Testicular Teratocarcinoma in a Ferret (Mustela putorius furo).

A 7-month-old male domestic ferret (Mustela putorius furo) was presented for evaluation of unilateral testicular enlargement. Microscopic examination of the left testicle revealed a neoplasm with differentiation along multiple cell lines (ectoderm, endoderm, mesoderm) including respiratory epithelium, bone and haired skin. A poorly differentiated epithelial component was dispersed throughout the neoplasm with invasion of testicular lymphatics. The animal developed progressive dysuria and was euthanized. At necropsy, metastasis of the poorly differentiated epithelial component was present in th

Two cases of interathoracic pathologies presented as secondory hypertrophic osteoarthopathy.

The vast majority of cases of Hypertrophic osteoarthropathy are associated with intra thoracic neoplasms mainly broncogenic cancer and benign fibrous tumor. Less commonly extra thoracic pathologies can present with this disorder. Two cases are presented here. The first is a 50-years-old male who came with complaints of significant weight loss, chest symptoms and multiple upper extremities joint pain. The second case was a 44 years old female who presented similarly with chest symptoms, joint and bone pain limited to lower extremities bilaterally. Chest x-ray and x-ray of extremities were sugge…

Pleural decortication and hyperthermic intrathoracic chemotherapy for pseudomyxoma.

Aim: Pleural dissemination of pseudomyxoma peritonei (PMP) is an extremely rare diagnosis, for which no standard therapy is available. Methods: We describe the successful treatment of a 67-year-old male diagnosed with left-sided intrapleural dissemination of PMP (low-grade appendiceal mucinous neoplasm), 2 years after treatment of abdominal PMP with cytoreductive surgery (CRS) and hyperthermic intra-peritoneal chemotherapy. Treatment consisted of extended pleural decortication (ePD) and oxaliplatin-based hyperthermic intrathoracic chemotherapy (HITHOC). The patient is doing well without complic

Ganglioneuroma, heterochromia iridis, and Horner's syndrome.

A child with mediastinal and supraclavicular ganglioneuroma, heterochromia iridis, and Horner's syndrome is described. The authors postulate that the thoracic neoplasm originated as a congenital neuroblastoma with metastasis to the supraclavicular lymph nodes and subsequent benign transformation, and that the ocular abnormalities resulted from sympathetic ganglion injury by the tumor in infancy. This seems to be the first published report of such ocular abnormalities associated with mediastinal ganglioneuroma.

[Prevalence of the neuroendocrine phenotype in thymus neoplasms].

To determine the prevalence of neuroendocrine differentiation in human thymic neoplasms, which are unusual tumours that may range from well-differentiated to overtly malignant, poorly differentiated lesions, an immunohistochemical study was conducted in 23 thymic neoplasms re-classified on the basis of the new 1999 WHO classification. Immunohistochemical evidence of neuroendocrine differentiation in the form of reactivity to the markers synaptophysin, neuron-specific enolase and chromogranin A was found in 6 of 23 tissues (26%). Two of 3 patients with thymic carcinoids (or well-differentiated

Normal and abnormal vascular structures that simulate neoplasms on chest radiographs: clues to the diagnosis.

Despite increased use of and reliance on cross-sectional imaging techniques in the thorax, conventional chest radiography remains the most commonly performed imaging examination. However, conventional radiographic appearances of normal and abnormal vascular structures can be misinterpreted as representing neoplasms or soft-tissue masses and lead to inappropriate diagnostic procedures. Vascular structures that can simulate neoplasms include normal structures such as the subclavian artery and left brachiocephalic, azygos, and pulmonary veins and abnormal structures such as congenital and acquire

Primary intrathoracic low-grade fibromyxoid sarcoma.

Low-grade fibromyxoid sarcoma (LGFMS) is a rare neoplasm commonly affecting young adults and typically arising in the somatic soft tissue of the proximal extremities. Its occurrence within the thoracic cavity is exceedingly rare. We report an LGFMS arising from the epicardial surface of the right side of the heart in a 44-year-old woman. Diagnosis was aided by a strikingly characteristic light microscopic appearance of a bland spindle cell sarcoma containing numerous so-called giant collagen rosettes and supported by immunohistology, evidence of FUS translocation by fluorescence in situ hybrid

Anterior Mediastinal Neuroblastoma Associated with Syndrome of Inappropriate Antidiuretic Hormone Secretion: A Morphologic, Immunohistochemical, and Genetic Case Report and Review of the Literature.

We report a mediastinal neuroblastoma in an octogenarian with paraneoplastic syndrome of inappropriate antidiuretic hormone secretion (SIADH). Neuroblastomas are very rare tumors in adults, with thoracic or mediastinal locations being especially uncommon. These neoplasms have been occasionally associated with the SIADH. Given the rarity of incidence and paucity of diagnostic and outcomes data, the significance of standard neuroblastoma prognostic characteristics is unclear, and no treatment paradigms exist for these patients. Further studies are needed to inform future clinical guidelines.

Leiomyoma of esophagus presenting as huge posterior mediastinal mass: 2 case reports.

Smooth muscle tumors presenting as mediastinal soft tissue masses are rare and are often mistaken for other neoplasms. Benign tumors of the esophagus account for fewer than 1% of all esophageal tumors, the most common of which are leiomyomas. They are frequently located in the lower third of the esophagus. We report two cases of esophageal leiomyoma in the thoracic esophagus which were firstly presented as huge mediastinal mass with intermittent dysphagia and radiologically flecks of calcification.

[A rare cause of hoarseness].

BACKGROUND: Hoarseness caused by Ortner's syndrome is a rare diagnosis, first described in 1897 in a patient with an enlarged left atrium due to mitral valve stenosis. Due to mechanical compression on the left recurrent laryngeal nerve patients present with hoarseness. CASE DESCIPTION: A 70 year old male presents with hoarseness and an inability to swallow. Examination reveals a paresis of the left vocal cord. A Computed Tomography scan shows a saccular aneurysm of the proximal descending thoracic aorta, consistent with Ortner's syndrome. Patient was successfully treated with a thoracic endov

Low-attenuation thoracic metastases in testicular malignancy.

Low-density abdominal and pelvic lymph nodes are seen in nearly half of patients with testicular cancers. Thoracic low-attenuation metastases are much less common. Three cases of low-density metastases to lung parenchyma and mediastinal nodes are presented. In a man of appropriate age, a low-attenuation pulmonary or mediastinal mass should prompt a search for a primary testicular neoplasm.

Imaging of haemodialysis: renal and extrarenal findings.

Electrolyte alterations and extra-renal disorders are quite frequent in patients undergoing haemodialysis or peritoneal dialysis. The native kidneys may be the site of important pathologies in patients undergoing dialysis, especially in the form of acquired renal cystic disease with frequent malignant transformation. Renal neoplasms represents an important complication of haemodialysis-associated acquired cystic kidney disease and imaging surveillance is suggested. Extra-renal complications include renal osteodistrophy, brown tumours, and thoracic and cardiovascular complications. Other import

Thoracic pheochromocytoma revealed by ventricular tachycardia. Clinical case and review of the literature.

A case of thoracic extra-adrenal pheochromocytoma was observed in a 13-year-old girl. The initial clinical symptoms consisted of her being unwell after effort over a period of 3 years. The diagnosis was suggested after a recorded episode of paroxysmal ventricular tachycardia with concomitant hypertension, which reproduced the characteristic symptoms. Biochemical analysis confirmed hypersecretion of catecholamines and morphological examinations showed the presence of an isolated left median thoracic tumor. One year after surgical excision, the patient remained completely asymptomatic, in spite

Dual-Energy CT Material Decomposition in Pediatric Thoracic Oncology.

Technical advances in CT have enabled implementation of dual-energy CT into routine clinical practice. By acquiring images at two different energy spectra, dual-energy CT enables material decomposition, allowing generation of material- and energy-specific images. Material-specific images include virtual nonenhanced images and iodine-specific images (iodine maps). Energy-specific images include virtual monoenergetic images. The reconstructed images can provide unique qualitative and quantitative information about tissue composition and contrast media distribution. In thoracic oncologic imaging,

Reconstruction of complex defects of the parotid region using a lateral thoracic wall donor site.

Radical treatment of parotid neoplasms may lead to complex parotid defects that present functional and aesthetic reconstructive challenges. We report our experience using the lateral thoracic wall as a single donor site. Between 2003 and 2009, four patients with malignant tumours in the parotid gland underwent radical parotidectomy and simultaneous reconstruction using a perforator latissimus dorsi cutaneous free flap (de-epithelialized and entire skin paddle in two cases each). A thoracodorsal nerve graft was used in all cases to replace the intraglandular branches of the facial nerve. Costal

Changes in categorization or nomenclature within neuroendocrine tumors.

The 5th edition of the World Health Organization (WHO) classification of neuroendocrine neoplasms (NENs) is built to achieve a uniform terminology and to define a similar diagnostic scheme across different anatomic locations. Since the 4th edition, a chapter discussing NENs in nonneuroendocrine organs has been introduced, which proposes a binary system for classification segregating well-differentiated neoplasms, termed neuroendocrine tumors (NETs), and poorly differentiated neoplasms, termed neuroendocrine carcinomas (NECs). A grading system for NETs is based on mitotic index and/or Ki-67 ind

Problems in pathologic staging of lung cancer.

CONTEXT: Tumor stage is the most important prognostic and predictive factor for patients with lung cancer, the most lethal neoplasm in the United States. It is used by thoracic surgeons, radiation therapists, and oncologists to determine whether patients with these neoplasms will be treated surgically with curative intent or with palliative radiation therapy and/or chemotherapy. OBJECTIVE: To review the variety of practical problems that can arise during the assessment of the pathologic stage and other prognostic/predictive factors included in the College of American Pathologist checklist for

[Solitary fibrous tumor. Two additional cases with urologic implications].

Solitary fibrous tumour is a well defined pathological entity originally described as a tumour of the pleura, but the occurrence of this neoplasm has increasingly been described at other sites. At present the development of these tumours is recognized as possible in virtually all anatomical sites. The rarity of this type of pathology has not allowed up to the present time clarification of the histogenesis, clinical behaviour, treatment and prognosis of these tumours. Differential diagnosis becomes important, especially in extra-thoracic sites so as not to mistake them for much more aggressive
