Satisfaction of patients with amyotrophic lateral sclerosis with an oral appliance for managing oral self-biting injuries and alterations in their masticatory system: A case-series study.

STATEMENT OF PROBLEM: About 10% of patients with amyotrophic lateral sclerosis (ALS) are candidates for oral treatment specifically because of traumatic injuries in the lips, cheeks, or tongue due to self-biting. However, patients with ALS have a prevalence of temporomandibular disorder (TMD) similar to that in the general population. PURPOSE: The purpose of this case-series study was to determine the degree of satisfaction of patients with ALS with an oral appliance for managing oral self-biting lesions or symptoms related to TMDs. This study also assessed the degree of improvement of the ch

Temporomandibular disorders in head and neck cancers: Overview of specific mechanisms and management.

Diagnosis classification system of Temporomandibular disorders (TMD) is based on the biopsychosocial model of pain. The pathogenesis is poorly understood, leading to difficulties in treating these multifactorial conditions. The predisposing factors are pathophysiological, psychological or structural processes that alter the masticatory system and lead to an increase in the risk of development of TMD. The purpose of this integrative review was then to point out the specific mechanisms of TMD in the oral oncologic context to optimize the TMJ functional results in the management of patients with

Morphofunctional changes of orofacial muscles in patients with unilateral or bilateral cleft lip, alveolus and palate.

The facial musculature is part of a biocybernetic feedback system within the stomatognathic system, the continuity of which is disturbed by clefts of the lip, jaw, and palate (cheilognathopalatoschisis). This results in topographic, fine-structural and functional alterations of the facial musculature. In two heads with unilateral clefts and two heads with bilateral clefts, the facial musculature was dissected and the M. orbicularis oris macroscopically and topographically examined. We found changes in the attachment of the M. orbicularis oris. The modiolus, which is coresponsible for the prope…

[Etiopathogenic and clinical aspects of asymmetry].

Complex dysmorphies, asymmetries can affect the various elements of the stomatognathic system: cranial base, maxilla, mandible, musculature and dental arches, in the three dimensions of space. Their etiopathogenesis, as well as their clinical forms, vary considerably. Dysembryopathies, growth disorders, TMJ damages, postural or functional disorders and dental anomalies can create, according to their severity and their moment of appearance, severe or minor facial asymmetries, or asymmetries of the arches. An exo- and endobuccal clinical examination endeavors to specify the localization and the …

Gnathological splint therapy in temporomandibular joint disorder.

Temporomandibular joint (TMJ) forms an integral functional part of stomatognathic system. Position, shape, structure and function of teeth have an influence on the proper functioning and health of TMJ. But a problem associated with TMJ is often neglected, and treatment for it is mostly restricted to palliative therapy. A proper understanding of the underlying cause of temporomandibular joint disorder (TMD) is necessary to device a proper treatment plan. Etiology of TMDs varies from idiopathic reasons to systemic disorders. The option of Gnathological splint is a conservative, safe and an effec…

Influence of antineoplastic therapy on function of the masticatory system, tooth development, and cariogenic status: a case report.

Antineoplastic therapy causes developmental disturbances in the dental enamel and root if children are treated during tooth development. Increased caries activity has also been reported. The effect of anticancer therapy on the function of the masticatory system (i.e. jaws, dentition, masticatory muscles) is not well known. A case report of a 9-year-old girl with right auricular rhabdomyosarcoma is presented. She received irradiation of 50 Gy to the right auricular area and chemotherapy. A year and a half after cessation of cancer therapy, she was disease free and the clinical stomatognathic ex

Dental occlusion and body balance: A question of environmental constraints?

BACKGROUND: In the past few years, growing interest was given to the relationship between the dental occlusion and the body balance. While most research focused on this relationship at static conditions, it is evident that the contribution of the sensory information for balance control is different depending on the environmental constraints. RESEARCH QUESTION: The aim of the present paper was to elucidate whether the stomatognathic system (SS) contributes differently on body balance regulation according to the presence of external disturbances. METHODS: Literature regarding the different sou…

Undiagnosed myasthenia gravis owing to a very unusual primary presentation.

Myasthenia gravis (MG) is an uncommon autoimmune disorder presenting with fluctuating, progressive muscle weakness. The typical initial presentation includes ocular symptoms of ptosis, diplopia, or both. In the literature, other late symptoms have been described. The inability to close the jaws or difficulty in closing the jaws is an uncommon symptom of muscle weakness. Studies have found that only 4% of patients with MG ever complain of, or demonstrate, masticatory muscular weakness with an inability to close the jaws or difficulty in closing the jaws. In the present case, although the patien

[Effect of the forward head syndrome in the development of temporomandibular disorders].

Bad habits, such as poor posture during the use of technological devices, poor abdominal control and oral breathing pattern can lead to a forward head position, which has important implications for the entire future of the individual, especially at the stomatognathic system. There is a strong association between temporomandibular disorders and forward head syndrome. Where the muscular component is the main affected, with the appearance of more trigger points at the level of the sternocleidomastoid muscles, upper trapezius, rectus capitis posterior and upper oblique capitis. Degenerative change…

Preventive orthodontic approach for functional mandibular shift in early mixed dentition: A case report.

The case reported here highlights the importance of examination of functional occlusion along with static occlusion. Any undetected malocclusion during early mixed dentition has potency to alter the growth and function of stomatognathic system. Premature contacts in primary canine region is the most prevalent cause of functional mandibular shift and can lead to TMJ problems for the prevalence of self correction of such malocclusion is very low if not intervened. A case of functional mandibular shift due to faulty canine guidance because of premature contacts is discussed here. Selective grindi…

Posture and posterior crossbite in oral and nasal breathing children.

The most known etiologic factors of oral breathing may influence the craniofacial development leading to anatomical and functional alterations. A proper head and cervical spine posture allows a well functioning of the stomatognathic system structures and vice versa. This study was conducted to evaluate the prevalence of posterior crossbite in a group of oral breathing children (OB) and nasal (NB) and associate the type of bite with the head and cervical spine posture. It was concluded that most of the children, either oral or nasal breathers, did not present a crossbite and any kind of head po…

[Temporomandibular disorders and headaches].

Temporomandibular disorder (TMD) is a common name for a series of pathologic conditions with similar signs and symptoms, which can lead to a disturbed and altered function of the stomatognathic system. Most of these conditions are of a multifactorial etiology, which can pose difficulties in obtaining a precise and accurate diagnosis. However, TMD is an ever more common and serious problem in contemporary dental practice and at general medical offices, so a more systematic approach in its diagnosis including data collection and interpretation is necessary. Accordingly, accurate diagnosis of TMD…

Macroglossia.

Macroglossia is defined as an enlarged tongue and it is usually clinically diagnosed. Pseudomacryglossia concerns a tongue that is of normal size but gives a false impression of being too large in relation to adjacent anatomical structures. The causes of macroglossia are numerous and this is why various classifications have been proposed for this condition. The consequences of macroglossia usually include a possible malfunction of the stomatognathic system, breathing and speech problems, increased mandible size, tooth spacing, diastema and other orthodontic abnormalities. The treatment of macr…
