Occipital Screw Placement Using a Navigation System for a Pathological Odontoid Fracture With a Dural Venous Sinus Variation.

Conventional fluoroscopic guidance can provide enough information to precisely insert an occipital screw in ordinary cases. However, the occipital screw creates a potential risk of dural venous sinus injury or thrombosis. In some cases, with dural sinus variation, surgeons must especially be cautious to avoid its injury. We present a rare case of proper occipital screw placement using a navigation system for a pathological odontoid fracture with a high risk of dural venous sinus injury because of anatomical variations in the transverse and occipital sinuses. A 60-year-old man who underwent thy

Bilateral proptosis, dilatation of conjunctival veins, and papilloedema: a neuro-ophthalmological syndrome caused by arteriovenous malformation of the torcular herophili.

A patient with an occipital dural arteriovenous malformation (AVM) developed signs of a carotid-cavernous sinus fistula and raised intracranial pressure. Bilateral transverse sinus occlusion associated with the AVM produced these signs by rerouting intracranial venous drainage anteriorly through the cavernous sinuses and superior ophthalmic veins. Angiography and computerised tomographic reformation techniques were used to define these extraordinary cranio-orbital venous pathways.

Paranasal sinuses in children: size evaluation of maxillary, sphenoid, and frontal sinuses by magnetic resonance imaging and proposal of volume index percentile curves.

Our objective was to establish the age-related 3D size of maxillary, sphenoid, and frontal sinuses. A total of 179 magnetic resonance imaging (MRI) of children under 17 years (76 females, 103 males) were included and sinuses were measured in the three axes. Maxillary sinuses measured at birth (mean+/-standard deviation) 7.3+/-2.7 mm length (or antero-posterior)/4.0+/-0.9 mm height (or cranio-caudal)/2.7+/-0.8 mm width (or transverse). At 16 years old, maxillary sinus measured 38.8+/-3.5 mm/36.3+/-6.2 mm/27.5+/-4.2 mm. Sphenoid sinus pneumatization starts in the third year of life after convers

Supracerebellar infratentorial resection of a torcular lesion causing fulminant intracranial hypertension: illustrative case.

BACKGROUND: Venous sinus stenosis has been implicated in intracranial hypertension and can lead to papilledema and blindness. The authors report the unique case of a cerebellar transtentorial lesion resulting in venous sinus stenosis in the torcula and bilateral transverse sinuses that underwent resection. OBSERVATIONS: A 5-year-old male presented with subacute vision loss and bilateral papilledema. Imaging demonstrated a lesion causing mass effect on the torcula/transverse sinuses and findings of increased intracranial pressure (ICP). A lumbar puncture confirmed elevated pressure, and the pa…

[A case of bleeding dural arteriovenous fistula of the transverse-sigmoid sinus with pure leptomeningeal drainage, varix: case report].

We reported a case of bleeding from the varix of a transverse-sigmoid sinus dural arteriovenous fistula with pure leptomeningeal drainage. A 55-year-old man presented with visual disturbance. neurological examination revealed left homonymous hemianopsia. CT scans demonstrated a subcortical hemorrhage in the right occipital lobe. Right external carotid angiograms showed a dural arteriovenous fistula of the transverse-sigmoid sinus fed by middle meningeal arteries and the occipital artery. The draining vein was the cortical vein with a varix. Transverse and sigmoid sinuses were patent. We conclu

Pediatric Cerebral Venous Sinus Thrombosis: A Case Report.

Cerebral venous sinus thrombosis (CVST) in infants is a rare vascular disorder that presents with nonspecific symptoms leading to a delay in diagnosis and treatment. Thrombus formation in the cerebral sinuses prevents blood from draining out of the brain leading to local and systemic complications. Here, we present an 11-week-old patient who presented to the emergency department (ED) with three days of lethargy, multiple episodes of projectile emesis, increased fussiness, and downward gaze. A CT scan demonstrated intraventricular hemorrhage (IVH) with acute hydrocephalus. A CT venogram of the

Spontaneous closure of transverse sinus dural arteriovenous fistula: case report.

A 60-year-old man presented with transverse sinus dural arteriovenous fistula (AVF) manifesting as sudden onset of headache and nausea, which underwent spontaneous closure 5 years after the onset. Computed tomography on admission revealed small intraventricular hemorrhage in the right lateral ventricle. No intracranial vascular lesion was detected and magnetic resonance angiography was used at yearly follow up. Two years after the first admission, he suffered diplopia and cerebral angiography revealed transverse sinus dural AVF. Right pulsatile tinnitus occurred 4 years after the first admissi

Differentiation of transverse sinus thrombosis from congenitally atretic cerebral transverse sinus with CT.

BACKGROUND AND PURPOSE: Transverse sinus thrombosis can have nonspecific clinical and radiographic signs. We hypothesized that the novel "sigmoid notch sign" (on head CT) can help differentiate transverse sinus thrombosis from a congenitally atretic sinus among individuals with absent signal in 1 transverse sinus by MR venography. METHODS: We retrospectively evaluated 53 subjects with a unilaterally absent transverse sinus signal on MR venography. Eleven had true transverse sinus thrombosis and 42 had an atretic transverse sinus. Reviewers were trained in the sigmoid notch sign: "positive" if

Anatomy of the dura mater venous sinus of Alouatta belzebul.

The anatomy of the dura mater's venous sinuses is important in the veterinary clinical and surgical area, for cranial procedures and interventions of wild animals, in addition to assisting in the management of neurological diseases, which can prevent serious complications. The macroscopic anatomy of the venous sinuses of the dura mater of Alouatta belzebul was studied. Five adult specimens, males and females, were dissected and fixed in 10% formaldehyde solution. In the dura mater of the Alouatta belzebul, nine venous sinuses were observed, being them dorsal sagittal sinus, ventral sagittal si

Precise Onyx embolization of dorsum sellae sinus dural arteriovenous fistula via unilateral inferior petrosal sinus approach: illustrative case.

BACKGROUND: The dorsum sellae sinus is a large transverse venous channel on the dorsum sellae, communicating with the bilateral cavernous sinuses and inferior petrosal sinuses. However, dural arteriovenous fistulas (DAVFs) of the dorsum sellae sinus have been rarely reported. OBSERVATIONS: A 55-year-old male presented with conjunctival edema and a foreign body sensation in the left eye. Through cerebral angiography, the patient was diagnosed with DAVF of the dorsum sellae sinus. The DAVF was successfully treated by embolizing the dorsum sellae sinus with Onyx via the inferior petrosal sinus a

The pharyngovertebral veins: an anatomical rationale for Grisel's syndrome.

UNLABELLED: Non-traumatic subluxation of the atlanto-axial joint following peripharyngeal inflammation (Grisel's syndrome) has been attributed to laxity of the transverse ligament caused by hyperemia, but a satisfactory anatomical rationale for the association of these conditions has been lacking. Injection of the cervical epidural sinuses of a series of human perinatal cadavera showed retrograde filling of a previously undescribed system of veins with frequent lymphovenous anastomoses. This system appears to drain the posterosuperior pharyngeal region. CLINICAL RELEVANCE: Because of their di

MDCT Venographic Patterns of Dural Venous Sinus Compromise After Acute Skull Fracture.

OBJECTIVE: Prior studies have shown that skull fractures overlying the dural venous sinuses predispose the patient to an increased risk of dural venous sinus thrombosis (DVST). However, extrinsic compression may also cause sinus compromise and simulate thrombosis. This study set out to evaluate the prevalence and discernibility of DVST versus direct sinus compression in the setting of an overlying skull fracture. MATERIALS AND METHODS: All initial head MDCT venography examinations performed at a level 1 trauma center over an 8-year period were reviewed (n = 347 patients). The examinations tha

Infratentorial developmental venous anomaly concurrent with a cavernoma and dural arteriovenous fistula.

OBJECTIVES: While developmental venous anomaly (DVA) may be associated with cavernous malformation, mixed vascular malformation associated with dural arteriovenous fistula (dAVF) has not been previously reported. We observed a case with rare association of infratentorial DVA, cavernous malformation, and dAVF that presented with cerebellar ataxia. We report our endovascular treatment for this complex cerebrovascular condition. CASE PRESENTATION: A 32-year-old woman with ataxia had an infratentorial DVA associated with a cavernoma and dAVF. The dAVF had two shunting points. The dAVF was fed by

Hydroacoustic analysis and extraluminal compression surgical insights of venous pulsatile tinnitus.

OBJECTIVE: This study aimed to quantitatively and qualitatively evaluate the hydroacoustic changes from "presence" to "disappearance" of pulsatile tinnitus (PT) with the extraluminal compression surgical technique. The recent issues of concern pertaining to the hydroacoustic characteristics of sigmoid sinus wall anomalies and distal transverse sinus stenosis (dTSS) were discussed. METHODS: This study was based on a retrospective case series. Seventy-seven patients with PT and transverse-sigmoid sinus enlargement with or without transverse-sigmoid sinus junction anomalies and transverse sinus

The cerebral venous system.

The authors discuss the gross and microscopic anatomy and the physiology of the cerebral venous system. Cerebral veins under pathological circumstances (hypercapnia, arterial hypertension, and increased intracranial pressure), pharmacological observations, the venous blood-brain barrier, and traumatic involvement are reviewed. Neoplastic involvement and radiological aspects are included. Surgical reconstruction of venous sinuses (including the Donaghy technique), tumor removal, sinus thrombectomy, and extraanatomical bypass of the transverse sinus are discussed.
