Oral and maxillofacial surgery: what are the French specificities?

PURPOSE: Oral and maxillofacial surgery has expanded rapidly over the past century. Recognition in France has grown since the first face transplantation in the world performed by Professor Bernard Devauchelle. This speciality, which seems to correspond to a narrow scope of services, actually involves oral, plastic, reconstructive, and cosmetic surgeries of the face. French training for maxillofacial surgeons differs from the Anglo-Saxon course of study. After examining surveys carried out in Great Britain, the United States, and Brazil, the perception of this speciality in the general public a

[Differential indication of tracheotomy in oral and maxillofacial surgery].

In the hospital for Oral and Maxillofacial Surgery at the Medical University Hannover 105 tracheostomies were performed between 1980 and 1990. We see a limited indication for a general prophylactic tracheotomy in patients with ablative surgery and microvascular reconstructive procedures. The indication for tracheotomy must be considered in each individual case.

[The use of prophylactic antibiotics in oral and maxillofacial surgery].

Cultures of postoperative wounds after head and neck surgery reveal a variety of oropharyngeal anaerobic and aerobic flora, aerobic gram negative rods and fungi. Control clinical trials have shown that preoperative antibacterial prophylaxis reduces the incidence of postoperative wound infections. When an antibiotic is used prophylactically, it must be active against the micro-organisms known to cause the infection which the patient is supposed to be protected from. Antibacterial prophylaxis should begin just before the operation beginning, earlier is unnecessary and beginning later is less eff

Effect of oral intake initiation-establishment interval on hospital stay after oral cancer surgery.

OBJECTIVES: To clarify the effect of the period between initiation of oral intake (IOI) and establishment of oral intake (EOI) on length of hospital stay. METHODS: This retrospective study included postoperative oral cancer patients. The number of days from surgery to IOI and EOI and between IOI and EOI were recorded. We performed intergroup comparisons and Cox regression analysis using the number of days until discharge, representing hospital stay length as the dependent variable. RESULTS: The median number of days between IOI and EOI was 3 days for eligible patients and 4.5 and 1.5 for old

Specialist practice for minor oral surgery: a comparative audit of third molar surgery.

Cost efficiency of removing third molars in a practice specialising in minor oral surgery was compared to that of a hospital oral surgery department. A total of 100 patients treated in each locality were prospectively audited during the financial year 1989-90. Surgical complexity, waiting time, complications and patient satisfaction were compared. The hospital cases were costed individually and compared to fees received for the patients treated in the practice. The practice fees were also compared to average expenses as assessed by the Dental Rates Study Group Inland Revenue Enquiry. Patients

Evaluation of the Effect of Surgery Type on Preoperative Anxiety in Oral and Maxillofacial Surgery Patients.

INTRODUCTION: In most surgical operations, the type of surgery has been shown to influence preoperative anxiety, which in turn leads to changes during surgery. The aim of this study is to investigate how preoperative anxiety is affected by the type of surgery in patients undergoing maxillofacial surgery under general anesthesia. MATERIAL AND METHODS: This prospective comparative observational study involved patients who underwent oral and maxillofacial surgery at Aydın Adnan Menderes University. Patients were divided into the minor surgery and the major surgery groups. Preoperative anxiety me

Direct Oral Anticoagulant Levels at Time of Elective Surgery.

IMPORTANCE: Before elective surgery, direct oral anticoagulants (DOACs) are discontinued following a standardized protocol. However, this could result in insufficient lowering of DOAC levels that could increase bleeding risk. OBJECTIVE: To estimate the proportion of patients with elevated DOAC levels at the time of elective surgery, evaluate factors associated with DOAC levels, and examine associated blood loss. DESIGN, SETTING, AND PARTICIPANTS: This cohort study (DOAC Level Prior to Incision [DALI]) assessed adult patients prescribed a DOAC (apixaban, dabigatran, or rivaroxaban) for any in

Civil liability in oral & maxillofacial surgery: Α systematic review.

Maxillofacial surgery is a surgical specialty with anatomical, functional, and aesthetic requirements, which makes it a field at increased risk of medical negligence and subsequent legal claims. The review was conducted in accordance with the PRISMA guidelines and used international medical and legal databases. Studies that analyzed court decisions, insurance claims, or recorded compensation data related to maxillofacial surgery were included. The extracted data included, among others, the year and country of publication, the causes of action, and the amounts of financial compensation. Most la

Piezoelectric Surgery and Oral Cavity Disorders in Small Mammals.

Piezoelectric surgery, leveraging ultrasonic vibrations for selective cutting of mineralized tissues while preserving soft tissue integrity, has revolutionized veterinary dental and maxillofacial procedures. This technique enhances precision, reduces postoperative pain, and accelerates healing by minimizing damage to surrounding structures. Its antibacterial cavitation effects and ability to address fragile bone structures make it suitable for rabbits and small exotic mammals. Applications include dental extractions, treatment of osteomyelitis, otitis media and chronic rhinitis. Ultrasonic wou

Early versus delayed oral feeding after major gynaecologic surgery.

BACKGROUND: This is an updated and expanded version of the original Cochrane review, first published in 2014. Postoperative oral intake is traditionally withheld after major abdominal gynaecologic surgery until the return of bowel function. The concern is that early oral intake will result in vomiting and severe paralytic ileus, with subsequent aspiration pneumonia, wound dehiscence, and anastomotic leakage. However, clinical studies suggest that there may be benefits from early postoperative oral intake. Currently, gynaecologic surgery can be performed through various routes: open abdominal,

Versatile midfacial degloving approach in oral and maxillofacial surgery.

OBJECTIVES: Oral and maxillofacial surgeons must gain mastery of various approaches to the midface due to the increasing incidence, complexity, and severity of presenting midfacial fractures. Unlike in the case of other body parts, the need to preserve facial aesthetics makes it more difficult for the surgeon to select an approach for managing the facial injuries. The midfacial degloving (MFD) approach is a combination of intraoral and intranasal incisions made to access the midface without any external incision. The aim of the present study was to evaluate the efficacy of MFD in maxillofacial

Prior oral conditions in patients undergoing heart valve surgery.

INTRODUCTION: Neuraxial anaesthesia is a common choice for most hip and lower limb operations. Pain associated with positioning is often a deterrent, and the vast literature suggests different regional blocks and opioids for these patients. Patients with acetabular fractures may experience increased pain, and thus are more difficult to position for the neuraxial block. We conducted a randomized controlled pilot study to assess and compare the analgesic efficacy of ultrasound-guided suprainguinal fascia iliaca block (SFICB) versus systemic fentanyl to facilitate positioning for combined spinal
