Age-related brain-region-specific changes in diffusion anisotropy in subarachnoid space of young adults.

The glymphatic system theory postulates that brain waste is removed through the cerebrospinal fluid (CSF) flow. According to this theory, CSF in the subarachnoid space (SAS) moves to the perivascular space around the penetrating arteries, flows into parenchyma to mix with interstitial fluid and brain waste, and then moves to the perivenous space to be flushed out of the brain. Despite the controversies about the glymphatic theory, it is clear that SAS plays a key role in waste clearance. For instance, the SAS around the middle cerebral artery is known to be highly involved in glymphatic influx…

Cerebrospinal Fluid Flow within Ventricles and Subarachnoid Space Evaluated by Velocity Selective Spin Labeling MRI.

This study aims to evaluate cerebrospinal fluid (CSF) flow dynamics within ventricles, and the subarachnoid space (SAS) using the velocity selective spin labeling (VSSL) MRI method with Fourier-transform-based velocity selective inversion preparation. The study included healthy volunteers who underwent MRI scanning with specific VSSL parameters optimized for CSF flow quantification. The VSSL sequence was calibrated against phase-contrast MRI (PC-MRI) to ensure accurate flow velocity measurements. The CSF flow patterns observed in the ventricles were consistent with those obtained using 3D ampl…

Non-invasive intracranial pressure estimation using ultrasonographic measurement of area of optic nerve subarachnoid space.

OBJECTIVE: To verify whether the area of the ONSAS (ONSASA) obtained by transorbital ultrasonography can be used to accurately evaluate the intracranial pressure (ICP). METHODS: The recorded indexes included the optic nerve diameter, the optic nerve sheath diameter (ONSD), the width of both sides of the ONSAS (ONSASW) at 3 mm from the optic nerve head and the entire ONSASA outlined between 3 and 7 mm. After exploring and comparing five models to describe the relationship between body mass index (BMI), mean arterial blood pressure (MABP), ONSASA and ICP, the best model was determined. RESULTS

Automated detection of imaging features of disproportionately enlarged subarachnoid space hydrocephalus using machine learning methods.

OBJECTIVE: Create an automated classifier for imaging characteristics of disproportionately enlarged subarachnoid space hydrocephalus (DESH), a neuroimaging phenotype of idiopathic normal pressure hydrocephalus (iNPH). METHODS: 1597 patients from the Mayo Clinic Study of Aging (MCSA) were reviewed for imaging characteristics of DESH. One core feature of DESH, the presence of tightened sulci in the high-convexities (THC), was used as a surrogate for the presence of DESH as the expert clinician-defined criterion on which the classifier was trained. Anatomical MRI scans were automatically segmen…

Disproportionately Enlarged Subarachnoid Space Hydrocephalus in Idiopathic Normal-Pressure Hydrocephalus and Its Implication in Pathogenesis.

Idiopathic normal-pressure hydrocephalus (iNPH) has become socially significant in Japan. Japanese guidelines for iNPH in 2011 described the diagnostic importance of "disproportionately enlarged subarachnoid space hydrocephalus" (DESH) on magnetic resonance imaging (MRI). However, some patients with iNPH have equivocal or no features of DESH. To clarify the diversity of MRI findings in iNPH, we classified iNPH into three types based on MRI findings. Using this, we investigate predictable MRI findings for shunt effectiveness in iNPH. A total of 83 patients with suspected iNPH who were treated w…

Morphological changes in the cochlear aqueduct following herpes simplex virus inoculation into the subarachnoid space.

Type 1 herpes simplex virus (HSV-1) was inoculated into the subarachnoid space through the cisterna magna of guinea pigs to study morphological changes of the inner ear and the ability of the cochlear aqueduct to protect the inner ear. Although most of the animals developed clinical manifestations of meningoencephalitis within a few days after inoculation, Preyer's reflex remained intact. Scanning electron microscopy revealed some significant changes in the cochlear aqueduct. Lymphocytes and macrophages were predominant, with narrowing of reticular tissue spaces caused by the swelling of the p…

Extensive grey matter pathology in the cerebellum in multiple sclerosis is linked to inflammation in the subarachnoid space.

AIMS: Multiple sclerosis (MS) is a progressive inflammatory neurological disease affecting myelin, neurons and glia. Demyelination and neuro … parenchyma. CONCLUSIONS: These data suggest that chronic inflammation is widespread throughout the subarachnoid space and contributes to a more severe subpial demyelinating pathology in the cerebellum.

Reversible distension of the subarachnoid space around the optic nerves in a case of idiopathic hypertrophic pachymeningitis.

Idiopathic hypertrophic pachymeningitis (IHP) is a chronic inflammatory disease of unknown cause. We report a case of IHP with bilateral distended subarachnoid space (SAS) of the optic nerves and unilateral visual disturbance. We observed marked amelioration of magnetic resonance (MR) imaging findings after initiation of treatment with prednisolone. This radiological finding implicates optic nerve sheath involvement that affects cerebrospinal fluid (CSF) dynamics around the optic nerve.

Communicating hydrocephalus in adult rats with kaolin obstruction of the basal cisterns or the cortical subarachnoid space.

Communicating hydrocephalus (CH) occurs frequently, but clinically-relevant animal models amenable to diagnostic imaging and cerebrospinal fluid shunting are not available. In order to develop and characterize models of subarachnoid space (SAS) obstruction at the basal cisterns (BC) or cerebral convexities (CX), 25% kaolin was injected in adult female Sprague-Dawley rats following halothane anesthesia; intact- or saline-injected animals served as controls. For BC animals (n=28 hydrocephalics, n=20 controls), an anterior approach to the C1-clivus interval was employed and 30 microl of kaolin or…

Controlled release of lipopolysaccharide in the subarachnoid space of rabbits induces chronic vasospasm in the absence of blood.

OBJECTIVE: Leukocyte-endothelial cell interactions appear to play a role in the development of vasospasm after SAH. Using a purely inflammat … ificant narrowing of the basilar artery was observed by insertion of 20% LPS/EVAc polymers into the subarachnoid space at a polymeric dose of 1.4 mg/kg (actual dose, 66 microg kg(-1) d(-1)) (75.4% +/- 4.2%; P < .01) and by SAH (80.3% +/- 8.1%; P < .01) as compared with the empty polymer group. A trend toward narrowing was observed in the 0.7 mg/kg polymeric LPS dose …

MR demonstration of brain abscess rupture into the subarachnoid space and its possible implication in management.

BACKGROUND: Rupture of brain abscess into the subarachnoid space as a cause of meningitis is rare. Early diagnosis improves the outcome. There is no previous report of MR demonstration of rupture of brain abscess into the subarachnoid space. CASE DESCRIPTION: Two young adults with chronic suppurative otitis media presenting with signs of increased intracranial pressure and meningeal irritation underwent magnetic resonance imaging, which showed brain abscess with evidence of rupture into the subarachnoid space and meningitis. This helped in early diagnosis and aggressive management. CONCLUSIO…

Distribution of Marcaine in an in vitro model of the subarachnoid space conforming to actual spinal column geometries.

To study factors influencing the distribution of local anaesthetics in the subarachnoid space, an in vitro model is constructed which takes into account the natural curvature of the spinal column and the volume occupation of spinal cord and nerve fibres to resemble the in vivo situation. Three Marcaine solutions of different baricity (1003, 1008, 1030 kg/m3) are injected with a 22 G, a 27 G Quincke point needle and a 18 G multiport catheter into three models of non-pathological spinal columns with injection flow speeds of 0.6, 0.2 or 0.1 ml/s. Methylene blue is added for visual and qualitative…

CT-based automatic segmentation of key CSF regions for detecting disproportionately enlarged subarachnoid space hydrocephalus.

BACKGROUND: Disproportionately enlarged subarachnoid space hydrocephalus (DESH) is a characteristic neuroimaging feature of idiopathic normal pressure hydrocephalus (iNPH), a treatable but frequently underdiagnosed condition. While brain MRI enables detailed region-based segmentation, CT is faster and widely used in daily clinical practice. However, objective and automated evaluation of DESH-related features on plain CT remains challenging. This study aimed to develop and validate an artificial intelligence-based segmentation model for automatically extracting four cerebrospinal fluid (CSF) re…

Factors Limiting Complete Resection in the Subarachnoid Space in Endoscopic Surgery for Giant Pituitary Adenoma.

OBJECTIVES: Giant pituitary adenomas (>4 cm, GPAs) have presented great challenges to surgeons because the residual tumor in the subarachnoid space can cause hemorrhage or vessel injury following apoplexy. This study aimed to investigate the factors limiting surgical success in endoscopic skull base surgery (ESS) for GPAs. METHODS: ESS was performed on 67 consecutive patients with GPAs from 2010 to 2020. We retrospectively analyzed the clinical and radiologic features and surgical outcomes. Correlations between the tumor characteristics and extent of resection were statistically presented wit…
