Rhinosinusitis in children: a comparison of patients requiring surgery for acute complications versus chronic disease.

BACKGROUND: Patient characteristics, risk factors, and microbiology are important to consider in the management of complications of acute bacterial sinusitis (ABS) in pediatric patients. This study evaluates this subset of patients and compares them to patients that undergo surgery for chronic rhinosinusitis (CRS). METHODS: This study is a retrospective review of all pediatric patients from 2002 to 2011, who underwent sinus surgery at a tertiary hospital. Patients who underwent surgery for ABS complication were compared to patients who underwent surgery for CRS. Statistical analysis was compl

Biomarker of neutrophil extracellular traps is associated with deep-seated infections and predicts mortality and cardiovascular morbidity in commensal streptococcal bacteremia.

BACKGROUND: Neutrophil extracellular traps (NETs) play important roles in sepsis and deep-seated infections, but whether NET formation corre … The rate of higher MPO-DNA levels (>1.87 μg/mL) were higher in abscess-prone streptococcal groups (streptococcus milleri group) (72.2% vs. 52.5%, p = 0.02). For patients with BSIs due to highly infective endocarditis (IE)-prone pathogens, the values of serum MPO-DNA were also higher in patients diagnosed of IE compared to their counterparts (p = 0.009). Notably, serum from …

Frequent hospital admissions for bacterial infections among aboriginal people with diabetes in central Australia.

OBJECTIVE: To determine hospital-based morbidity and mortality from bacterial infections among Aboriginal people with diabetes in central Australia. DESIGN: Examination of medical records on diabetic inpatients between January 1984 and December 1986. SETTING: Alice Springs Hospital in central Australia, covering an Aboriginal population of about 12,500 people. PATIENTS: Subjects admitted with a bacterial infection (n = 165) were selected from the database of all Aboriginal people known to have diabetes in the region (n = 374; all had non-insulin dependent diabetes). MAIN OUTCOME MEASURES:

Necrotizing fasciitis of the chest wall.

Necrotizing fasciitis (NF) is an uncommon infection caused by microorganisms called 'flesh eating bacteria'. It remains a life-threatening condition associated with high mortality rate. Its location to the chest wall is exceptional. Herein, we report the case of a 39-year-old female, without comorbidity, presenting a NF of the chest wall complicating an empyema. We describe the surgical management with a three-step procedure: antibiotherapy-debridement, vacuum-assisted closure and delayed surgical reconstruction.

[Gangrenous and perforating appendicitis in a provincial hospital: a 48-month retrospective study. Clinical and microbiological aspects, course and postoperative morbidity].

The appendectomy for gangrenous or perforated appendicitis is made more serious by a very high frequency of infection. The antibiotic prophylaxis should be able to reduce the frequency of such complications. We have considered retrospectively the patients who underwent appendectomy for gangrenous and perforated appendicitis in the period from June 1st, 1986 to May 31st 1990 in the Ivrea-Castellamonte hospital (Province of Turin), focusing our attention on certain microbiological and clinical variables in relation to the post-operative course. The average age of the 43 patients admitted to the

[Sepsis after pharyngitis].

We report about a 36-year-old patient, who developped 4 days after a pharyngitis a sepsis with high temperature und recurrent vomiting. The chest radiograph showed multiple pulmonary abcesses and in the computed tomography additionally a thrombosis was detected in a communicans vein between the right jugularis anterior and the jugularis interna. This disease is commonly known as Lemierre Syndrom. The most common pathogen is the Fusobacterium necrophorum, but other bacteria of the normal oropharyngeal flora can also be the causative organisms. Given an adequate antibiotic therapy and supportive

Bilateral intraorbital abscesses and cavernous sinus thromboses secondary to Streptococcus milleri with a favorable outcome.

A 51-year-old woman with left proptosis, diplopia, headache, and nausea was found to have bilateral intraorbital abscesses, left superior ophthalmic vein thrombosis, bilateral cavernous sinus thromboses, and a left temporal lobe intracerebral abscess. Because the paranasal sinuses were unaffected, a dental origin was suspected and confirmed. The causative organism was Streptococcus milleri. Aggressive surgical intervention included bilateral orbital abscess drainage and dental extraction, and medical therapy included intravenous metronidazole, ceftriaxone, heparin, and methylprednisolone. Paediatric periorbital cellulitis and its management.

TOPIC: Periorbital cellulitis is often difficult to distinguish from orbital cellulitis, which is a rare but potentially fatal disease. There are only a few small studies in the literature and we aim to look at an ideal way of managing periorbital cellulitis in a paediatric population using our department's experience. MATERIALS AND METHODS: Retrospective analysis of case notes and computer records of children attending our hospital with periorbital cellulitis over 26 month period. RESULTS: Thirty-four patients met the criteria. Sixteen patients had reduced visual acuity, proptosis or ophtha

The clinical importance of gram-positive anaerobic cocci isolated at St Bartholomew's Hospital, London, in 1987.

The clinical importance of the gram-positive anaerobic cocci (GPAC) isolated in 1987 at St Bartholomew's Hospital, London, is assessed. Of about 800 anaerobic isolates, 209 (27%) were GPAC, of which 67 (32%) were from abscesses and 22 (11%) were in pure growth. Four species comprised 77% of the 168 isolates available for study: Peptostreptococcus magnus (55 isolates, 33%), P. micros (23, 14%), P. asaccharolyticus (24, 14%) P. asaccharolyticus (24, 14%) and P. anaerobius (27, 16%). Different species were associated with different sites, from P. magnus (usually skin-associated sites; normally cu

[A case of urgent aortic valve replacement for infective endocarditis in pregnancy].

A 31-year-old woman underwent urgent aortic valve replacement due to infective endocarditis at 16 weeks' gestation. Because of rapid and severe congestive heart failure, maternal rescue was regarded as the priority in the anesthetic management. Previous literature shows that high-flow high-pressure normothermic perfusion during cardiopulmonary bypass provides the best way of reducing fetal risk. Though it was not performed, the fetus was alive for 24 hours after the operation. Perioperative administration of dopamine and furosemide could have diminished uterine blood flow. If these drugs could

Molecular genetics of resistance to macrolides, lincosamides and streptogramin B (MLS) in streptococci.

Resistance to macrolides-lincosamides-streptogramin B (MLS phenotype) appears in almost all streptococcal species isolated from man. Genes coding for MLS resistance are located on plasmids and one MLS resistance transposon has been described. MLS resistance genes have also been found in a large number of plasmid-free strains. Plasmids of 17 to 20 megadaltons (Mdal) that code either for MLS or for both MLS and chloramphenicol resistance are found in streptococci of groups A, B, C, D (Streptococcus faecalis) and G. These plasmids have broad host ranges (conjugative intraspecies, interspecies and

Auto-aggregation in Streptococcus intermedius is driven by the Pel polysaccharide.

The Streptococcus milleri group (SMG), comprising Streptococcus intermedius, anginosus, and constellatus, can asymptomatically colonize various mucosal sites of healthy individuals. These bacteria are opportunistic pathogens that cause different types of infections across various anatomical sites. Although the pathogenic mechanisms leading to infections are not well defined in the SMG, auto-aggregation is a key driver of biofilm adhesion and cohesion in many Streptococci and Staphylococci. Here, we identify a S. intermedius gene cluster with significant homology to the Bacillus cereus and Pseu…

Ten-year statewide cross-sectional review of pediatric sinogenic intracranial abscess and empyema in Queensland, Australia: microbial profile before and after COVID-19.

OBJECTIVE: Sinogenic intracranial infections in children, such as subdural empyema or intracranial abscess, are a rare disease process with significant associated morbidity. Recent literature has suggested that there may have been an increase in frequency of these infections following the COVID-19 pandemic, but the literature has been conflicting, perhaps related to the heterogenous management of COVID-19 lockdowns in various states and differences in data capture between methods. The collection of statewide Australian data overcomes these limitations by capturing a comprehensive sample though

Metagenome and Metabolic Pathways in Plaque Biofilms of Thai ELANE-Associated Neutropenic Patients: An Original Study and Scoping Review.

OBJECTIVE: Congenital neutropenia, particularly ELANE-associated forms, is associated with recurrent oral infections and aggressive periodontitis. While ELANE deficiency compromises oral health, its relationship to plaque biofilm ecology and metabolic function remains unclear. The oral microbiome-metabolome interplay in this condition remains largely uncharacterized globally. Here, we address this gap by characterizing the dental plaque metagenome and inferred metabolic pathways in a defined cohort of Thai neutropenia patients. MATERIALS AND METHODS: In this exploratory study, we sequenced de
