The Etiology, Antibiotic Therapy and Outcomes of Bacteremic Skin and Soft-Tissue Infections in Onco-Hematological Patients.

OBJECTIVES: to assess the current epidemiology, antibiotic therapy and outcomes of onco- hematological patients with bacteremic skin and soft-tissue infections (SSTIs), and to identify the risk factors for Gram-negative bacilli (GNB) infection and for early and overall mortality. METHODS: episodes of bacteremic SSTIs occurring in cancer patients at two hospitals were prospectively recorded and retrospectively analyzed. RESULTS: Of 164 episodes of bacteremic SSTIs, 53% occurred in patients with solid tumors and 47% with hematological malignancies. GNB represented 45.5% of all episodes, led by

Laser and light-based therapy for cutaneous and soft-tissue metastases of malignant melanoma: a systematic review.

Invasive cutaneous melanoma is a growing health concern. Although surgical excision can effectively treat in situ tumors, use for metastatic melanoma is limited. Laser and light-based therapies may be a valuable palliative treatment option for patients with stage III and stage IV cutaneous metastatic melanoma. Our goal is to review the published literature and provide evidence-based recommendations on laser and light-based palliative therapies for metastatic melanoma. A search of the databases Pubmed, EMBASE, Web of Science, and CINAHL was performed on March 10, 2016. Key search terms were rel

How influential is the stiffness of the foam dressing on soft tissue loads in negative pressure wound therapy?

Negative pressure wound therapy (NPWT) is an established adjunctive modality for treatment of both acute and chronic wounds. However, little is known about the optimal settings and combination of treatment parameters and importantly, how these translate to target tissue strains and stresses that would result the fastest healing and buildup of good-quality tissues. Here we have used a three-dimensional open wound computational (finite element) model that contains viscoelastic skin, adipose and skeletal muscle tissue components for determining the states of tissue strains and stresses in and aro

Skin and soft tissue infections in biological therapy for psoriasis-A case report and systematic review of the literature.

BACKGROUND: Biological therapies are widely used for moderate to severe chronic plaque psoriasis owing to their high efficacy and safety profile. However, skin and soft tissue infections (SSTIs) have been reported in association with biological treatment in psoriasis. METHODS: We report a case of necrotizing fasciitis in an 18-year-old psoriasis patient with a history of severe combined immunodeficiency treated with secukinumab and conducted a systematic literature review of SSTIs associated with biological therapy for psoriasis. The literature review related to biological therapies for psori

Antibiotic therapy for skin and soft tissue infections: a protocol for a systematic review and network meta-analysis.

BACKGROUND: Skin and soft tissue infections (SSTIs) in hospital and community settings impose a substantial socio-economic burden. Therapeutic uncertainty due to the availability of a wide range of antibiotics and the need for empirical treatment decisions complicate SSTI clinical management. Completion of numerous pairwise meta-analyses to account for this variability in antibiotics is impractical, and many head-to-head comparisons of potential interest are likely not available. In comparing multiple antibiotics simultaneously, this network meta-analysis aims to identify the antibiotic(s) wit

[Application of negative-pressure wound therapy in complex therapy of open tibia fractures IIIB and IIIC with massive soft tissue loss].

Patients with high-grade open tibia fractures and massive soft tissue damage pose a demanding clinical challenge, requiring a complex inter-disciplinary approach and multiple orthopedic, vascular and plastic-reconstructive procedures. These types of fractures still have a 50% risk of loss, although amputation remains a treatment method of last resort. In our experience negative-pressure wound therapy (NPWT), used in the form of the vacuum assisted closure - VAC System (KCI, USA) improves healing of excessive soft tissue damage with exposure of the bone accompanying open fracture of the tibia.

Association of Prior Local Therapy with Outcomes Within Systemic Therapy Regimen Cohorts in Advanced Soft Tissue Sarcoma: A Retrospective Cohort Study.

Background/Objectives: Evidence regarding associations between local therapy before an indexed systemic regimen and subsequent outcomes in advanced soft tissue sarcoma (STS) remains limited. We evaluated outcomes within four regimen-defined cohorts and explored associations with prior local-therapy history. Methods: This retrospective cohort included 75 patients with recurrent or metastatic STS who contributed 155 treatment episodes: doxorubicin (n = 44), trabectedin (n = 31), pazopanib (n = 42), and eribulin (n = 38). Progression-free survival (PFS) and overall survival (OS) were measured fro

Emerging, personalized, and minimally invasive technologies for periodontal and peri-implant soft and hard tissue regenerative therapy.

BACKGROUND: Regenerative therapies have become central to periodontics and implant dentistry and are becoming more widely applied to reconstruct supporting bone and associated soft tissues. With increased recognition of the benefits of using biomimetic materials as scaffolds in contemporary regenerative technologies, the field is seeking to develop and optimize predictable, personalized regenerative strategies for hard and soft tissue reconstruction. This review aims to: (1) evaluate the current evidence on these approaches in the context of hard and soft tissue deficiencies around teeth and i

Multi-Targeted Kinase Inhibitor Therapy in Pediatric Bone and Soft Tissue Sarcoma Patients-A Single Centre Experience.

BACKGROUND: Patients with relapsed or refractory soft tissue and bone sarcomas have dismal outcomes. Multi-targeted kinase inhibitors (mTKI) have proven to be potent agents in several malignancies, both as primer therapy and as a salvage option. Our aim was to evaluate the clinical outcomes of mTKI treatment in a heterogeneous group of pediatric sarcoma patients retrospectively. PROCEDURES: A total of 18 patients were treated with sorafenib, regorafenib, or pazopanib; 13 of them had osteosarcoma (OSC), 3 had synovial sarcoma (SySa), and 1-1 patient had chondrosarcoma and rhabdomyosarcoma. Instrument-assisted soft tissue mobilization and percussion massage therapy in cervical disc herniation: a randomized controlled study.

BACKGROUND: Cervical disc herniation (CDH) is a common musculoskeletal disorder characterized by chronic neck pain, impaired proprioception, kinesiophobia, and functional limitations, often requiring multimodal conservative care. Myofascial techniques, including Instrument-Assisted Soft Tissue Mobilization (IASTM) and percussion massage therapy (PMT), have emerged as supportive physiotherapy interventions. This randomized controlled trial compared the effects of IASTM and PMT on pain, disability, kinesiophobia, and proprioceptive function in individuals with CDH. METHODS: In this double-blind

Assessing Functional Outcomes and Health-Related Quality of Life After Radiation Therapy in Extremity Soft Tissue Sarcoma.

AIMS: Extremity soft-tissue sarcomas (ESTS) are rare neoplasms treated with limb-sparing surgery and radiation therapy (RT). While RT improves local control, it can be associated with late functional impairments that may affect health-related quality of life (HR-QOL). As survivorship care gains importance, understanding the relationship between functional outcomes (FO) and HR-QOL is crucial. This study evaluates the relationship between FO, measured by the Toronto Extremity Salvage Score (TESS), and HR-QOL, assessed with the European Organisation for Research and Treatment of Cancer Quality of
