Task-switching inefficiencies in currently ill, but not remitted anorexia nervosa.

OBJECTIVE: Models of anorexia nervosa (AN) posit that set-shifting deficits may contribute to behavioral inflexibility and extreme dietary restriction. Findings from neurocognitive studies of set-shifting in AN have been somewhat mixed, perhaps due to the use of tasks that cannot distinguish shifting from other processes (i.e., learning). To more precisely characterize cognitive flexibility and selectively assess this process independent of rule learning and feedback sensitivity, we examined task-switching ability in AN. METHOD: Women ill with AN, subthreshold AN or atypical AN (IAN; n = 40),

Verbal Mediation During Shifting in School-Aged Children With Developmental Language Disorder.

PURPOSE: Children with developmental language disorder (DLD) often exhibit deficits in executive function. These deficits may be linked to less effective verbal mediation or the use of self-directed language to guide task performance. This study examined the role of verbal mediation during an executive shifting task. METHOD: Twenty-seven children aged 8-12 years (15 with DLD, 12 with typical development [TD]) completed a cued local-global task with verbal mediation (baseline) and without (articulatory suppression). Shifting performance was measured as accuracy and reaction time. Trial-type co

Set-shifting among adolescents with bulimic spectrum eating disorders.

OBJECTIVE: Set-shifting difficulties are observed among adults with bulimia nervosa (BN). This study aimed to assess whether adolescents with BN and BN spectrum eating disorders exhibit set-shifting problems relative to healthy controls. METHODS: Neurocognitive data from 23 adolescents with BN were compared with those from 31 adolescents with BN-type eating disorder not otherwise specified and 22 healthy controls on various measures of set-shifting (Trail Making Task [shift task], Color-Word Interference, Wisconsin Card Sorting Test, and Brixton Spatial Anticipation Task). RESULTS: No signif

Individual differences in cognitive control: the role of psychoticism and working memory in set-shifting.

Set-shifting refers to a process of cognitive control which is shown through flexible behavioural adaptation to changes in task parameters or demands, such as the switching of an explicit rule (extra-dimensional rule shifting) or the reversal of a reinforcement-contingency (reversal-learning). Set-shifting deficits are widely documented in specific neuropsychological disorders, but seldom investigated in relation to normally-occurring individual differences. In a sample of healthy adults (N=78, 28% male), we demonstrate that Working Memory and trait Psychoticism have independent involvement in

Multiple dopamine receptor subtypes in the medial prefrontal cortex of the rat regulate set-shifting.

Dopamine (DA) input to the prefrontal cortex (PFC), acting on D1 receptors, plays an essential role in mediating working memory functions. In comparison, less is known about the importance of distinct PFC DA receptor subtypes in mediating executive functions such as set-shifting. The present study assessed the effects of microinfusion of D2 and D4 receptor antagonists, and D1, D2, and D4 receptor agonists into the PFC on performance of a maze-based set-shifting task. In Experiment 1, rats were trained on a response discrimination task, and then on a visual-cue discrimination task requiring rat

Attentional Demand of Motor Speech Encoding: Evidence From Parkinson's Disease.

PURPOSE: While the involvement of attention in utterance planning is well established at the conceptual and lexical levels, the attentional demands of postlexical processes are still debated. This study investigates the involvement of attentional resources on motor speech encoding during utterance production in the context of Parkinson's disease (PD), a population allowing to assess if the attentional demands observed in a dual-task paradigm (the dual-task costs [DTCs]) are explained by postlexical difficulties and not solely by executive impairment. METHOD: Speech production was analyzed in

Cognitive overcontrol as a trait marker in anorexia nervosa? Aberrant task- and response-set switching in remitted patients.

Individuals with anorexia nervosa (AN) often present inflexible behaviors and rigid thinking styles, which may contribute to disorder maintenance. Studies of set shifting have documented impairments in AN, but results have varied across samples. Moreover, the hypothesis that deficient set shifting may constitute an endophenotype rests largely on observations made with neuropsychological tests with limited ability to isolate component cognitive control processes. The current behavioral study used a task switching paradigm with a demonstrated ability to fractionate the hierarchical organization

Community health worker-supported oral health promotion in low- and middle-income countries: a scoping review of roles, interventions, and outcomes.

BACKGROUND: Oral diseases are among the most prevalent conditions worldwide and disproportionately affect populations in low- and middle-income countries (LMICs). The shortage and maldistribution of the oral health workforce have widened inequalities in prevention and treatment. Task-sharing through community health workers (CHWs) has been promoted as a cost-effective and sustainable strategy for extending services to underserved populations; however, evidence on their roles in oral health promotion in LMICs remains fragmented. This scoping review mapped evidence on CHW-supported oral health p

Interprofessional collaboration between nurses and physicians in intensive care and nurse's role in task allocation: A scoping review.

OBJECTIVES: Interprofessional collaboration is essential in intensive care units, where care is complex and highly demanding. However, overlapping tasks and responsibilities may create unclear professional boundaries between nurses and physicians and nursing roles. This review aimed to map international research on interprofessional collaboration between physicians and nursing professionals in ICUs, with a focus on task and responsibility sharing and the description of nursing roles within collaborative practice. METHODS: This scoping review followed PRISMA-ScR guidelines. A comprehensive sea

Motor interference on lateral pelvis shifting towards the paretic leg during walking and its cortical mechanisms in persons with stroke.

Motor interference, where new skill acquisition disrupts the performance of a previously learned skill, is a critical yet underexplored factor in gait rehabilitation post-stroke. This study investigates the interference effects of two different practice schedules, applying interleaved (ABA condition) and intermittent (A-A condition) pulling force to the pelvis during treadmill walking, on lateral pelvis shifting towards the paretic leg in individuals with stroke. Task A involved applying resistive pelvis force (pulling towards the non-paretic side), and Task B applied assistive force (pulling

Multiple components of lateral posterior parietal activation associated with cognitive set shifting.

Posterior parietal activation has commonly been observed in previous neuroimaging studies in association with flexible shifting of cognitive set. However, it is not clear whether the parietal activation reflects cognitive processes intrinsic to the shifting itself or other confounding factors such as spatial attention. To address this issue, the Wisconsin Card Sorting Task (WCST) was modified such that spatial components were eliminated from the sensory and motor aspects of the task. Moreover, a visual instruction of a next dimension was introduced to eliminate cognitive processes related to t

Prefrontal cortex and hippocampus in behavioural flexibility and posttraumatic functional recovery: reversal learning and set-shifting in rats.

Within one experiment and one T-maze, we examined the consequences of (i) bilateral lesions of the anteromedial prefrontal cortex (PFC), (ii) bilateral transections of the fimbria-fornix (FF), or (iii) combined lesions of both PFC and FF (COMB) on rats' ability to perform reversal or set-shifting. Postoperatively, the animals were trained to perform a spatial discrimination go-right task. This was followed by (1) a spatial reversal go-left task (reversal learning), or (2) a visual pattern discrimination task (set-shift). Neither single (PFC or FF) lesion nor combined (COMB) lesions affected th

Differential Neural Correlates of Set-Shifting in the Bingeing-Purging and Restrictive Subtypes of Anorexia Nervosa: An fMRI Study.

In this study, possible differences in the neural correlates of set-shifting abilities between the restrictive (AN-R) and bingeing/purging (AN-BP) subtypes of anorexia nervosa have been explored. Three groups of participants performed a set-shifting task during functional magnetic resonance imaging: patients with AN-R (N = 16), AN-BP (N = 13) and healthy control participants (N = 15). As in a typical set-shifting experiment, participants had to switch between two easy tasks (i.e. 'Is the presented number odd/even' or 'Is the presented number smaller/larger than 5'). The trials in which the tas

Cognitive and Experienced Flexibility in Patients With Anorexia Nervosa and Obsessive Compulsive Disorder.

OBJECTIVE: Anorexia nervosa (AN) and obsessive-compulsive disorder (OCD) share a neuropsychological profile characterized by cognitive inflexibility as evident in set-shifting problems, and by strong detail focus. Clinically, both patient groups display a strong rigidity which may be explained by these neurocognitive difficulties. Cognitive inflexibility may hinder treatment uptake and help explain suboptimal treatment outcomes in both AN and OCD. This is the first study to compare clinical AN and OCD groups andto examine similarities and differences in cognitive flexibility. Specifically, thi

Selective cognitive impairment in the YAC128 Huntington's disease mouse.

People with HD have a demonstrated early extra-dimensional set-shifting deficit. In the present study, we use a novel water T-maze set-shifting procedure and demonstrate its validity as a set-shifting task in a mouse model of Huntington's disease. Three groups of YAC128 mice of different ages (27, 69 and 117 weeks) were run on the task, which incorporated six distinct stages in which the mice must learn a rule and then switch to a different rule. The six stages were: directional learning, directional learning reversal, light discrimination, light discrimination reversal, return to place learni

An Empirical Study of Executive Function in College Students: The Predictive Role of Health-Promoting Behaviors and the Time-Course Effects of Motor-Cognitive Dual-Task Training.

BACKGROUND: As people’s quality of life improves, maintaining long-term physical, mental, and cognitive health has become a focus of attention. However, the effects of different health-promoting behaviors on executive function (EF) are still unclear. In daily life, motor-cognitive dual-tasks (MCDTs) often need to be performed simultaneously. Research on MCDT training mainly focuses on the elderly, the sick, athletes, and other groups, with a lack of research on college students, who represent a population of normal young people. Based on this, the present study investigated the predictive effe

Testing a "content meets process" model of depression vulnerability and rumination: Exploring the moderating role of set-shifting deficits.

BACKGROUND AND OBJECTIVES: MacCoon and Newman's (2006) "content meets process" model posits that deficits in cognitive control make it difficult to disengage from negative cognitions caused by a negative cognitive style (NCS). The present study examined if the interactive effect of cognitive set-shifting abilities and NCS predicts rumination and past history of depression. METHODS: Participants were 90 previously depressed individuals and 95 never depressed individuals. We administered three laboratory tasks that assess set-shifting: the Wisconsin Card-Sorting Task, the Emotional Card-Sorting

Perfectionism and cognitive rigidity in anorexia nervosa: Is there an association?

Little is known about the relationship between neuropsychology, personality, and eating psychopathology in anorexia nervosa (AN). We aimed to investigate the interaction between set shifting and perfectionism in AN and to ascertain the role of perfectionism as a mediator between set shifting and eating psychopathology. Eighty-five patients with AN and 71 healthy controls completed Eating Disorder Inventory-2 (using 8 as a cut-off for generating groups with high vs. low perfectionism), Beck Depression Inventory, Wisconsin Card Sorting Test, Trail Making Task, and Hayling Sentence Completion Tas
