The author considers the anatomic basis and indications for a dynamic peroneus brevis tendon transfer to treat ankle instability and pain caused by rupture of the lateral fibular ligaments. He describes the surgical technique. The peroneus brevis tendon is passed through a tunnel in the distal fibula and cuboid bone, proximally and deep to the belly of the short toe extensor muscle and is looped back and sutured to itself. Thirty-two patients with ankle instability and pain of 10 years' duration or acute ligamentous rupture underwent dynamic peroneus brevis tendon transfer in a prospective stu…

With tendon transfers or reconstructions, the tenorrhaphy must be strong enough to withstand early mobilization in the immediate postoperative period to decrease adhesion formation and optimize functional outcomes. The purpose of this study was to compare the strength, bulk, and gliding resistance of four common tendon-to-tendon attachment constructs. A biomechanical study was performed utilizing 80 cadaveric tendons to compare four common tendon tenorrhaphy constructs: the end-weave (EW); Pulvertaft (PT); single-pass, side-to-side (SP-STS); and simple, side-to-side (STS) attachments. The prim…

This review summaries the knowledge of the treatment of peroneal nerve palsy. Isolated peroneal nerve palsy is often seen after fracture of the knee or knee dislocation. In cases with chronic peroneal nerve palsy and foot drop, tendon transfer of the posterior tibial muscle tendon to the dorsum of the foot is a possibility to be considered. This procedure is indicated for isolated peroneal nerve palsy with good ankle mobility, good strength of the posterior tibial muscle and no chance of spontaneous remission.

Complex digital deformities and metatarsophalangeal joint instability encompass a wide range of pathology, and we must identify the different degrees of ligamentous disruption. It is important to address a combination of procedures to treat gross deformities of the lesser toes. Surgical treatment should be individualized and requires a sequential process for adequate reduction and deformity correction. There is no gold standard procedure for every deformity. Although residual stiffness can result from tendon transfer, overall patient satisfaction levels remain high when it is performed under t…

Hallux varus is an infrequently encountered deformity of the first ray characterized by a medial deviation of the hallux on the first metatarsal head at the first metatarsal phalangeal joint. Iatrogenic flexible hallux varus often requires surgical repair to create a functional, pain-free, shoeable foot. Although arthrodesis remains the mainstay of treatment, many soft tissue transfer procedures have been described in the literature as joint-sparing alternatives to fusion. This article explores in detail the tendon transfer procedures that have been described for repair of flexible hallux varu…

Tendon transfers are used to restore balance and function to a paralyzed, injured, or absent neuromuscular-motor unit. In general, tendon transfer is indicated for restoration of muscle function after peripheral nerve injury, injury to the brachial plexus or spinal cord, or irreparable injury to tendon or muscle. The goal is to improve the balance of a neurologically impaired hand. In the upper extremity, tendon transfers are most commonly used to restore function following injury to the radial, median, and ulnar nerves. An understanding of the general principles of tendon transfer is importan…

OBJECTIVES: The purpose of this study was to evaluate the results of tendon transfers in patients with irreparable radial nerve palsy.
METHODS: The study included 29 patients (5 females, 24 males; mean age 29 years; range 9 to 51 years) who underwent tendon transfers for radial nerve palsy. Four patients had low- and 25 patients had high-level radial nerve palsy. The mean duration of tendon transfer after injury (paralysis time) was 19.7 months (range 1 day to 180 months). Clinical evaluations included joint range of motion and muscle strengths. Functional evaluation was made using the Tajima…

Pathology of the biceps tendon is often a factor in the etiology of shoulder pain. However, diagnosis and treatment of such pathology remains controversial. When conservative management fails to relieve symptoms, surgical options include tenotomy or tenodesis. Tenotomy has provided excellent results with regard to local pain relief, but a potential cosmetic deformity and occasional painful cramping are common in younger patients. Tenodesis has also had high failure rates resulting from persistent local pain. We have used an all-arthroscopic technique for transfer of the long-head biceps to the

Flexor to extensor transfer was used to treat painful second metatarsophalangeal joint instability in thirteen feet in eleven patients. All patients had their pain reproduced with vertical stress motion of 50% to 100% at the metatarsophalangeal joint. Seven feet had concomitant hallux valgus correction, two feet had no hallux valgus, and four feet underwent no correction for asymptomatic hallux valgus. Results at an average of 33.4 months followup showed that all patients had substantial pain relief, with eight patients becoming pain-free, and five patients experiencing mild pain. All but one

Between 1983 and 1988, 32 patients with cervical spinal cord injuries underwent 124 upper limb tendon transfers during 85 procedures. Of the numerous surgical procedures that have been recommended for treatment of this condition, my experience indicates that the most successful are posterior deltoid-to-triceps transfer, restoration of finger flexion, and restoration of thumb opposition. Longitudinal incisions prove to be quite cosmetic.

1. Nerve injuries in the lower extremity not infrequently result in paralysis of the muscles innervated by the injured nerve. 2. Disability caused by residual paralysis can be reduced by appropriate muscle transfers. 3. Deformity must be corrected before muscle transfer. 4. The most useful transfers in the lower extremities are the transfer of the tibialis posterior tendon anteriorly for correction of the equinovarus deformity of peroneal nerve injury; the transfer of the tibialis anterior with or without the peroneus longus to the calcaneus to prevent calcaneus deformity caused by loss of the

INTRODUCTION AND IMPORTANCE: Irreparable sciatic nerve palsy is a cause of foot drop and resulting in absent or weak most of the muscles in … pting Achilles tendon and plantaris tendon following irreparable sciatic nerve palsy was presented. Tendon transfer using medial gastrocnemius tendon, lateral gastrocnemius tendon and plantaris tendon seemed to be a good choice for treatment of this injury. It allowed reconstruction of a stable, painless, plantigrade foot.
LEVEL OF EVIDENCE: Case report.

The posterior tibial tendon (PTT) is the principal dynamic stabilizer of the medial longitudinal arch of the foot. The basic goal of surgically reconstructing PCFD is to restore the foot's medial longitudinal arch, often through a combination of bony and soft tissue procedures. While the FDL transfer has long been the gold standard for reconstruction, allograft reconstruction of the PTT has recently been increasing in popularity.

The treatment of the massive irreparable rotator cuff tear poses a challenging problem. Tendon transfers offer a solution for irreparable posterosuperior rotator cuff tears. The lower trapezius tendon transfer with incorporation of an Achilles tendon allograft has emerged as an effective way to restore strength and function in select patients. Both open and arthroscopic-assisted techniques have been described.

Calcific insertional Achilles tendinopathy can result in significant pain and disability. Although some patients respond to nonoperative therapy, many patients are at risk for long-term morbidity and unpredictable clinical outcomes. There is no evidence-based data to support the timing of operative invention, choice of procedures, or whether equinus requires treatment. This article suggests the need for a classification system based on physical examination and imaging to help guide treatment. There is an obvious need for evidence-based studies evaluating outcomes and for properly conducted sci

IMPORTANCE: To our knowledge, orthodromic temporalis tendon transfer (OTTT) for dynamic facial reanimation has not been described for use in children.
OBSERVATIONS: Three pediatric patients with permanent facial paralysis underwent OTTT using our modified technique between August 30, 2010, and January 23, 2012. Outcomes were assessed by the surgeons, patients, and patient families, with the longest follow-up period being 13 months after surgery. Two patients were 4 years old at the time of surgery, and the third patient was 17 years old. All underwent upper eyelid gold weight placement and OT…

BACKGROUND: The transfer of the tibialis posterior tendon to the dorsum of foot can restore the function of theparalyzed dorsiflexor muscles of the foot and ankle. In order to reduce the wound complication in the insertionsite of tendon to bone by a plantar knop we used a new method of fixation by an absorbable screw inserted dorsally.
METHODS: we performed this operation on 15 patients in a 3 years period. All patients had drop foot deformitydue to irreversible lesions of the peroneal nerve. The inclusion criteria was the peroneal nerve palsy that wasnot improved after 18 months even by usin

Dynamic extensor carpi radialis longus tendon transfer to the distal pole of the scaphoid acts synchronously and synergistically with wrist motion to restore the slider crank mechanism of the scaphoid after scapholunate interosseous ligament (SLIL) injury. The procedure is designed to simulate a hypothetical dorsal radioscaphoid ligament that more closely approximates the normal viscoelastic forces acting on the scaphoid throughout all phases of wrist motion than does the static checkrein effect and motion limitations of capsulodesis or tenodesis. Extensor carpi radialis longus transfer may be…
