We report a 79-year-old woman with a decreased level of consciousness. Investigation revealed an elevated serum ammonia and a portal-systemic shunt on angiography. Thus, her symptoms were thought to be due to metabolic encephalopathy. Her electroencephalogram (EEG) showed bilaterally synchronous runs of three-phase waves consistent with the triphasic waves typically seen in metabolic encephalopathy. Alcoholics are predisposed to certain metabolic disorders that cause stupor and coma. These entities include acute ethanol intoxication, ethanol-induced hypoglycemia, alcoholic ketoacidosis, ethylene glycol and methanol intoxications, thiamine deficiency, and hepatic encephalopathy. The recognition and management of these entities and the evaluation of stuporous alcoholics are discussed.

There are several levels of O2 deprivation with different possibilities of adaptation and compensation. All of them appear to be highly conservative in phylogenesis and are active early in ontogenetic development. Two cases are described in which a dissociative stupor originating from conversion neurosis simulated a coma following a sustained trauma. At first both patients showed no response to being addressed or to pain stimuli. They presented an upward eye gaze deviation, cardiorespiratory functions were stable. A case of adrenal carcinoma with severe hypertension referred for treatment of "catatonia" illustrates the need for a high index of suspicion of organic mental disorder in medical patients, even when there is clinical evidence of a psychiatric condition and a history of psychosocial stressors. This case illustrates the need for concurrent medical and psychiatric investigation when there is a problem differentiating medical from psychiatric disturbance.
