Overview of vaccines.

This article lists the vaccines current available for the control of both viral and bacterial infections. They may be attenuated live or inactivated whole microorganisms, or subunit preparations. Many more are in the pipeline and increasing attention is being given to establishing their safety before registration. Following the earlier eradication of smallpox, good progress is now being made toward the global eradication of poliomyelitis and a new program to eliminate measles from the Americas has begun. A variety of new approaches to vaccine development is now available. The hepatitis B virus…

[Anti-typhoid vaccines].

There are three commercially available vaccines against typhoid fever: parenteral inactivated whole cell vaccine, oral live attenuated (strain ty21a), parenteral polysaccharide vaccine (capsular antigen-Vi). Their protective efficacy is 50 to 88% (inactivated vaccine), 50 to 96% (live attenuated vaccine) and 75 to 80% (polysaccharide vaccine). The frequency of notable adverse reactions with the inactivated vaccine justifies the use of the new vaccines.

Salmonella-based vaccines.

There continues to be considerable interest in the development of a safe, effective, live, oral vaccine to combat typhoid fever of humans. Such a vaccine may be a derivative of the causative agent of the disease, Salmonella typhi. The prototype of such a vaccine, Ty21a, is not ideal, but no replacement for Ty21a is yet obvious. The construction and trial of bivalent vaccines, in which an attenuated Salmonella strain expresses determinants from another pathogen, awaits the development of a suitably attenuated derivative. In parallel with vaccine development programmes, a variety of techniques h…

[Preparation of vaccines].

A historical review on the preparation and use of vaccines is presented. The example of smallpox vaccination in man is interesting, as it affords an insight into the development of the principle of vaccination, the improvement of the vaccine and the eradication of the disease. Moreover, the various stages of vaccinology were described. Initially, vaccines were prepared on the basis of the attenuated or inactivated pathogens. After the introduction of animal cell culture techniques among others it became possible to realise an amplification of the preparation of virus vaccines. Since the introd…

New cholera vaccines.

Development of improved cholera vaccines has progressed rapidly in recent years. An oral killed vaccine, designed to evoke antibacterial as well as antitoxic intestinal immunity, has proved to be completely safe and to protect against cholera for at least 3 years. This vaccine also confers substantial though more short-lasting immunity against diarrhoea caused by enterotoxigenic Escherichia coli. Significant progress has also been made towards developing a live attenuated cholera vaccine using recombinant DNA techniques. The advent of effective oral cholera vaccines gives hope for improved con…

[Modern bacterial vaccines].

A well founded vaccination strategy is of crucial importance for controlling communicable diseases. The WHO Enlarged Vaccination Programme of 1976 provides to protect by vaccination all children in the world against six infection diseases - diphtheria, pertussis, tetanus, measles, poliomyelitis, and tuberculosis, a sure immunization prophylaxis being possible against them. The perspectives of further development of some classical vaccines as against cholera, typhoid fever, and pertussis are reviewed. Some recent bacterial vaccines are discussed being of special significance for controlling nos…

Vaccines against monkeypox.

The monkeypox virus is a virus that has 90% genomic homology with the human (smallpox), but it is naturally transmitted between different wild animal reservoirs and is considered a zoonosis. Throughout the 20th century, different vaccines based on the vaccinia poxvirus were developed and used for vaccination against smallpox. After the eradication of smallpox, these vaccines were no longer used. Current vaccines against monkeypox virus are classified by the WHO as replicative (ACAM2000), minimally replicative (LC16m8) and non-replicative (MVA-BN), the latter being the one currently used. The 2…
