Dilemma in the Treatment of Sports Injuries in Athletes: Tendon Overuse, Muscle Strain, and Tendon Rupture.

Injuries to the musculoskeletal system are frequent in elite sports and they are detrimental to athletic performance. This can be due to, for example, (1) overuse disorders of tendon (tendinopathy) that not only lower the training efficiency but also, in many cases, are career-ending for the athlete due to pain; (2) acute muscle strain injury that both causes prolonged absence from competition and results in many re-injuries; or (3) tendon rupture that, apart from a very long rehabilitation period, will also result in many athletes never resuming their original high level of competitive sports

[Close injury of the tendon at wrist].

Because of the complicated causes and variable clinical signs, closed injury of tendons at wrist is difficult to diagnosis and treat. Twenty-six cases of tendon ruptur were reported. Among them, 11 cases were caused by bone fracture or dislocation, 8 cases were caused by rheumatoid synovitis, 5 cases were caused by synovial tuberculosis, and 2 cases caused by other. The pathogenesis and clinical signs were analyzed. Twenty-three cases were treated by tendon transfer and 3 cases were treated by tendon transplantation. By average follow-up of 16 months (ranged 6 months to 4 years), the results w

[Injuries to the extensor tendons of the fingers].

The main risks in case of extensor tendon laceration are firstly, the possibility of misdiagnosis, and secondly the occurrence of adhesions which can compromise the excursion of the extensor apparatus. Extensor tendon lesions at the dorsum of the wrist and hand have a better prognosis regarding the postoperative results than lesions at the finger level. Protected early mobilisation methods represent a real advance in the postoperative management of extensor tendon lesions.

Injuries of the flexor and extensor tendons.

Unless there is vascular compromise, tendon injuries of the hand need not be considered emergencies and can be successfully treated with primary or even delayed primary repair. Methods of treatment for common injuries of the flexor and extensor tendons are discussed.

The climber's finger : imaging of finger flexor tendon pulley injuries.

INTRODUCTION: Finger flexor tendon (FT) pulleys are delicate anatomical structures whose primary function is to anchor the tendon sheaths to the underlying bone. They maintain the tendons in close proximity to the phalanges, thereby stabilizing them during finger flexion. This arrangement allows for efficient force transmission, preserves an effective lever arm, and prevents bowstringing, ensuring a full range of motion. These structures can be accurately assessed using high-resolution ultrasound (US) or high-field magnetic resonance imaging (MRI). METHODOLOGY: We conducted a structured narra

Rehabilitation following surgery for flexor tendon injuries of the hand.

BACKGROUND: Various rehabilitation treatments may be offered following surgery for flexor tendon injuries of the hand. Rehabilitation often includes a combination of an exercise regimen and an orthosis, plus other rehabilitation treatments, usually delivered together. The effectiveness of these interventions remains unclear. OBJECTIVES: To assess the effects (benefits and harms) of different rehabilitation interventions after surgery for flexor tendon injuries of the hand. SEARCH METHODS: We searched the Cochrane Central Register of Controlled Trials, the Cochrane Bone, Joint and Muscle Trau…

Complications Following Transosseous Repair of Zone I Flexor Tendon Injuries.

PURPOSE: Multiple techniques are described for repair of zone I flexor tendon injuries, many of which are fraught with complications. This study evaluated the clinical complications after a transosseous repair technique. METHODS: A retrospective review of a single institutional database identified all zone I flexor digitorum profundus (FDP) injuries repaired using a transosseous technique. In this technique, 2 nonabsorbable sutures were passed from volar to dorsal through transosseous tunnels and tied dorsally over the distal phalanx proximal to the germinal matrix. Demographics, injury chara…

[Patellar tendon injuries after total knee arthroplasty : Classification and management].

BACKGROUND: Ruptures of the patellar tendon after total knee arthroplasty represent a rare but severe complication, which in general requires surgical therapy. OBJECTIVES: To implement a classification and correspondent therapy algorithm in consideration of the current literature for the treatment of patellar tendon ruptures after TKA. MATERIAL AND METHODS: A review of the recent literature and the author's experience are summarized in a classification and correspondent therapy algorithm for the treatment of patellar tendon ruptures after TKA. RESULTS: Ruptures of the patella tendon can be

CONCLUSIONS: Subscapularis tendon injuries in adolescents are rare. Diagnosis requires careful attention to the physical examination and imaging. [Primary treatment of acute extensor tendon injuries of the hand].

OBJECTIVE: Reconstruction of extensor functions after extensor tendon injuries of the hand. INDICATIONS: Acute injuries of extensor mechanism with corresponding loss of function. CONTRAINDICATIONS: Complex injuries with loss of soft tissue. Limited possibility of extensor tendon reconstruction with combined injuries of the interphalangeal joints (in situations with irreparable joints: primary arthrodesis). SURGICAL TECHNIQUE: The treatment of extensor tendon injuries depends on the various levels of tendon laceration. Zones 1 and 2: in case of tendon disruption close to the base of the dist…

Achilles tendon sports injuries: a review of classification and treatment.

Achilles tendon injuries are among the three most frequent sports-related injuries of the foot and ankle. Proper function of the Achilles tendon is critical to performance in sports. A thorough knowledge of the anatomy and biomechanical function of this tendon is essential to the effective treatment of these injuries. Distinguishing among the various pathologies of the Achilles tendon is an important first step toward successful treatment and return of the athlete to sports activity. The term Achilles tendinitis is a nonspecific diagnosis that does not accurately describe an actual injury. Thi…

Closed tendon injuries of the hand and wrist in athletes.

Closed-tendon injuries of the hand and wrist are very common in the athletic population. Most of these injuries, if seen acutely, can be treated successfully with nonoperative management, although some do require operative treatment. It is those injuries, however, that go undiagnosed until the end of the season, which may result in permanent disability.

Early active mobilization for extensor tendon injuries. The Norwich regime.

Dynamic splinting following extensor tendon repair gives better results than static splinting, but involves cumbersome splints and recommended protocols are often complicated. We prefer controlled active mobilization of extensor tendon repairs without dynamic splinting. Six weeks after repair, excellent or good function was obtained in 22 out of 24 simple extensor tendon injuries and in 11 out of 13 complex injuries. The results of this prospective study are comparable with those reported after dynamic splinting; this regime does not require outrigger splintage and is simple to follow.

Presurgical Perspectives and Post-treatment Evaluation of Achilles Tendon Injuries.

The Achilles is a commonly injured tendon, prone to degeneration and tear. Achilles tendon treatment ranges from conservative management to injections, tenotomy, open versus percutaneous tendon repair, graft reconstruction, and flexor hallucis longus transfer. Interpreting postoperative imaging of the Achilles tendon is a difficult task for many providers. This article clarifies these issues by presenting the imaging findings following standard treatments and depicts expected appearance versus recurrent tear and other complications.
