and sex-matched healthy individuals were enrolled. Clinical evaluation and blinded US examinations of the SC joint were performed bilaterally in both groups. The presence of gray-scale synovitis, osteophytes, erosions, and intraarticular power Doppler (PD) was recorded. Interobserver agreement was calculated. RESULTS: A total of 412 SC

High resolution computed tomography of the cadaveric sternoclavicular joint: findings in degenerative joint disease.

High resolution, narrow collimation, axial computed tomography of the sternoclavicular joint was used to describe changes secondary to degenerative joint disease in 32 cadaveric specimens. The distribution and pattern of sclerosis, cystic changes, and osteophyte formation in the axial plane were well demonstrated using this technique. Joint space narrowing was sometimes difficult to assess. Subtle joint space calcification was exquisitely demonstrated using computed tomography. Clavicular head cupping, an anatomic variant, may predispose to more severe degenerative change. Computed tomography …

An Unusual Cause of Shoulder Pain: Sternoclavicular Joint Septic Arthritis.

Septic arthritis of the sternoclavicular joint (SCJ) is an uncommon location of septic joint infection and is often a diagnostic challenge. Patients may present with insidious, vague symptoms, such as chest pain or shoulder pain, and imaging modalities are often nonspecific, leading to a delay in diagnosis and treatment. We present a case of a young male patient initially presenting with right shoulder pain, but who then returned days later and was diagnosed with SCJ septic arthritis. This patient was treated successfully with SCJ resection and intravenous antibiotics. This case illustrates th…

A multi-institutional analysis of sternoclavicular joint coverage following osteomyelitis.

BACKGROUND: Sternoclavicular joint (SCJ) osteomyelitis is a rare pathology requiring urgent intervention. Several operative approaches have been described with conflicting reports. Here, we present a multi-institutional study utilizing multiple surgical pathways for SCJ reconstruction. METHODS: A multi-institutional retrospective cohort study was conducted to identify patients who underwent surgical repair for sternoclavicular osteomyelitis between 2008 and 2019. Patients were stratified according to reconstruction approach: single-stage reconstruction with advancement flap and delayed-recons…

Three-point suture anchor repair of traumatic sternoclavicular joint dislocation.

BACKGROUND: Traumatic dislocation of the sternoclavicular joint (SCJ) is a rare injury. Previous techniques are associated with hardware migration, loss of reduction and infectious complications. We aim to describe our preferred surgical technique and summarize our experience with three-point suture technique of acute SCJ dislocation (SCJD). The objective of this study was to describe and assess the safety of three-point suture anchor repair technique in acute traumatic dislocation in the SCJ. METHODS: A 7-year retrospective study in a university affiliated level 1 trauma centre. Operative ca…

Sternoclavicular joint infection in an adult without predisposing risk factors.

Septic arthritis of the sternoclavicular joint (SCJ) is an uncommon condition and it has been associated with numerous predisposing factors. We describe a rare case of SCJ infection due to Staphylococcus aureus in an adult without known underlying predisposing conditions and in which recovery was achieved with medical therapy alone.

Current presentation and optimal surgical management of sternoclavicular joint infections.

BACKGROUND: Infection of the stemoclavicular joint is unusual, and treatment of this entity has not been standardized. We sought to characte … ion at long-term follow-up. CONCLUSIONS: Aggressive surgical management including resection of the sternoclavicular joint and involved ribs with pectoralis flap closure would appear to be the preferred treatment for all but the most minor infections of the sternoclavicular joint. This approach has minimal impact on upper extremity function.

Increasing Incidence of Sternoclavicular Joint Infections in Intravenous Drug Users.

Sternoclavicular joint septic arthritis is rarely seen in the general population. The majority of cases are seen in intravenous drug users. Given the lack of reported cases in the literature, there is no standardized treatment algorithm for this disease. Described treatment methods range from medical management to wide local debridement. The incidence of sternoclavicular joint septic arthritis will likely continue to increase with rising usage of intravenous drugs in the United States. Within the past year, our institution treated four cases.

Morphologic Alterations of the Sternoclavicular Joint following Ipsilateral Clavicle Fractures.

RATIONALE: To our knowledge, no study has investigated concomitant injuries of the sternoclavicular joint (SCJ) in acute clavicle fractures. The purpose of this study was to determine the effect of an ipsilateral clavicle fracture on the SCJ in a systematic computer tomography (CT) morphologic evaluation. METHODS: CT scans in the axial and coronal plane of 45 consecutive patients with clavicle fractures were retrospectively analyzed. The scans were assessed regarding anatomic congruence of bilateral SCJs-joint space width (JSW); the position of bilateral medial clavicles (PC); and the non-fus…

Sternoclavicular joint septic arthritis: an interesting lesson in clinical anatomy.

An otherwise healthy 45-year-old man presented to our emergency department holding his left arm in abduction and external rotation. He reported pain in his left shoulder and chest for the preceding 10 days that had become much worse over the last 48 h and was eased only by holding his arm in this position. The CT of the chest revealed soft tissue swelling around the left sternoclavicular joint consistent with sternoclavicular joint septic arthritis with significant inflammatory involvement of the medial component of the left pectoralis major muscle. The patient was taken to theatre for washout…

The sternoclavicular joint: can imaging differentiate infection from degenerative change?

OBJECTIVE: The purpose of this study was to determine if there are imaging and clinical findings that can differentiate a septic sternoclavicular joint from a degenerative one. MATERIALS AND METHODS: Search of radiology reports from 2000-2007 revealed 460 subjects with imaging of the sternoclavicular joint, of whom 38 had undergone aspiration or biopsy. The final study group consisted of nine subjects with pathologic proof of sternoclavicular joint infection and ten subjects with pathologic and clinical findings excluding infection consistent with degenerative change. Available ultrasound, co…

Sternoclavicular joint septic arthritis with small-colony variant Staphylococcus aureus.

Small-colony variants of Staphylococcus aureus may cause invasive disease in adults that is prolonged and refractory to standard therapies. We present a case of sternoclavicular arthritis with small-colony variant S. aureus that occurred in an 11-year-old child and discuss the importance of identification of these variants in the clinical microbiology laboratory.

Medial Epiphyseal Fracture-detachment of the Sternoclavicular Joint with Posterior Displacement in a Judo Athlete - equivalent of Posterior Sternoclavicular Dislocation.

Posterior sternoclavicular dislocation is a rare traumatic injury that presents a potential risk of injury to mediastinal structures. The diagnosis is fundamentally clinical and treatment is done on an emergency basis. The authors report the clinical case of a young judo athlete with post-traumatic medial epiphyseal fracture-detachment, with posterior displacement (lesion equivalent to posterior sternoclavicular dislocation at pediatric ages). He underwent open reduction and ligament repair by means of a mini-anchor. The radiological and clinical outcome was excellent, and the athlete returned
