We report the use of transesophageal echocardiography in 3 children whose sternums remained open for several days postoperatively. In these patients transesophageal echocardiography provided critical information when transthoracic echocardiography was ineffective due to limited acoustic windows. This report points out the application of transesophageal echocardiography in children with chest wall distortion.

Results of examination and treatment of 24 patients with primary neoplasms of the sternum including chondrosarcoma (18 cases), solitary plasmacytoma (5) and osteogenic sarcoma (1) were analysed. The diagnosis was based mainly on X-ray procedures and was further improved by computed tomography which allowed to precisely evaluate the extent of intrathoracic mass and its relation to adjacent mediastinal structures. We report a large solitary plasmacytoma arising in the sternum of a 62-year-old man. On computed tomography the tumor demonstrated a multilobular appearance associated with bone destruction and expansion and absence of intrinsic calcification. Computed tomography was helpful in determining the size and extent of the tumor and in suggesting the correct diagnosis.

Studies have confirmed that, for severe flail chest or sternal fractures and even multiple rib fractures, surgical treatment can effectively reduce hospital stay and relieve chest wall pain. However, fixation of multiple costal cartilage fractures in such a small area is a challenge if an internal fixator is simply placed directly on the sternum. One of the most severe forms of blunt chest trauma is multiple floating rib fractures. Incidence of this event is up to 20% of all chest injuries. Mortality rate is about 40%.
