The Role of Companion Animals in the Transition of Care: A Case Report.

Numerous individuals experiencing homelessness have a pet. When a homeless person is hospitalized for an emergency medical need, discharge planners are sometimes faced with tough options and a lack of resources for safe discharges from the hospital. We detail the case of a 64-year-old female patient who was admitted due to a witnessed syncopal event. The patient was admitted with her companion dog, which remained at the bedside through her hospitalization. The workup for her syncopal event was negative (CT brain, carotid US, ECG, troponins, orthostatic). Her discharge process was complicated b

Transition to Adult Rheumatology Care: A Disease-Specific Guide.

Young adults with childhood-onset rheumatic diseases are more frequently establishing and continuing care with adult rheumatologists. The transfer of care can be challenging for both the young adult patients and their adult rheumatologists, in large part due to differences between pediatric-onset rheumatic diseases and their adult-onset counterparts, or due to the rarity of some pediatric-onset rheumatic conditions. Other challenges are due to cultural differences between pediatric and adult medical care and to the young adult needing to increasingly perform self-management skills that were pr

Progressive Supranuclear Palsy: Challenges and Considerations for Care Transitions.

Progressive supranuclear palsy (PSP) is a fatal neurodegenerative disorder. Care requires a patient-centered approach encompassing compassion, communication, and empathy. Despite concerted actions to streamline PSP patient transitions, the care is multifaceted and cause of concern. Patients with PSP undergoing transitional care are at an increased risk of undesirable outcomes, frequently endure poor communication, and encounter inconsistent care. Therefore, patients with PSP and families worry about the uncertainty in care, including who is accountable for the care and available resources. Thr…

Patient Perspectives on Care Transitions From Hospital to Home.

IMPORTANCE: Understanding the patient's perspective of their care transition process from hospital or skilled nursing facility (SNF) to home may highlight gaps in care and inform system improvements. OBJECTIVE: To gather data about patients' care transition experiences and factors associated with follow-up appointment completion. DESIGN, SETTING, AND PARTICIPANTS: A survey tool was developed with input from patient advisors and organizations participating in a collaborative quality initiative. Seventeen hospitals, 12 practitioner organizations, and 6 SNFs in Michigan collaborated to identify

Transition of care in patients with epidermolysis bullosa: A survey study.

There are little published data on the transition of care in EB. We conducted a survey study recruiting EB patients from the Dystrophic EB Research Association (debra) website and centers caring for high numbers of EB patients in the United States and internationally from Sept 17, 2019 to Nov 3, 2021. The majority of participants had not discussed the transition of care with their healthcare providers, nor the healthcare needs to be required as an adult. Ongoing pediatric subspecialty care was reported by 12% of adults, most commonly in pediatric dermatology. Identified barriers to transition

Ongoing care for the patient with an anorectal malfromation; transitioning to adulthood.

Planned health care transition can improve the ability of young adults to manage their own health care to effecively use health services and ultimately maximize life-long functioning and well-being. Transitional care is a purposeful, planned process that addresses the medical, psychosocial and educational needs of adolescents and young adults with chronic physical and medical conditions as they move from child-centered to adult-oriented healthcare systems. Unsuccessful surgical transtion may result in physical and mental health implications for young patients, negative long-term outcomes and s…

Transition to adult care for young people with cerebral palsy.

Cerebral palsy (CP) is associated with a high burden of comorbid respiratory disease subsequent to multiple risk factors associated with increasing levels of disability. Correspondingly, respiratory disease is the leading cause of death in CP, including amongst young people who are transitioning or who have just transitioned between paediatric and adult healthcare services. Therefore, consideration of both preventive and therapeutic respiratory management is integral to transition in patients with CP, as summarised in this review.

Transition of patients with mucopolysaccharidosis from paediatric to adult care.

Mucopolysaccharidoses (MPS) are rare disorders associated with enzyme deficiencies, resulting in glycosaminoglycan (GAG) accumulation in multiple organ systems. As patients increasingly survive to adulthood, the need for a smooth transition into adult care is essential. Using case studies, we outline strategies and highlight the challenges of transition, illustrating practical solutions that may be used to optimise the transition process for patients with MPS disorders. Seven MPS case studies were provided by four European inherited metabolic disease centres; six of these patients transferred

Patient perspective on care transitions after colorectal surgery.

BACKGROUND: The surgical care pathway is characterized by multiple transitions, from preoperative assessment to inpatient stay, discharge from hospital, and follow-up care. Breakdowns in one phase can affect subsequent phases, which in turn can cause delays, cancellations, and complications. Efforts to improve care transitions focused primarily on post-discharge care coordination and inpatient education for medically complex patients have not demonstrated consistent effects. This study aimed to understand the expectations and perceptions of postoperative inpatients regarding transition from ho

Impact of a scalable care transitions program for readmission avoidance.

OBJECTIVES: To evaluate the Care Transition Solution (CTS) as a means to improve quality through reduction of preventable hospital readmissions among patients with readmission-sensitive conditions subject to penalties imposed by the Affordable Care Act. STUDY DESIGN: A retrospective quasi-experimental evaluation of the impact of the CTS among admitted patients diagnosed with heart failure, acute myocardial infarction, chronic obstructive pulmonary disease, and/or pneumonia (CMS readmission-penalty diagnoses) in 14 acute care hospitals in Texas. The program, designed for scalable delivery, inc

Health care transition for youth living with HIV/AIDS.

There are ~1 million people in the United States living with HIV/AIDS, and >50,000 new infections occur each year. With an estimated 13% of all new infections occurring among young people aged 13 to 24 years and an increasing number of perinatally infected youth surviving to adulthood, there is now an increasing need to transition both perinatally and behaviorally infected youth to the adult health care setting. Recently, pediatric providers and professional societies have prioritized the development of transition programs for adolescents with chronic disease to address the many challenges the

Transition in care in persons with antidepressant prescription in naturalistic conditions.

BACKGROUND: Few studies have explored transition to psychiatric care in persons treated by antidepressants in primary care. OBJECTIVE: To assess the rates of transition in care after a dispensing of antidepressant and the characteristics associated with transition from primary to psychiatric care. METHODS: A follow-up study of persons (n = 19 975) starting antidepressant treatment was carried out on a representative sample of the 2005-07 French Social Security Insurance national database. Prescribers were categorized as GPs, hospital practitioners (as a proxy for public psychiatrist), privat

[The importance of comprehensive transition of care in women with Turner's syndrome].

Turner's syndrome is the most common chromosomal abnormality in females; it affects 1:2000 live female births. It is the result of the complete or partial absence of one X chromosome. The most frequently occurring clinical features are short stature and ovarian failure. Adults with Turner's syndrome have a reduced life expectancy, mainly due to an increased cardiovascular risk. Other morbidities include hypothyroidism, skeletal problems, renal and gastrointestinal disease, and hearing loss. While most females with Turner's syndrome present during childhood and adolescence, one quarter are diag

Transitions between care provisions demanded by Dutch elderly.

UNLABELLED: We follow people who are considered eligible for an elderly care provision. Through the estimation of a duration model we determine when they can be expected to express demand for another care provision. Data are drawn from the registrations of needs assessment agencies in The Netherlands. MAIN FINDINGS: Age positively effects the probability of a reassessment if initially care at home was assessed. Living alone, access to informal care, and applications initiated by professionals increase transition probabilities. Also initial applications for care at home due to dementia are mor

Balancing Personal and Professional Obligations Across the Transitions of Care.

PURPOSE/OBJECTIVES: To illustrate a critical paradox: health care professionals, and especially case managers, possess knowledge of regulatory requirements, care processes, and available resources that enable them to navigate complex systems more effectively than patients and families without this expertise. Yet, patients should not need a health care insider, professional credentials, or personal relationships within a health system to receive comprehensive, person-centered case management. PRIMARY PRACTICE SETTINGS: Across all settings. FINDINGS/CONCLUSIONS: The lived experience illustrate
