[The frequency profiles of size of two-step tuberculin skin tests with histories of BCG vaccinations and tuberculin tests among hospital employees].

In 1998, Prevention Committee of the Japanese Society for Tuberculosis announced guidelines for dealing with nosocomial tuberculosis infections. These guidelines recommended the two-step tuberculin tests (TST) as baseline data for each medical employee when they started to work. If accurate records of previous TSTs are available in addition to baseline data, they are useful to evaluate the presence of tuberculosis infection when they started to work. We therefore studied the frequency profile of size of TST among medical employees in INBA-HITEC Medical Center and discussed methods to improve i…

Tuberculin reactivity in first-year schoolchildren in Madagascar.

OBJECTIVE: The tuberculin skin test (TST) is an important tool in the diagnosis of tuberculosis infection in children. However, the interpretation of TST may be complicated by prior Bacillus Calmette-Guerin (BCG) vaccination. We evaluated the effect of vaccination with BCG on TST reactivity in first-year pupils attending state schools in Antananarivo. METHODS: STs were performed on 376 first-year schoolchildren, aged 6 and 7, attending two state primary schools. The relationships between epidemiological information, BCG status (vaccination, BCG scars) and TST reactivity were assessed to compa…

Characteristics of children with positive tuberculin skin test.

The aim of the study was to define the characteristics of children with latent tuberculosis diagnosed with positive tuberculin skin test (TST) and evaluate potential risk factors in children with positive TST. Children followed with the diagnosis of latent tuberculosis infection were included in the study retrospectively. Demographic characteristics of patients including history of atopy, respiratory infections, family history of tuberculosis and atopy, number of BCG vaccinations, findings of physical examination and laboratory data were extracted from patient's file. Eighty-one children (51 m…

Tuberculin reactivity in healthy school children in Pondicherry.

Five hundred healthy school children in the age group 5-9 years underwent tuberculin testing. Seventy nine per cent of the study population had a BCG scar. 9.2% children were found to be suffering from malnutrition, using the Quetlet's Index and 7.8% were found to be malnourished by weight for age method. 18.6% of the children were tuberculin positive (induration > or = 10 mm) and the prevalence of tuberculin positivity increased with age. The numbers of tuberculin reactors or the mean tuberculin reaction were not affected by malnutrition. BCG scar status neither affected the mean tuberculin s…

Transfer in vivo of tuberculin hypersensitivity by sensitised lymphocytes.

An account is given of the experimental in vivo transfer of PPD-specific delayed hypersensitivity with a very small number of autologous lymphocytes pre-sensitised with PPD-specific transfer factor (TF). Transfer was assessed in 17 volunteers both in vivo by means of the Mantoux skin reaction, and in vitro by means of the leukocyte migration inhibition test, the lymphocyte locomotion test, and the leukocyte adherence inhibition test. Positivization of the tests suggests that TF triggers a reaction which expands the effect of hypersensitivity.

[Detection of a pathological cutaneous reaction to tuberculin in an adolescent].

The diagnosis of tuberculosis infection is based on a pathological dermal induration (positive Mantoux reaction) after injection of a standard dose of tuberculin (2 U). The positivity criteria of the Mantoux reaction vary according to whether the adolescent was or was not given BCG vaccination in the past. If the intradermal reaction is pathological at the time of the school screening visit, the subject should be referred to his physician, who will perform a second test (control Mantoux). If the pathological reaction is not confirmed, no further investigations will be needed. If the positive M…

Giant reactions to tuberculin in lepromatous leprosy patients.

A detailed account and definition is given of the previously inadequately described "giant reactions" to tuberculin occasionally seen in leprosy patients. The reaction is an accelerated and exaggerated response to species-specific antigens of Mycobacterium tuberculosis found in both PPD and New tuberculin. Our studies were performed in Malaysia, Uganda, Spain, and England. There was a significantly higher incidence of the phenomenon in Malaysia than in the other centers, but this may have been because there alone previously untreated lepromatous (LL and BL) patients were serially tested for up…

The adjuvant effect of tuberculin in experimental sensitization to merthiolate.

When guinea pigs were sensitized to tuberculin by exposure to complete Freund's adjuvant the size of the intradermal PPD test was positively correlated to the body weight of the animal. Old Tuberculin showed stimulating effect in a subsequent sensitization to merthiolate (thimerosal). The high frequency of delayed cutaneous allergy to merthiolate in Swedish population is probably explained by sensitization through intradermal testing with tuberculin containing merthiolate as a preservative.
