PURPOSE OF REVIEW: To evaluate the role of varicocelectomy in the management of patients with varicoceles and nonobstructive azoospermia and to review predictors of successful outcomes.

PURPOSE OF REVIEW: Several recent clinical studies have added to the existing literature on the impact of varicocelectomy on serum testosterone levels. Spontaneous thrombosis of a varicocele is a rare event and difficult to diagnose, as the clinical symptoms during the acute phase can simulate torsion of the spermatic cord or strangulated inguinal hernia leading to a useless surgical exploration. The authors report a case of spontaneous thrombosis of a varicocele which was diagnosed clinically, allowing conservative medical treatment.

Seven target veins (in 5 dogs) were embolized with nitinol conical rugby embolus (NT-CRE). The pathological examination showed firm impact between NT-CRE and intravenous membrane without misembolization from retrograde flow. Compact collagenous fibrous tissue was seen in and out NT-CRE on spaces. On the basis of animal experiment, 10 patients with varicocele were treated with NT-CRE. After the operation, the varicocele and symptoms disappeared and venae spermatica collapsed. The follow-up for 1.5-2.3 years showed no recurrence in the 10 patients. The varicocele is benign pathology which sometimes involves disorders of testicular growth. Those can cause a hypofertility. We saw in a retrospective way the 243 patients having been treated by percutaneous embolisation from May 1985 to August 1993. At 150 patients being 1 year at least of postoperative retreat, 70% of cases present a cure, 15% of patients repeated and were taken again surgically. In a group of 31 patients with idiopathic varicocele (IV), testicular biopsy showed a decreased tubular diameter, hyperplasia in the number of Leydig's cells (LC; many with cytoplasmic vacuolization and atrophy) and a decrease in the number of positive LC in testicular tissue sections stained with the testosterone peroxidase-antiperoxidase method. Varicocele is a common, but often overlooked, disorder. It appears in pre- and para-pubertal boys as well as adults. Between 1954 and 1982 23 boys with varicocele were referred to Alder Hey Children's Hospital. This small number (less than a patient a year) does not represent the true incidence of varicocele in the community as is evident from a review of the world literature. A man with a left varicocele and the physical finding of deep and superficial scrotal varicosities is described. The recurrence of these veins after high retroperitoneal ligation was the first indication of recollateralization. A review of the early literature reveals that varicocele refers to the varices of the scrotum and cirsocele (earlier term to denote the physical finding of scrotal varicosities) applies to the varices of the testicle and cord. It appears that these may be the same.

As men age, serum testosterone levels decrease. This reduction in testosterone levels may cause symptoms leading to hypogonadism. In addition, the presence of a varicocele can impair Leydig cell function, resulting in decreased testosterone production in men. Varicocelectomy and testosterone replacement therapy are both viable treatment options for hypogonadism in elderly male patients. A method has been developed which uses a radionucleotide to demonstrate retrograde bloodflow in the testicular vein and the size of the pampiniform plexus in patients with a varicocele. This method and phlebography were both performed in 71 patients. INTRODUCTION: Among the different options for varicocele surgery, microsurgical varicocelectomy demonstrates the best results, but a relapse is subsequently detected in 1-3% of cases. It was previously believed that the cause of recurrence lies in the presence of various sources of venous outflow from the testicle, but recent studies show that the collaterals of the gonadal vein are the main reason of recurrence.
