OBJECTIVE: To study the incidence and type of pulmonary function abnormalities after thoracotomy in children.
METHODS: Children below 12 y of age who had undergone thoracotomy for any condition and have at least 2 y follow up were included in the study. Detailed assessment of the patients included history and general examination, clinical assessment of pulmonary function, bedside tests to assess pulmonary function and laboratory pulmonary function test using portable spirometer.

A 55-year-old woman who had a history of percutaneous vertebroplasty was referred to our institution with sudden onset of chest pain. Computed tomography (CT) scan demonstrated a long, linear, highly-attenuated segment in the right side of the heart and fragmented pieces in the right pulmonary artery. This report describes a 22-year-old male who presented with penetrating trauma to the left second intercostal space. He was found to be clinically stable; however, Focused Assessment with Sonogrpahy in Trauma (FAST) exam revealed large pericardial effusion suspicious for cardiac injury. Management of the patient with penetrating trauma to the chest is discussed and appropriate literature is reviewed. This report supports delayed thoracotomy in the appropriately selected patient as safe. It avoids the morbidity and unnecessary dangers incumbent in emergency department (ED) thoracotomy.

The authors retrospectively review their experience in children with the latest addition to the postoperative analgesic armamentarium: interpleural analgesia (IPA). IPA was used in 14 children following thoracotomy. There were 9 boys and 5 girls. Patients varied in age from 2 months to 17 years 4 months (mean +/- SEM = 7.6 +/- 1.6 yr). The effect of indomethacin on postoperative pain was studied in 60 adult patients undergoing thoracotomy in a prospective, randomized, double-blind manner. Patients receiving indomethacin required significantly less opioid after operation and had significantly lower pain scores compared with the control group. Pain on movement and on coughing were reduced also. No major adverse effects were encountered.

Twenty-two patients treated with cryoanalgesia of the intercostal nerves in connection with thoracic surgery were re-examined with special attention to possible late neurological sequelae. Half of the patients had some degree of hypoaesthesia in the scar or the adjacent skin segment, but apart from one patient (a known morphine addict) none of the patients had any neurological complaints.

Seventeen consecutive patients underwent pull-through esophagectomy using blunt dissection from laparotomy and cervical incisions for carcinoma of the esophagus. Fifteen patients had a middle-third lesion while 2 patients had a distal-third lesion. INTRODUCTION: Emergency resuscitative thoracotomy (ERT) is a resource-intensive procedure that can deplete a combat surgical team's supply and divert attention from casualties with more survivable injuries. Two cases of pneumothorax occurring during thoracotomy under one-lung ventilation are reported. One case occurred before pneumonectomy was carried out. The decrease in Spo2 shown by pulse oximetry, together with the increase in airway pressures, rapidly led to the diagnosis of pneumothorax. Between 1987 and 1990 twenty report cases who suffered carcinoma of the esophagus underwent transmediastinal esophagectomy without thoracotomy at the Surgery Service of the Edgardo Rehabilitation Hospital. Of the report cases 90% were at stage III, 5% of stage II and 5% at stage I of the TNM Classification.
