The purpose of this study was to report on 12 patients with acquired strabismus following cosmetic blepharoplasty and to identify patterns of strabismus related to the surgical procedure. Clinical ophthalmologic examinations were performed to specifically clarify the type of strabismus. Operative reports of the blepharoplasty procedures were reviewed. PURPOSE: Patients with overcorrected strabismus (and several patients with undercorrection after extraocular muscle resection) underwent exploration of previously operated muscles, with the intention of advancing their tendons to prevent the need for surgery on additional muscles. Fibro-elastic and mechanical factors are more important in reinterventions than in traditional strabismus surgery. It is imperative to detect them preoperatively and intraoperatively. For this purpose, the patient's personal history and complaints are analysed and some specific examination techniques are discussed such as the intraoperative eye position, the forced duction test, the muscle elasticity test and the spring back test.

The prevalence and pattern of cyclodeviation can differentiate among superior oblique palsy, dysthyroid ophthalmopathy, myasthenia gravis, and skew deviation. Excyclodeviation was detected in 30 of 33 patients with superior oblique palsy, eight of 15 patients with dysthyroid ophthalmopathy, three of six patients with nondysthyroid restrictive ophthalmopathy, and one of 13 patients with myasthenia. No cyclodeviation was found in cases of skew. Incyclodeviation was found in two patients with dysthyroid ophthalmopathy, two with nondysthyroid restrictive ophthalmopathy, and three with myasthenia.

Strabismus surgery has been viewed as being associated with a high degree of mortality. Many past studies have used small surgical samples and have stressed the various theoretical causes of the deaths rather than view the procedure from a statistical and scientific standpoint. We present here a large series of surgical cases collected nationwide showing that strabismus surgery is a relatively safe procedure associated with a mortality less than that of a tooth extraction.
