The neurobiology of swallowing and dysphagia.

The neurobiological study of swallowing and its dysfunction, defined as dysphagia, has evolved over two centuries beginning with electrical stimulation applied directly to the central nervous system, and then followed by systematic investigations that have used lesioning, transmagnetic stimulation, magnetoencephalography, and functional magnetic resonance imaging. The field has evolved from mapping the central neural pathway and peripheral nerves, to defining the importance of specific regions of the lower brain stem in terms of interneurons that provide sequential control for multiple muscles

Swallowed partial dentures.

Swallowed or inhaled partial dentures can present a diagnostic challenge. Three new cases are described, one of them near-fatal because of vascular erosion and haemorrhage. The published work points to the importance of good design and proper maintenance. The key to early recognition is awareness of the hazard by denture-wearers, carers and clinicians.

Swallowing in patients with tracheotomies.

Swallowing and respiration are well-coordinated and interdependent functions. When one of these processes is impaired, the ramifications may be negative for the other. This article describes the mechanics of normal swallowing, the disorders of swallowing, the effects of tracheotomy and mechanical ventilation on swallowing, and the procedures used to assess and treat swallowing. Combining a basic understanding of these concepts with practical management can increase safe and efficient oral intake in patients with artificial airways.

Swallowing in Alzheimer's disease.

Using a prospective case series design, we examined the incidence of oropharyngeal swallowing abnormalities in 25 patients with moderate or severe Alzheimer's disease by videofluoroscopy. Aspiration occurred in 6 of 25 (28.6%). Only four patients showed unequivocally normal performance. Exploratory statistical analyses showed that swallowing abnormalities associated significantly with duration of dementia, eating dependency, and abnormal oral praxis. We observed a trend toward a higher incidence of aspiration in patients with more severe dementia. We conclude that oropharyngeal swallowing abno

Swallowing disorders in the elderly.

The classic symptoms of dysphagia, heartburn, and chest pain are the presenting manifestation of oropharyngeal and esophageal disorders in the elderly as well as the young. Several unique conditions, including Zenker's diverticulum and vascular compression of the esophagus, may occur in the elderly. Certain disorders, such as oropharyngeal dysphagia due to neurologic disorder, increase in frequency with age. Treatment approaches are similar in elderly and younger patients, but the potential for adverse drug effects and interaction is greater in the elderly than in the young.

Manofluorographic analysis of swallowing.

A quantitative evaluation of the pharyngeal swallow is described using a new method--manofluorography. Eleven parameters have been established, and the clinical significance of the most important ones has been highlighted. The propulsive tongue action and hypopharyngeal suction pump have been designated as the major bolus driving forces.

Physiological and pathological roles of the accommodation response in lower esophageal sphincter relaxation during wet swallows.

The preparatory accommodation response of lower esophageal sphincter (LES) before swallowing is one of the mechanisms involved in LES relaxation during wet swallows, however, the physiological and/or pathological roles of LES accommodation remain to be determined in humans. To address this problem, we conducted a prospective observational study of 38 patients with normal high-resolution manometry (HRM) and 23 patients with idiopathic esophagogastric junction outflow obstruction (EGJOO) to assess dry and wet swallows. The LES accommodation measurement was proposed for practical use in evaluatin…

Chronic noise exposure has context-dependent effects on stress physiology in nestling Tree Swallows (Tachycineta bicolor).

Anthropogenic noise is increasing in intensity and scope, resulting in changes to acoustic landscapes and largely negative effects on a range of species. In birds, noise can mask acoustic signals used in a variety of communication systems, including parent-offspring communication. As a result, nestling birds raised in noise may have challenges soliciting food from parents and avoiding detection by predators. Given that passerine nestlings are confined to a nest and therefore cannot escape these challenges, noise may also act as a chronic stressor during their development. Here, we raised Tree

Tongue pressure generation during tongue-hold swallows in young healthy adults measured with different tongue positions.

Tongue-hold swallow (THS) has the potential to be a resistance exercise not only for the pharyngeal constrictor but for the tongue muscles. To elucidate the physiological mechanisms of THS, this study investigated intraoral pressure generation during THS in relation to different extents of tongue protrusion. Tongue pressure was measured by a 5-point pressure sensor sheet placed onto the hard palate of 18 healthy young subjects who performed three swallow tasks: normal dry swallow, THS with slight tongue protrusion, and THS with greater tongue protrusion. Subjects randomly repeated each task fi

Conformal Swallowing Accelerometry: Reimagining the Acquisition and Characterization of Swallowing Mechano-Acoustic Signals.

(1) Background: Non-invasive instrumental measurement of swallowing acoustic signals has rested upon the assumptions of signal symmetry and reproducibility along the cervical region and has hence taken the form of single-point acquisition on optimal sites. This study aimed to (i) revisit such assumptions by adopting a conformal array of accelerometers, and hence (ii) lay the foundation for the future design of swallowing accelerometry. (2) Methods: Thirteen young healthy individuals, including eight females (mean age ± SD = 24.38 ± 0.92) and five males (mean age ± SD = 24 ± 3.74), were recruit

A revised version of the Seirei Swallowing Questionnaire for people with cognitive decline (Swallow-10).

The Seirei dysphagia screening questionnaire (namely, Seirei dsq) has been used to screen for oropharyngeal dysphagia (OD). Some of the questions in the questionnaire are difficult for people with cognitive decline to answer. We selected 10 items from the Seirei dsq that could be assessed by others, such as care workers (named Swallow-10). This study aims to verify the validity of the scoring method used in Swallow-10. The dataset used in this study was the one used in the development of Seirei dsq : 50 patients with cerebrovascular disease who were diagnosed with OD, 145 patients with cerebro

Swallowing Dynamics and Breathing-Swallowing Coordination Are Altered in Patients with Mild Cognitive Impairment.

INTRODUCTION: Postinspiratory activity, which is essential for laryngeal closure during swallowing to prevent aspiration of food into the airways, is reduced in a mouse model of tauopathy. Therefore, we hypothesized that patients at the stage of mild cognitive impairment (MCI) exhibit alterations in swallowing dynamics and coordination between swallowing and breathing. METHODS: We examined breathing-swallowing coordination in patients with MCI. Patients who scored ≥24 on the Mini-Mental State Examination and <26 on the Japanese version of the Montreal Cognitive Assessment were recruited at Su

The Effect of the Cervical Orthosis on Swallowing Physiology and Cervical Spine Motion During Swallowing.

Cervical orthosis is used to immobilize the neck in various disorders such as trauma and post-operation. However, it is still uncertain how cervical orthosis restricts the degree of movement of the cervical spine during swallowing and how they affect swallowing physiology. The purpose of this study was to evaluate these issues using the Philadelphia(®) Collar. We conducted videofluorography of swallowing in 39 healthy subjects (23 men, 16 women; mean age of 34.3 years) with and without cervical orthosis. To compare the two conditions regarding the cervical spine motion, we determined the angul
