Unusual forehead tremor in four patients with essential tremor.

Forehead tremor has only been reported in two patients with essential tremor, one with rhythmic tremor and the other with dystonic tremor. We report 4 new patients with essential tremor who present a 4-6 Hz frontal tremor registered by electromyography and unusual features like frontal tremor preceding limb tremor or unilateral involvement. Frontal tremor is present in some patients with essential tremor, sometimes preceding limb tremor. Treatment with botulinum toxin may be useful.

Prevalence of unilateral tremor in autosomal dominant essential tremor.

The presence of bilateral arm tremor is a key diagnostic feature of essential tremor (ET). We analyzed the presence of unilateral arm tremor in familial ET cohort of 133 autosomal dominant ET kindreds with 412 affected individuals. Inclusion criteria in patients with unilateral arm postural and/or kinetic tremor required the duration of tremor for at least 5 years, without hypokinetic-rigid syndrome, dystonic posturing, or history of sudden onset of tremor. Only subjects with at least one living first degree relative who met diagnostic criteria for definite ET were included. Eighteen subjects …

Cortical tremor (FCMTE: familial cortical myoclonic tremor with epilepsy).

For 15 years, 50 Japanese and European families with cortical myoclonic tremor and epilepsy were reported in the literature under various names. More recently, the acronym familial cortical myoclonic tremor with epilepsy (FCMTE) has been proposed for this new clinical entity based on both clinical and electrophysiological criteria: irregular postural myoclonic tremor of the distal limbs, familial history of epilepsy, autosomal dominant inheritance, and a rather benign outcome. The diagnosis is confirmed by electrophysiological features favoring cortical reflex myoclonus (enhanced C reflex at r…

Pathophysiology of Cerebellar Tremor: The Forward Model-Related Tremor and the Inferior Olive Oscillation-Related Tremor.

Lesions in the Guillain-Mollaret (G-M) triangle frequently cause various types of tremors or tremor-like movements. Nevertheless, we know relatively little about their generation mechanisms. The deep cerebellar nuclei (DCN), which is a primary node of the triangle, has two main output paths: the primary excitatory path to the thalamus, the red nucleus (RN), and other brain stem nuclei, and the secondary inhibitory path to the inferior olive (IO). The inhibitory path contributes to the dentato-olivo-cerebellar loop (the short loop), while the excitatory path contributes to the cerebrocerebellar…

Magnetic resonance support vector machine discriminates essential tremor with rest tremor from tremor-dominant Parkinson disease.

BACKGROUND: The aim of the current study was to distinguish patients who had tremor-dominant Parkinson's disease (tPD) from those who had essential tremor with rest tremor (rET). METHODS: We combined voxel-based morphometry-derived gray matter and white matter volumes and diffusion tensor imaging-derived mean diffusivity and fractional anisotropy in a support vector machine (SVM) to evaluate 15 patients with rET and 15 patients with tPD. Dopamine transporter single-photon emission computed tomography imaging was used as ground truth. RESULTS: SVM classification of individual patients showed …

Prevalence of asymptomatic tremor in relatives of patients with essential tremor.

BACKGROUND: The extent to which essential tremor (ET) clusters within families (ie, the familial aggregation of ET) is not precisely known. In part, this is because studies assign disease status in relatives of patients with ET based solely on interviews without conducting physical examinations. This may lead to underascertainment of affected relatives with mild asymptomatic ET. OBJECTIVE: To determine the prevalence of asymptomatic ET among relatives of patients with ET. METHODS: Interview and examination of 25 patients with ET and 58 of their relatives. The interview included 12 questions …

Differential Diagnosis of Parkinson Disease, Essential Tremor, and Enhanced Physiological Tremor with the Tremor Analysis of EMG.

We investigate the differential diagnostic value of tremor analysis of EMG on Parkinson's disease (PD), essential tremor (ET), and enhanced physiological tremor (EPT). Clinical data from 25 patients with PD, 20 patients with ET, and 20 patients with EPT were collected. The tremor frequency and muscle contraction pattern of the resting, posture, and 500 g and 1000 g overload were recorded. The frequency of PD tremor was 4-6 Hz, and the frequency of ET was also in this range; the frequency of EPT is 6-12 hz having some overlap with PD. The muscle contraction patterns of the ET and EPT group were…

[The problem of pharmacoresistant tremor in Parkinson's disease and essential tremor].

MR-guided focused ultrasound (MRg-FUS) is a new noninvasive method for the treatment of contralateral disabling and pharmacoresistant tremor. Clinical studies have confirmed the high efficacy and safety of using MRg-FUS in patients with essential tremor and Parkinson's disease, in short and long-term studies. Advantages of this method in comparison with currently used invasive and noninvasive technics, potential brain target areas, the possibility of bilateral intervention, indications and contraindications are discussed.

Rare tremors and tremors occurring in other neurological disorders.

Although tremor is deemed to be the commonest movement disorder, in adults the differential diagnosis usually boils down to whether the patient has Essential Tremor or Parkinson's Disease, which has likely led to an overdiagnosis of these conditions; yet, many important rare syndromes should be considered in the differential diagnosis of patients with tremor. The aim of this review is to focus on rare forms of tremor, also in view of the new tremor classification, as well as on tremor occurring in other neurological disorders to aid their recognition. Some of the conditions reviewed here are t…

Tremor Distribution and the Variable Clinical Presentation of Essential Tremor.

In addition to having postural and kinetic tremor of the upper limbs, some patients with essential tremor (ET) may have head tremor as well as cognitive and psychiatric disorders. We aimed to investigate whether the variable clinical presentation in ET patients, including motor and non-motor symptoms, differs in patients with and without head tremor. We consecutively enrolled 70 patients with a diagnosis of ET. Tremor severity was assessed by means of clinical rating scales. Patients also underwent kinematic recordings of postural and kinetic tremor of the upper limbs based on an optoelectroni…

Intermittent cortical involvement in the preservation of tremor in essential tremor.

Cortical involvement in essential tremor, an involuntary action tremor supposedly of subcortical origin, is uncertain. Conflicting results of corticomuscular coherence studies in essential tremor suggest an intermittent corticomuscular coupling. On the basis of the literature, we hypothesized that corticomuscular coupling is influenced by bilateral motor synchronization and "cognitive states" such as awareness of tremor. In the present study, we investigated 1) the existence of intermittent corticomuscular coherence (CMC) in essential tremor and 2) factors that influence CMC strength. In 18 es…

Electrophysiologic characteristics of tremor in Parkinson's disease and essential tremor.

UNLABELLED: Tremor in essential tremor (ET) and Parkinson's disease (PD) usually present specific electrophysiologic profiles, however amplitude and frequency may have wide variations. OBJECTIVE: To present the electrophysiologic findings in PD and ET. METHOD: Patients were assessed at rest, with posture and action. Seventeen patients with ET and 62 with PD were included. PD cases were clustered into three groups: predominant rest tremor; tremor with similar intensity at rest, posture and during kinetic task; and predominant kinetic tremor. RESULTS: Patients with PD presented tremors with a…

Is action tremor in Parkinson's disease related to resting tremor?

OBJECTIVE: Action tremor (AT) and resting tremor (RT) have been widely cited in many studies with Parkinson's disease (PD) patients, but studies looking at the association between the two tremor types are few and show inconsistent results. This study will look at the prevalence and association of AT and RT in a large sample of idiopathic PD patients, and will put the results into context with the literature. METHODS: A retrospective chart review analysis of 332 patients with idiopathic PD was performed. Prevalence rates of particular tremor types were noted. The presence of AT was analyzed re…

[Diagnosis and treatment of tremor in Parkinson's disease and essential tremor].

For patients presenting predominantly or purely with tremor, the correct diagnosis of tremor-dominant Parkinson's disease (PD) versus essential tremor (ET) is very important for prognosis and effective therapy. ET tremor is usually characterized by symmetric bilateral postural and kinetic tremor, which may respond to low alcohol consumption. Many patients have a family history of ET tremors. Medical treatment with primidone or beta-blockers effectively controls ET tremor, but in many cases no treatment is needed at all. The typical tremor form of PD is an asymmetric rest tremor, which is treat…

Effect of stimulation frequency on tremor suppression in essential tremor.

We sought to determine the effect of deep brain stimulation (DBS) frequency on tremor suppression in essential tremor (ET) patients with deep brain stimulators implanted in the ventral intermediate nucleus (VIM) of the thalamus. A uniaxial accelerometer was used to measure tremor in the right upper extremity of subjects with a diagnosis of ET who had DBS electrodes implanted in the left VIM. The root-mean-square acceleration was used as the index of tremor magnitude and normalized to the OFF DBS condition. There was a highly significant inverse sigmoidal relationship between stimulation freque…

Association Between Tremor Severity and Caregiving Intensity in Essential Tremor.

BACKGROUND: Essential tremor (ET) is a common movement disorder that affects the upper limbs in vital activities of daily living. Few studies have examined the impact of tremor severity on caregiving intensity. METHODS: Clinic-based data were collected in the United States (US) from March 2021 to August 2021 through the Adelphi ET Disease Specific Programme (DSP). Linked data between physician and care partner/patient pairs were used to evaluate care partner-reported weekly hours of patient care. Tremor severity was assessed with the Essential Tremor Rating Assessment Scale (TETRAS) Performan…
