A retrospective study was conducted at the Hospital Sultanah Aminah Johor Bahru to determine the outcome of trabeculectomy surgeries over a period of 4 years. One hundred and two eyes were followed up to a maximum of 63 months (mean 34.2 months). The 2-year survival rates for plain trabeculectomies, 5-Fluorouracil augmented trabeculectomies and Mitomycin-C augmented trabeculectomies were 52.9%, 27.3% and 60.5% respectively. The commonest complications noted were cataract formation (25%) and hyphaema (11%). Mitomycin-C induced complications were rarely seen. At last follow-up, 54% of eyes had i…

A comparison of 90 day-case and 102 in-patient trabeculectomies is presented. There was no statistically significant difference in the incidence of post-operative hyphaema (Chi-squared test p = 0.84), shallow anterior chamber (p = 0.46), flat bleb (p = 0.84) or hypotony (p = 0.87) between the two groups. Casualty attendances, readmissions and further surgery in the postoperative period were seen with similar frequency in each group. Intraocular pressure control three months following surgery was not compromised by undergoing the procedure as a day-case (p = 0.29). This study demonstrates the s

Sixteen eyes requiring surgical reformation of the anterior chamber (AC) in our Unit from December 1987 to August 1991 were studied retrospectively to determine the incidence of the problem in our hospital and to evaluate the operative measures used to restore the AC. The mean preoperative AC depth of these 16 eyes was 1.92 mm (range, 1.50 to 2.50 mm). Nine had a history of angle-closure glaucoma. Poor surgical technique was a contributing factor in the development of a flat AC in 3. Of the remaining 13 eyes, 11 had a trabeculectomy with a fornix-based, and 2 with a limbus-based, conjunctival …

In this retrospective study the results of trabeculectomy in 21 patients with open-angle glaucoma and no previous eye surgery who were followed up for at least 1 year were analysed. In 10 patients sodium hyaluronate had been injected during the operation to cushion the anterior chamber; the other 11 patients served as controls. The two groups had been comparable preoperatively with respect to characteristics assessed postoperatively. The subjects in the sodium hyaluronate group had better visual acuity and less postoperative hypotony. Although the series was too small to statistically validate…

PRECIS: This clinical trial compares a modified trabeculectomy technique [extended subscleral tunnel (ESST)] with conventional trabeculectomy [subscleral trabeculectomy (SST)] in terms of success rate and bleb morphology. ESST showed comparable results, with lower incidence of bleb-related complications and need for postoperative antiglaucoma medications.
BACKGROUND: To evaluate the outcome of modified trabeculectomy with extended subscleral tunnel "ESST" versus conventional subscleral trabeculectomy "SST" in the management of uncontrolled primary open-angle glaucoma.

The creation of a scleral flap during trabeculectomy can be complicated by a buttonhole, partial amputation at the limbus, and extensive thinning. In some cases, the procedure must be aborted to prevent more serious postoperative complications. This report describes a technique of converting complicated trabeculectomy into ab externo cyclodialysis. A 41-year-old patient with congenital glaucoma presented with a perforated scleral wall with the choroidal tissue exposed during the dissection of the partial-thickness scleral flap. By using a Barraquer cyclodialysis spatula through the scleral per…

PURPOSE: To investigate the long-term changes in lamina cribrosa (LC) depth following trabeculectomy.
METHODS: Serial horizontal B-scan images of the optic nerve head were obtained using spectral-domain optical coherence tomography from 28 primary open-angle glaucoma patients who underwent trabeculectomy and were followed up for at least 2 years. Approximately 65 B-scans covering the optic discs were obtained before surgery and at 6 months and ≥2 years postoperatively. The pre- and postoperative LC depth (the distance from the opening plane of Bruch's membrane to the level of the anterior LC …

Trabeculectomy with anti-fibrotic treatment is still the most popular incisional procedure for glaucoma filtration surgery (GFS) worldwide. The advent of anti-fibrotic agents reduced failure due to scarring but resulted in increased complications. Advances in trabeculectomy surgery have been driven by the need to minimise the risk of: (1) complications and (2) surgical failure. This chapter covers preoperative, intraoperative and postoperative strategies, which improve the outcome of GFS. Strategies to reduce the risk of complications centre on the prevention of postoperative hypotony by minim…
